Osteoporoz, düşük kemik kütlesi ve kemik dokusunun mikro yapısının bozulması sonucu kemik kırılganlığının ve kırık olasılığının artması ile karakterize sistemik bir iskelet hastalığıdır. Antikoagülan olarak kullanılan heparinin kemik kaybı ve osteoporoza yol açtığı bilinmektedir. Kemik iyileşmesinin hızlandırılması için literatürde pek çok farklı yöntem araştırılmaktadır. Girişimsel olmayan ve düşük komplikasyon oranına sahip bir tedavi şekli olan darbeli elektromanyetik alan uygulamasının kemik oluşumunu uyardığı bilinmektedir. Bu çalışmada, heparin ilişkili sekonder osteoporoz oluşturulmuş ratlarda, darbeli elektromanyetik alan uygulamasının kemik defektlerinin iyileşmesi üzerine etkilerinin histomorfometrik ve biyokimyasal olarak değerlendirilmesi amaçlanmıştır Prospektif, randomize, tek kör çalışmada 12 aylık Sprague-Dawley cinsi erkek ratlar; heparin + PEMF grubu, heparin grubu ve kontrol grubu olmak üzere üç gruba ayrıldı. Heparin + PEMF grubu ve heparin grubu ratlara 33 gün boyunca 2 IU/g heparin verilerek osteoporoz oluşturuldu. 35. günde tüm ratların sağ femur kemiklerinde 3mm'lik defektler açıldı. Heparin + PEMF grubunda operasyonu takip eden 28 gün boyunca hergün 1 saat, ortalama 0.8 mT şiddetinde ve 7.3 Hz frekansında darbeli elektromanyetik alan uygulaması yapıldı. Kemik iyileşmesi 4. haftanın sonunda histomorfometrik ve biyokimyasal analizlerle değerlendirildi Histomorfometrik değerlendirmede en yüksek yeni kemik alanı heparin + PEMF grubunda tesbit edilmiştir. Heparin + PEMF grubunun yeni kemik oluşum alanı, heparin grubundan yüksektir ve bu fark istatistik olarak anlamlıdır (p=0,002). Biyokimyasal değerlendirmede gruplar arasında 63. günde CTx değerlerinin ortalaması heparin + PEMF grubunda en düşüktür ve bu fark istatistik olarak anlamlıdır (p<0,05). Osteoporosis is a systemic skeletal disorder and as a result of low bone mass and microarchitectural deterioration of bone tissue, characterised by increased bone fragility and the likelihood of sustaining fractures. Heparin, which is used as an anticoagulant, is known to result in bone loss and osteoporosis. Many different methods have been discussed in the literature to accelerate bone recovery. As a non-interventional mode of therapy associated with low complication rates, pulsed electromagnetic field therapy is known to stimulate bone formation. The aim of the present study was to evaluate the histomorphometric and biochemical effects of pulsed electromagnetic field therapy on the healing of bone defects in rats with heparin-induced bone defects. In this prospective, randomized, single-blind study, 12-month-old Sprague-Dawley male rats were divided into three groups as heparin + PEMF group, heparin group, and the control group. In order to produce osteoporosis, the rats in the heparin + PEMF and the heparin group received heparin 2 IU/G for 33 days. At day 35, a 3-mm defect was created in the right femur of the rats. The rats in heparin + PEMF group received pulsed elektromagnetic field therapy at 0.8 mT and 7.3 Hz frequency 1 hour/day for the subsequent 28 days. At the end of the 4th week, bone healing was evaluated using histomorphometric and biochemical analyses. Histomorphometric assessment revealed that the rats in heparin + PEMF group had the highest level of new bone. The level of new bone was higher in the heparin + MEPF group compared with the heparin group, and the difference was statistically significant (p=0.002). In biochemical assessment, average CTx values at day 63 were lower in the heparin + PEMF group, and the difference was statistically significant (p<0.05). The results of this study, showed that; the administration of heparin has been lead to bone loss and osteoporosis and the pulsed electromagnetic field theraphy has been usefull to reduce this negative effect.
