Sagittal ve vertikal yönde maloklüzyonu bulunan bireylerde kraniyofasiyal yapıların konumsal ve boyutsal açıdan değerlendirilmesi, vakanın hangi bölgelerde normalden saptığının bilinmesi açısından ve kompanzasyonun yetersiz kaldığı bu bölgelerde gelişimin ortodontik tedavi ile istenilen şekilde yönlendirilmesi açısından önemlidir. Bu çalışmada iki amaç hedeflenmiştir. Çalışmanın ilk aşamasında, ideal fonksiyonel oklüzyonun sağlanabilmesi için sagittal ve vertikal yön anomalisine sahip gruplar arası farklıklıklardan yola çıkarak kompanzasyonların nereden kaynaklandığının araştırılması amaçlanmıştır. Çalışmanın ikinci aşamasında ise, bu bireylerde ideal oklüzyona ulaşmak için dentoalveoler yapıların literatürde kabul görmüş norm değerlerine ne kadar yaklaştığını ya da bu değerlerden ne kadar uzaklaştığının incelenmesi amaçlanmıştır. Bu çalışma her bir alt grupta 15'er bireyin bulunduğu 9 grupta toplam 135 birey üzerinde yürütülmüştür. 135 birey sagittal yönde ANB açılarına göre İskeletsel Sınıf I, Sınıf II ve Sınıf III olmak üzere 3 ana gruba ayrılmıştır. Her bir ana gruptaki 45 birey de vertikal yönde SN/GoGn açılarına göre 15'er kişilik 3 alt gruba ayrılmıştır. Araştırma gruplarını oluşturan bireylerin tedavi öncesi lateral sefalometrik radyografileri üzerinde belirlenen parametreler doğrultusunda açısal ve milimetrik ölçümler yapılmıştır. Çalışmanın istatistiksel yöntemi olarak, gruplar arası karşılaştırmalarda varyans analizi (ANOVA) ve bu farkın hangi gruplar arasında olduğunu belirlemek içinse Duncan testi kullanılmıştır. Ayrıca Pearson korelasyon analizi ile de belirlenen parametreler arasındaki ilişki ortaya konmuştur. Çalışmamızın ilk aşamasında elde edilen bulgulara göre, tüm hiperdiverjan gruplarda birlikte palatal ve oklüzal düzlemin kraniyal kaide ile yaptığı açı ile birlikte çenelerarası açı artmıştır. Ayrıca simfiz yüksekliği ve simfiz iç eğim açısının (IdoBoMe) artmıştır. Simfiz iç yüzeyi daha düzdür. Hipodiverjan bireylerde ise simfiz iç yüzeyi daha eğimli ve kıvrımlı bir hal almıştır. İdeal overjet-overbite ilişkisine sahip Sınıf I, Sınıf II ve Sınıf III bireylerin tümünde keserlerarası açının korunduğu tespit edilmiştir. Çalışmamızın ikinci aşamasında elde edilen bulgulara göre, keserler arası açı Sınıf II bireylerin tümünde kontrol grubuna kıyasla azalmıştır, Sınıf III bireylerin tümünde ise kontrol grubuna kıyasla artmıştır. Kontrol grubuna kıyasla Sınıf II bireylerin tümünde üst keserler, kendi kaidelerine göre daha retrüziv, Sınıf III bireyler ise daha protruziv konumlanmıştır. Sınıf II hiperdiverjan bireyler hariç Sınıf II normodiverjan ve hipodiverjan bireylerde alt keserler, kontrol grubuna kıyasla kendi kaidelerine göre daha protrüziv konumlanmıştır. Sınıf III bireylerde ise kontrol grubuna kıyasla alt keserler daha retrüziv konumlanmıştır.
The dimensional evaluation of craniofacial structures in subjects with sagittal and vertical malocclusion is important for determining the areas in which the case has deviated from the norm. In these areas where compensation is insufficient, the development may be oriented as desired with orthodontic treatment. Two objectives were targeted in this study. In the first phase, it was aimed to examine how the craniofacial and dentoalveolar structures adapt in order to achieve optimal functional occlusion. In the second phase of the study, it is aimed to examine how close or far the dentoalveoler structures are from the norm values accepted in the literature, in order to achieve ideal occlusion in subjects with sagittal and vertical intermaxillary skeletal discrepancy. This study was carried out on a total of 135 individuals who have been divided into three main groups according to their sagittal ANB angles. 45 individuals in each main group were then divided into three subgroups of 15 subjects according to their vertical SN/GoGn angles. Angular and millimetric measurements were conducted in the direction of the parameters determined on the pre-treatment lateral cephalometric radiographs of the subjects constituting the research groups. As the statistical method of the study, ANOVA was used for intergroup comparisons and the Duncan test to determine to which groups these differences belong. In addition, the Pearson correlation analysis revealaed the relation between the determined parameters. According to the findings obtained at the first stage of our study, the angle between the palatal and occlusal plane and the cranial base, as well as the intermaxillar angle, increased in all hyperdivergent groups. There was also an increase in symphysis height and IdoBoMe angle and with the increase in this angle, the inner surface of the symphysis is flatter. In hypodivergent subjects however, the angle is decreased and the inner surface of the symphysis is rather curved. In all groups with an ideal overjet-overbite relationship, the interincisal angle was preserved regardless of the differences in vertical direction dimensions. In all groups, the extrusion of the upper and lower incisors with the increase of vertical dimension has caused an increase in maxillary and mandibular anterior dentoalveolar heights. According to the findings of the second phase of our study, the interincisal angle was reduced in all Class II subjects and increased in all Class III subjects in comparison with their respective control groups. In comparison with the control group in all of the Class II and Class III subjects, the position of upper and lower incisors are changed.
