Bu çalışmanın amacı, nanofil içerikli bir yüzey örtücü uygulanan ve uygulanmayan Sınıf V kompozit rezin restorasyonlarda, ev tipi vital ağartma sonrası kenar uyumunun ve mikrosızıntının in vitro olarak değerlendirilmesidir. Doksan adet yeni çekilmiş ön bölge insan dişi randomize olarak 3 gruba ayrıldı (n=30): Adeziv rezin ve kompozit rezin uygulanan grup (Grup 1, kontrol), adeziv rezin, kompozit rezin ve ev tipi ağatma ajanı uygulanan grup (Grup 2) ve adeziv rezin, kompozit rezin, yüzey örtücü ve ağatma ajanı uygulanan grup (Grup 3). Standart olarak açılan dişlerin bukkal yüzeylerinin orta üçlüsünde, tüm sınırları minede olan Sınıf V kaviteler iki basamaklı asitlenen ve yıkanan bir adeziv sistem (Prime and Bond NT, Dentsply, York, Pensilvanya, ABD) ve nanofil kompozit rezin (Clearfil Majesty ES-2, Kuraray, Okayama, Japonya) kullanılarak restore edildi. Grup 1'deki örneklere restorasyondan sonra hiçbir işlem yapılmadı. Grup 2'deki örneklere yüzey örtücü uygulanmadan ev tipi (Opelescence PF % 10'luk karbamit peroksit, Ultradent, South Jordan, UT, ABD) ağartma işlemi uygulandı. Grup 3'teki örneklere nanofil doldurucu içerikli yüzey örtücü (G-Coat Plus, GC, Tokyo, Japonya) uygulandıktan sonra ev tipi ağartma ajanı (Opalescence PF % 10 karbamit peroksit, Ultradent, South Jordan, UT, ABD) ile ağartma işlemi yapıldı. Tüm örnekler kenar uyumu analizi için X75-X2000 büyütmede taramalı elektron mikroskobunda incelendi. Mikrosızıntı değerlendirmesi için % 0.05'lik bazik fuksin kullanılarak boya penetrasyonu yöntemi uygulandı. Daha sonra örnekler stereomikroskopta incelendi. İstatiksel değerlendirmede Bağımsız İki Grup T-Testi ve Kruskal Wallis testi kullanıldı (p=0.05). Taramalı elektron mikroskobu sonuçlarına göre kenar aralanması yüzdeleri Grup 1'de 4.2±2.6, Grup 2'de 12.0±6.3; mükemmel kenar yüzdeleri ise, Grup 1'de 95.7±2.6, Grup 2'de 88.0±6.3 olarak bulundu. Grup 3'e ait örneklerde kavite kenarları izlenemediği için taramalı elektron mikroskobunda değerlendirme yapılamadı. Grup 2'nin kenar aralanmasıı değerleri, Grup 1'e göre istatistiksel olarak anlamlı derecede yüksek bulundu (Bağımsız İki Grup T-Testi, p<0.01). Grup 2'nin mükemmel kenar değerleri, Grup 1'e göre istatistiksel olarak anlamlı derecede düşük bulundu (Bağımsız İki Grup T-Testi, p<0.01). Boya penetrasyonu yöntemi ile yapılan mikrosızıntı testi sonucu stereomikroskopta incelenen örnekler iki farklı gözlemci tarafından değerlendirildi (Gözlemciler arası uyum yüzde 89.6, Kappa skoru 0.77). Mikrosızıntı testi değer ortalamaları Grup 1'de 134.08, Grup 2'de 197.24 ve Grup 3'de 75.18 olarak saptandı. Tüm gruplar arasındaki farklar istatistiksel olarak anlamlı bulundu (Kruskal-Wallis, p= 0.00). Bu çalışmada elde edilen bulgulara göre, kompozit rezin restorasyon bulunan dişlerde yapılan ev tipi vital ağartma işleminden önce, restorasyonlara nanofil yüzey örtücü uygulanmasının kenar uyumunu artırarak mikrosızıntıyı azalttığı sonucuna varılabilir.

The aim of this in vitro study was to analyse the marginal adaptation of class V composite restorations used with or without nano-filled surface coating agent after home bleaching treatment using a scanning electron microscope (SEM) and to evaluate microleakage via dye penetration method. Ninety fresh extracted human anterior teeth were randomly divided into three experimental groups (n=30): adhesive resin and composite resin applied group (Group 1, control); adhesive resin, composite resin applied and bleaching treated group (Group 2); adhesive resin, composite resin, surface coating agent applied and bleaching treated group (Group 3). Standart class V cavities were prepared on the middle third of the buccal surface and all margins located at enamel. Cavities were restorated with two steps etch and rinse adhesive system (Prime and Bond NT, Dentsply, York, Pensilvania, USA) and a nanofill composite resin (Clearfil Majesty ES-2, Kuraray, Okayama, Japan). No treatment was applied to specimens after the restoration in group 1. In group 2, home bleaching (Opelescence PF 10% carbamide peroxide, Ultradent, South Jordan, UT, USA) was applied to specimens without surface coating agent (G-Coat Plus, GC, Tokyo, Japan) application. In group 3, home bleaching was applied to specimens following the nano-filled surface coating agent. The examination of the restorations' margins were performed at X75-X2000 magnification using SEM. Microleakage test was applied using 0.05% basic fuchsin. The dye penetration degree of the sections were evaluated using stereomicroscope. Two Sample T-test and Kruskal-Wallis test were used for statistical analysis (p=0.05). According to scanning elecron microscobe results, the percentages of marginal gaps were determined as 4.2±2.6 for Group 1 and 12.0±6.3 for Group 2. The percentages of perfect margin was obtained as 95.7±2.6 for Group 1 and 88.0±6.3 for Group 2. Scanning electron microscobic assesment of the cavity margins in Group 3 could not performed due to the surface coating agent. There were significant differences between the groups in terms of the percentages of marginal gaps and the percentages of perfect margin (Two Sample T-test, p=0.00). The degree of dye penetration of specimens was evaluated by two researchers at stereomicroscope (aggrement between researchers 89.6%, Kappa score 0.77). Mean of rank values was determined as 134.08 for Group 1, 197.24 for Group 2 and 75.18 for Group 3. The differences between all groups was found statistically significant (Kruskal-Wallis, p= 0.00). Within the limits of this study; it might be concluded that application of the nano-filled surface coating agent to composite restoration margins before home bleaching treatment can reduce the microleakage due to preventing the degradation of marginal adaptation.
