Hemiplejik Omuz Ağrısında Klinik ve Ultrasonografik Bulgular Bu çalışmanın amacı, hemipleji sonrası farklı üst ekstremite motor iyileşme evrelerindeki hastaların klinik özelliklerini ve ultrasonografi yardımıyla rotator kaf problemlerini saptamak ve bu bulgularla hemiplejik omuz ağrısı arasındaki ilişkiyi değerlendirmektir. Çalışmaya hemipleji tanısı alan 48 hasta alındı. Çalışmaya alınan hastalar Brunnstrom üst ekstremite motor iyileşme evresine göre iki gruba ayrıldı. Brunnstrom iyileşme evresi 1-2-3 olanlar kötü üst ekstremite motor fonksiyon grubu (Grup 1), 4-5-6 olanlar iyi üst ekstremite motor fonksiyon grubu (Grup 2) olarak tanımlandı. Her iki gruba klinik ve ultrasonografik değerlendirme yapıldı. Ultrasonografik değerlendirmede hemiplejik omuzdaki biseps, supraspinatus, infraspinatus, subskapularis tendonları, subakromiyal subdeltoid (SASD) bursa ve akromiyoklavikular eklem incelendi. Hastaların hemiplejik omuz ağrısı şiddetini belirlemek için Numerik Derecelendirme Skalası (NDS) kullanıldı. Üst ekstremite motor fonksiyonunun belirlenmesinde Brunnstrom Motor Değerlendirme Ölçeği (BMDÖ) ve Fugl-Meyer Motor Değerlendirme Ölçeği (FMDÖ) kullanıldı. Hastaların günlük yaşam aktivitelerindeki bağımlılık düzeyi Barthel İndeksi (BI) ile değerlendirildi. Hastaların depresyon ve anksiyete semptomları Beck Depresyon Envanteri (BDE) ve Beck Anksiyete Envanteri (BAE) ile değerlendirildi. Gruplar arasında yaş, cinsiyet, VKİ (Vücut Kitle İndeksi) , hemipleji süresi, hemiplejik taraf, dominant el açısından anlamlı farklılık yoktu. Supraspinatus tendon (p=0.01) ve SASD bursa (p=0.03) patalojilerinin görülme sıklığı grup 1'de grup 2'ye göre anlamlı olarak yüksekti. Biseps, subskapularis, infraspinatus tendon patolojileri ve akromiyoklavikular eklem patolojileri açısından gruplar arasında istatistiksel olarak anlamlı fark yoktu. Hemiplejik omuz ağrısı (HOA) varlığı açısından gruplar arasında istatistiksel olarak anlamlı farklılık yoktu. Birinci grupta spastisite görülme sıklığı 2. gruba göre anlamlı olarak daha yüksekti (p=0.002). Spastisitesi olan hastalarda HOA görülme sıklığı spastisitesi olmayanlara göre istatistiksel anlamlı olarak daha yüksekti (p=0.04). NDS skorları ile omuz abduksiyon (r=-0.470, p<0.001) ve ER açıları (r=-0.411, p<0.001) arasında orta düzeyde anlamlı negatif korelasyon saptandı. Sonuç olarak, üst ekstremite motor iyileşme evresi kötü olan hemiplejik hastalarda supraspinatus tendon patolojileri ve bursal patolojiler daha sık görülmektedir. Spastisite varlığı hemiplejik hastalarda omuz ağrısı prevalansını ve rotator kaf yaralanması riskini artırmaktadır. Omuz ER ve abdüksiyon kısıtlılığı ile HOA arasında anlamlı ilişki olduğu saptanmıştır.

Clinical and Sonographic Findings in Hemiplegic Shoulder Pain The aim of this study is to evaluate clinical findings and rotator cuff problems of hemiplegic patients by ultrasonography at different motor recovery stages, and to evaluate correlation between clinical/sonographic findings and hemiplegic shoulder pain (HSP). Forty-eight patients with stroke were included in the study. Patients were assigned to groups according to the Brunnstrom recovery stages of hemiplegic upper extremity. Patients with stages 1-2-3 were defined as poor motor function group (Group 1, n=28), and those with stages 4-5-6 were defined as good motor function group (Group 2, n=20). Both groups were evaluated by clinical and ultrasonographic examination. US examination of hemiplegic shoulder included biceps tendon, supraspinatus tendon, infraspinatus tendon, subscapularis tendon, subacromial subdeltoid (SASD) bursa and acromioclavicular joint in all patients. Numeric Rating Scale (NRS) was used for evaluation of pain intensity. The level of motor function was assessed by Brunnstrom Motor Recovery Stages (BMR) and Fugl Meyer Motor Assesment Scale (FMA). Activities of daily living were assessed by Barthel Index (BI). Beck Depression Inventory (BDI) and Beck Anxiety Inventory (BAI) were used to determine depression and anxiety level. No statistically significant differences in age, sex, BMI (Body Mass Index), duration of hemiplegia, hemiplegic side, hand dominance were determined between the two groups. Abnormal findings of supraspinatus tendon (p=0.01) and SASD bursa (p=0.03) were more frequent in poor motor function group. No statistically significant differences in biceps, subscapularis, infraspinatus tendon findings and AC joint were determined. No statistically significant differences in HSP were determined between the two groups. Spasticity was more frequent in poor motor function group (p=0.002). The frequency of HSP was higher in patients with spasticity (p=0.04). There was a moderate negative corelation between NRS scores and shoulder abduction (r=-0.470, p<0.001) and external rotation (r=-0.411, p<0.001) ranges. In conclusion, supraspinatus tendon and SASD bursa abnormalities were more frequent in hemiplegic patients with poor motor function. Prevalance of HSP and rotator cuff injuries were increased with spasticity. There was a significant association between HSP and shoulder abduction and ER restriction.
