Çalışmanın amacı, total abdominal histerektomi geçiren hastalarda, epidural kataterden verilen bupivakaine eklenmiş deksmedetomidinin postoperatif aneljezik kullanımına etkilerini araştırmak. Bu çalışmada; operasyonu 150 dakikayı geçmeyen genel anestezi uygulanmış, operasyon öncesi postoperatif aneljezi sağlamak için epidural katater takılmış, cilt kapatılmaya başlayınca epidural kataterden bolus şekilde bupivakain veya bupivakain+deksmedetomidin yapılmış ve postoperatif epidural HKA cihazı takılmış, ASA I-II, 30-60 yaş arası hastaların verileri çalışmaya dahil edildi. Dosya kayıtları incelenen hastalar 2 gruba ayrıldı; Deksmedetomidin grubu (GRUP D): 30-60 yaş arası 5ml 0.5% bupivakain, 1ml deksmedetomidin (0.5μg/kg), 10ml 0.9% sodyum klorür kullanılan, Bupivakain grubu (GRUP B): 30-60 yaş arası 5ml 0.5% bupivakain, 11 ml 0.9% sodyum klorür kullanılan. Anestezi takip fişlerinden; demografik ve hemodinamik veriler, algoloji takip formlarından; analjezik talep sayıları, analjezik tüketimleri, VAS skorları, Ramsay sedasyon değerleri ve yan etkiler kaydedildi. Demografik veriler, hemodinamik parametreler, VAS skorları, Ramsay sedasyon değerleri ve yan etkiler açısından gruplar arasında fark bulunmadı. Postoperatif 30.dakikadan itibaren Grup D de, Grup B ye göre daha az analjezik talebi, daha az bupivakain ve morfin tüketimi olduğu görüldü ve istatiksel olarak anlamlı bulundu (p<0.05). Sonuç olarak postoperatif analjezi sağlamak amacıyla epidural bupivakaine eklenen 0,5 μg/kg deksmedetomidin; postoperatif 30. dakikadan itibaren analjezik talep sayısıyla birlikte morfin ve bupivakain tüketiminini azalttığı kanısındayız.

The aim of the study was to investigate the effects of epidurally admistrated dexmedetomidine + bupivacaine for postoperative analgesic use in patients undergoing total abdominal hysterectomy. In the study; ASA I-II patients, aged 30-60 years, under general anesthesia, whose operation was not exceed 150 minutes; epidural bupivacaine or bupivacain+dexmedetomidine were given via catheter when the skin was closed to provide postoperative analgesia; and patients who had postoperative epidural PCA device were included in the study. Patients whose file records are being examined are divided into two groups. Group Dexmedetomidine (Group D): 30-60 years of age, 5 ml 0.5% bupivacaine + 1 ml dexmedetomidine (0.5 μg / kg)+10 ml 0.9% sodium chloride, Group bupivakain (group B): 30-60 years of age 5 ml 0.5% bupivacaine + 11 ml 0.9% sodium chloride. According anesthesia forms; demographic and hemodynamic data, algology forms; analgesic demand rate, analgesic consumption, VAS scores, Ramsay sedation scales and side effects were recorded. There was no difference between the groups in terms of demographic data, hemodynamic parameters, VAS scores, Ramsay sedation scales and side effects. From the 30th minute post-operative, group D showed less analgesic demand, bupivacaine and morphine consumption than group B and statistically significant (p<0.05). As a result, we believe that 0.5 μg/kg dexmedetomidine added to the epidural bupivacaine; after the 30th minute post-operative, the consumption of morphine and bupivacaine with analgesic demand rate was decreased.
