Çalışmamızın amacı invaziv mide adenokarsinomlu hastalarda Her-2 ekspresyon sıklığının saptanması, klinikopatolojik özelliklerin değerlendirilmesi ve Her-2 ekspresyonunun progresyonsuz sağkalım ve genel sağkalım üzerine etkisini belirlemektir. Çalışmaya toplam 198 hasta dahil edildi. Her-2 sıklığı %18,2 olarak saptandı. Her-2 pozitif tümörlü hastaların yaş ortalaması 66,5, negatiflerin ise 62,6 idi. Her-2 pozitif grupta hastaların çoğu kadınken (%75), Her-2 negatif grupta hastaların çoğu erkekti (%64,8). Her iki grupta da sigara içmeyenler fazlaydı. Hem Her-2 pozitif hem de Her-2 negatif grupta tümör en sık antrum yerleşimliydi. İntestinal tip her iki grupta da sıklıkla rastlanan histolojik alt tipti. Lenfovasküler invazyon ve perinöral invazyonu pozitif tümörler her iki grupta da fazlaydı. Her-2 pozitif ve negatif hastalar tanı anında sıklıkla evre 3 ve evre 4 te idi. En sık metastaz bölgesi her iki grupta da karaciğerdi. Sistemik tedavi alan olgulara 5 FU tabanlı kemoterapi uygulanmıştı. Trastuzumab tabanlı kemoterapi sadece 4 olguda metastatik basamakta kullanılmıştı. Her-2 pozitif ve negatif grupta sırasıyla tüm popülasyonda medyan progresyonsuz sağkalım 7,1 ay, 10,3 ay; nonmetastatik olgularda 7,1 ay, 13,9 ay; metastatik olgularda ise 6,8 ay, 4,9 aydı. Her-2 pozitif ve negatif grupta sırasıyla tüm popülasyonda medyan genel sağkalım 23,4 ay, 15,5 ay; nonmetastatik olgularda 35,9 ay, 28 ay; metastatik alt grupta 11,8 ay, 5,1 aydı. Her-2 ile Lauren sınıflaması (p<0,01), histolojik grade (p<0,01) ve akciğer metastazı (p<0,05) arasında anlamlı ilişki saptanırken diğer parametrelerle anlamlı ilişki bulunmadı. Her-2 ile progresyonsuz sağkalım, genel sağkalım arasında ilişki saptanmadı. Her-2 ile karaciğer metastazının genel sağkalıma etkisi anlamlı saptandı (p<0,05).

The aim of our study is to find the frequency of stomach adenocarcinoma, also know as gastric cancer on patients to evaluate the clinicopathological features and to determine the pregression-free survival and overall survival of Her-2. A total of 198 patients were admitted in the study. Frequency of Her-2 was found to be %18,2. The mean age of the patients with Her-2 positive tumors was 66.5 and the negatives were 62.6. In the Her-2 positive group, most of the patients were female (75%) and most of the patients in the Her-2 negative group were male (64,8%). The people who don’t smoke are over in both groups. In both the Her-2 positive and Her-2 negative groups, the tumor was most frequently located in the antrum. Intestinal type which is found frequently in both groups was histological subtype. Lymphovascular invasion and perineural invasion positive tumors were more common in both groups. Her-2 positive and negative patients were often stage 3 and stage 4 at the time of diagnosis. The most frequent metastatic site was liver in both groups. Patients receiving systemic treatment received 5 FU based chemotherapy. Trastuzumab-based chemotherapy was used only in 4 metastatic stages. Median progression-free survival over the whole population in the Her-2 positive and negative groups, respectively, was 7.1 months, 10.3 months; in nonmetastatic cases 7,1 months, 13,9 months; in metastatic cases, 6,8 months, 4,9 months. Median overall survival over the entire population in the Her-2 positive and negative groups, respectively, was 23.4 months, 15.5 months; in nonmetastatic cases 35,9 months, 28 months; in the metastatic subgroup 11,8 months, 5,1 months. Her-2 frequency was found to be 18.2% in our study. While there was a significant correlation between Her-2 and Lauren classification (p <0.01), histological grade (p <0.01) and lung metastasis (p <0.05), there was no significant correlation with other parameters. There was not significant relation between Her-2 and progression free survival, overall survival. Between overall survival effect of liver metastasis and Her-2 was found significant (p <0.05).
