Obstrüktif uyku apne sendromu (OSAS) uykuda solunum bozuklukları arasında en sık görülen ve yaş, cinsiyet ve obezite gibi bazı predispozan faktörlerin eşlik ettiği çeşitli klinik patolojilere neden olan kompleks, poligenik ve multifaktöryel bir hastalıktır. HLA polimorfizmleri ve uyku bozuklukları arasındaki ilişki ortaya konulmuş fakat HLA alleleri ile ilişkisi tam olarak açıklık kazanmamıştır. Ayrıca OSAS hastalarında meydana gelen uyku yoksunluğu savunma mekanizmalarına zarar vererek immün sisteminin bileşenlerini ve tepkilerini etkileyen değişikliklere neden olabileceği belirtilmiştir. Çalışmamızın amacı, OSAS hastalığının HLA-DRB1 allelleri ile ilişkisini ve immün sistem hücreleri üzerine etkilerini araştırmaktır. Çalışmamızda, Şubat 2017-Kasım 2017 tarihleri arasında Göğüs Hastalıkları Polikliniğine başvuran tüm bireylerin BMI, yaş ve cinsiyetleri PSG öncesi kaydedildi. PSG sonuçlarını değerlendirerek OSAS hasta ve kontrol grubu oluşturuldu ve ODİ, SaO2 değerlerini kaydedildi. Ayrıca, OSAS hastaları AHI değerlerine göre hafif, orta ve ağır OSAS olarak gruplandırıldı. Kontrol ve hasta gruplarında PCR-SSP yöntemi ile HLA-DRB1 allellerine ve flow sitometri yöntemiyle de lenfosit alt gruplarına bakıldı. BMI, erkek cinsiyet ve yaş faktörünün OSAS hastalığında artmış olduğunu ve bu hastalarda SaO2 değerinin de anlamlı derecede düştüğünü belirledik. OSAS hastalığında HLA-DRB1*03 allelinin, ağır OSAS'lılarda ise HLA-DRB1*08 allelinin artmış olduğunu belirledik. OSAS hastalığında, immün sisteme ait lenfosit alt gruplarında ise CD19+B lenfosit ve CD3+T lenfositlerin değişmediğini, CD4+T lenfositlerin azaldığını ve HLA DR antijeni, CD8+T lenfosit ve CD16+56+(NK) hücrelerinin artmış olduğunu tespit ettik. Sonuç olarak, HLA-DRB*03 alleli varlığının OSAS hastalığına yatkınlığı arttırabileceği ve HLA-DRB*08 allelinin de hastalığın şiddeti ile ilişkili olabileceği kanaatine vardık. Ayrıca HLA DR antijeninin, CD8+T sitotoksik lenfositlerin ve NK hücrelerinin artışı OSAS hastalığının otoimmünite ile de ilişkili olabileceğini düşündürmektedir. Obstructive sleep apnea syndrome (OSAS) is a complex, polygenic and multifactorial disease that causes various clinical pathologies associated with some predisposing factors, such as age, sex, and obesity, which are the most common respiratory disorders in sleep. The relationship between HLA polymorphisms and sleep disturbances has been established, but the relationship with HLA alleles has not been fully clarified. It has also been suggested that sleep deprivation in OSAS patients may cause changes that affect the components and responses of the immune system by damaging the defense mechanisms. The aim of our study is to investigate the association of OSAS disease with HLA-DRB1 alleles and the effects on the immune system cells. BMI, age, and gender of all individuals who applied to the Chest Diseases Policlinic between February 2017 and November 2017 were recorded before the PSG in our study. Evaluating PSG results, OSAS patient and control group were formed and ODI, SaO2 values were recorded. In addition, OSAS patients were grouped as mild, moderate and severe OSAS according to their AHI values. In control and patient groups, lymphocyte subgroups were examined by the PCR-SSP method and HLA-DRB1 alleles and flow cytometry. We have found that BMI, male gender and age factor are increased in OSAS disease and that SaO2 value falls significantly in these patients. We found that HLA-DRB1 * 03 allele in OSAS disease and HLA-DRB1 * 08 alleles in heavy OSAS were increased. We found that CD19 + B lymphocytes and CD3 + T lymphocytes did not change, CD4 + T lymphocytes decreased and HLA DR antigen, CD8 + T lymphocytes and CD16 + 56 + (NK) cells increased in OSAS disease and immunocyte subgroups. As a result, we conclude that HLA-DRB * 03 alleles may increase susceptibility to OSAS disease and HLA-DRB * 08 alleles may be associated with severity of the disease. Furthermore, the increase in HLA DR antigen, CD8 + T cytotoxic lymphocytes and NK cells suggests that OSAS disease may also be related to autoimmunity.
