Bu çalışmada üst gastrointestinal yakınmalı hastaların endoskopi ile alınan mide biyopsi örneklerinde kültür, seroloji, histopatoloji ve PZR yöntemiyle Helicobacter pylori araştırılması amaçlanmıştır. Bu çalışmada 45 hastanın 23’ünün biyopsi örneklerinde H. pylori belirlenmiştir ve oran % 51,1 olarak gerçekleşmiştir. Yapılan serolojik testlere göre; IgA antikoru 18 hastada (% 78,3) ve IgG antikoru 22 hastada (% 95,7) tespit edilmiştir. IgA'nın duyarlılığı ve özgüllüğü sırasıyla % 78,3 ve % 81,8 iken IgG’nin % 95,7 ve % 72,7 olarak hesaplanmıştır. Bu çalışmada H. pylori suşlarının genotipleri çalışılmış ve bunların hastalıklarla ilişkisi de araştırılmıştır. 23 H. pylori suşunun incelenmesi sonucunda vacA 22, cagA ise 15 suşta pozitif olarak tespit edilmiştir. cagA/vacA genotipinde olan suşların sayısı ise 12 olarak bulunmuştur. Bu çalışmada; vacA s1 allelini taşıyan H. pylori sayısı 10 (% 43,5) iken 12 (% 52,2) suş s2 allelini taşımaktadır. Bununla birlikte; m1 allelini taşıyan 13 (% 56,5), m2 allelini taşıyan 6 (% 26,1) s1/m1 allelini taşıyan 6 (% 26,1), s1/m2allelini taşıyan 2 (% 8,7), s2/m1 allelini taşıyan 8 (% 34,8), s2/m2 allelini taşıyan 8 (% 34,8) suş tespit edilmiştir. Buna göre; vacA, cagA, vacA/cagA, s1/m1, s1/m2, s2/m1, s2/m2 genotiplerine sahip olan suşların gastrit, ülser ya da mide kanseri ile doğrudan bir ilişkisi olmadığı belirlenmiştir. Helicobacter pylori varlığını belirlemede biyopsi kültür sonuçları “altın standart” olarak alındığında bu çalışmada duyarlılığı en yüksek test olarak belirlenen PZR yönteminin; maliyetinin nispeten yüksek olması, uygulanması özel deneyim ve laboratuar donanımı gerektirmesi gibi dezavantajlarına rağmen, kolay ve hızlı bir tanı yöntemi olması, çok küçük materyallerde çok az sayıda bakterinin bile saptanmasına olanak sağlaması ve epidemiyolojik çalışmalarda olası farklı suşları tanımlayabilmesi nedeniyle öncelikle tercih edilebileceği kanısına varılmıştır. This study was planned in order to investigate the presence of Helicobacter pylori by culture, serology, histopathology and PCR in gastric biopsy specimens obtained by endoscopy from the patients with upper gastrointestinal tract complaints and to compare the results obtained from the used methods. In this study, 45 patients and 23 biopsy specimens of H. pylori was determined, and the rate was 51.1%. According to the serological tests; IgA antibody in 18 patients (78.3%), and IgG antibody in 22 patients (95.7%) were identified. The sensitivity and specificity of IgA IgG, respectively, 78.3% and 95.7%, while 81.8% and 72.7%. In this study we were investigated H. pylori strains genotypes studied, and their relevance to disease. As a result of analysis of 23 strains of H. pylori vacA 22, cagA positive strains have been identified as a 15. cagA/vacA genotype, while the number of strains were found to be 12. In this study, vacA s1 allele carrying the H. pylori infection with 10 (43.5%), whereas 12 (52.2%) strains are allele s2.However, m1 allele carrying the 13 (56.5%), with 6 m2 allele (26.1%) s1/m1 allele carrying six (26.1%), which s1/m2 alleles2 (8.7%), s2/m1 allele carrying eight (34.8%), with s2/m2 allele 8 (34.8%) strains were identified. vacA, cagA, vacA / cagA, s1/m1, s1/m2, s2/m1, s2/m2 genotype strains were found that have no relationship with gastritis, peptic ulcer or gastric cancer. The results of the biopsy to determine the presence of Helicobacter pylori culture "gold standard" in this study was taken as the PCR method is defined as the highest test sensitivity, the cost is relatively high, in spite of the disadvantages, such as the application requires specific experience and laboratory equipment, diagnostic method is easy and fast, and not too small materials that allow for the detection of a very small number of bacteria, and even in epidemiological studies can be preferred due primarily to identify different strains of the possible.
