Elektrik enerjisinin iletiminde, dağıtımında ve tüketiminde çeşitli nedenlerden dolayı meydana gelen arklar, patlamalara hatta yangınlara yol açmaktadır. Bu yangınlar, gerek yaşam alanlarında gerekse iş yerlerinde gerçekleştiğinde maddi hasarlara, yaralanmalara ve can kayıplarına sebep olmaktadır; bu durum, ülkemizin önemli problemlerinden biridir. Birçok ülkede, elektrik nedenli yangınları (ENY) konu alan çok sayıda çözüm odaklı çalışma ve araştırma vardır; ülkemizde ise -sadece araştırma boyutunda kalan- çok az sayıda çalışma yer almaktadır. Bu çalışma, ülkemizin bu konudaki ihtiyacını gidermeye odaklanmıştır. Çalışmada ilk önce, konuyla ilgili yurt dışında yapılan diğer çalışmalar incelenmiş ve bunların özetlerine 2. bölümde yer verilmiştir. İkinci olarak da Ankara, İstanbul, İzmir ve Antalya'da meydana gelen yangın verileri detaylı biçimde incelenmiş, elde edilen istatistik veriler bölüm 3.1., 3.2., 3.3. ve 3.4'te sunulmuştur. Yine aynı veriler aracılığıyla ülkemizdeki ENY'ler hakkında genel bir görünüm oluşturulmuş, bunlar da bölüm 3.5'te ortaya konulmuştur. Elektrik nedenli yangınların önlenmesi amacıyla geliştirilen (ark hatası oluştuğunda sistemin enerjisini kesen) cihazların özellikle endüstriyel tesislerde beklenen korumayı sağlayamadığı, incelenen çalışmalardan anlaşılmaktadır. Bu nedenle de endüstriyel tesislerde elektrik nedenli yangınların önlenmesi için en doğru yöntemin, etkin bir risk analizi çalışması olduğuna karar verilmiştir. Çalışmada üçüncü olarak iş güvenliği konusu ele alınmış ve konu, bölüm 3.6'da özetlenmiştir. Ardından yine aynı bölümde, FMEA (Failure Mode and Effects Analysis) yöntemi tanıtılmıştır; elektrik nedenli yangınların oluşturduğu tehlikeyi en aza indirmek veya tamamen ortadan kaldırmak için en uygun risk analiz yönteminin neden FMEA olduğu, gerekçeleriyle açıklanmıştır. Ancak FMEA'nın ülkemizde yanlış uygulandığı belirlenmiştir. Doğru uygulanmasını sağlamak için de, ilk olarak bu çalışmada, beş yeni yöntem ortaya konulmuştur. Bunlar: 1. Elektrik tesisatlarında yapılacak kontrol işleminin akış sıralaması, 2. Elektrik nedenli yangınlara yol açacak tehlikelerin kontrol listesi yöntemiyle (check- lists) yöntemiyle belirlenmesi, 3. "Bir iş kazasının meydana gelme olasılığı" için ülkemizin dört büyük ilinden elde edilen ENY istatistiklerinin kullanılması, 4. "Bir iş kazasının meydana gelmesinin fark edilmesi" için saha kontrol listeleri oluşturulması, 5. "Bir iş kazası meydana geldiğinde olası hasarın şiddetinin belirlenmesi" için saha kontrol listeleri oluşturulması. Her iş yerinde rahatça kullanılabilecek biçimde geliştirilen bu yöntemler, bir iş yeriyle bir ilköğretim okulunda uygulanmış ve uygulama sonuçları bölüm 4.1. ve 4.2'de sunulmuştur. Son çalışma olarak kontrol listesi (check-lists) yöntemi üzerinde - risk analizi sonrasında periyodik saha kontrollerinin yapılmasına olanak sağlayacak biçimde - geliştirme çalışmaları yapılmış ve yöntem, her iş yerinde rahatça uygulanabilecek hâle getirilmiştir. Geliştirilmiş yöntem, daha önce FMEA yapılan iş yeriyle ilköğretim okulunda uygulanmış, sonuçlar bu çalışmanın son bölümünde yer almıştır. Arcs that occurred a variety of reasons during the transmission, distribution and consumption of electricity energy lead to explosions and even fires. When these fires occurred in living spaces and places of work, they lead to loss of property and cause loss of lives and continue to be one of the important problems of our country. In many countries, there are a lot of researches and solution- oriented studies on electricity-induced fires, but there are only few studies in our country with a focus on research. This work is focused on responding the need of our country on this issue. Secondly, obtained data with detail examined and compilation of fires which occurred in Ankara, İstanbul, İzmir and Antalya separately are submitted in section 3.1, 3.2, 3.3, 3.4. At chapter 3.5 by aggregating these data, a general view of our country about that issue is presented. Devices which is developed to prevent fires of caused by electricity (which interrupt energy of systems when an arc fault occurs), it was understood from performed studies that it cannot provide expected protection especially in industrial facilities. For this reason, it has been decided that the most appropriate method for preventing fires caused by electricity in industrial facilities is an effective risk analysis study. Thirdly, it was given a summary of the occupational safety issue at chapter 3,6. Right after, Failure Mode and Effects Analysis (FMEA) which is a the risk analysis method is introduced and it is explained that the reasons why it is decided that the most appropriate risk analysis method is the FMEA method to minimize and eliminate the dangerous caused by electricity fires. It has been determined that the FMEA method was applied incorrectly in our country during the studies conducted within the scope of the study. Therefore, five new methods are presented for the first time in this study to ensure correct implementation of the method. 1. Flow sequence of the control process to be carried out in electrical installations, 2. Determination of the hazards that lead to electricity fires by means of check lists, 3. Using statistics of fires of caused by electricity from the four major provinces of our country for the calculation of the likelihood of a job accident or a fire, 4. Establishment of field checklists for "awareness of the occurrence of a job accident" 5. Establishment of field check lists for "determination of the severity of possible damage when a job accident occurs". These methods, which are developed in such a way that they can be used easily in every work place, have been applied in a work place and a primary school. Information about implementation is presented in sections 4.1 and 4.2. Fourthly, a new method has been developed and presented in this study within the scope of preventive studies. A checklist procedure was developed to allow periodic field controls after risk analysis and was also applied at two separate workplaces where FMEA method was applied to prevent electric fires.
