İmplant destekli protezler, kısmi ve tam diş kayıplarının tedavisinde önemli bir gelişme olarak kabul edilmiştir. Buna rağmen diş kaybı tedavisine ihtiyacı olan bazı hastaların konvensiyonel protezlerden implant destekli protezlere geçme konusunda tereddütleri devam etmektedir. Bu tereddütler kantilever uzantıya sahip implant destekli tam ve kısmi protezler ile doğal diş-implant destekli kısmi protezler için diş hekimleri tarafından da paylaşılmakta ve kantilever uzantının protezlerde yaşanan hasar oranını artıracağı düşünülmektedir. Temel mekanikten bilindiği üzere basit mesnetli kirişlere kıyasla kantilever uzantılı kirişlerin mesnetlerinde daha büyük kuvvetler ve momentler meydana gelir. Aynı sonuç, bazı durumlarda kullanımı kaçınılmaz olan kantilever uzantılı implant destekli diş protezleri için de geçerlidir. Dolayısıyla implant destekli protezlere kantilever uzantı eklemenin meydana getireceği kuvvetlerin ve momentlerin gerilme açısından olumsuz etkilerinin nasıl yok edileceği veya azaltılacağı ile ilgili çalışmalar bu alanda ilk sıralarda yer almaktadır. Bu çalışmada, alt çenede (mandibula) kullanılacak kantilever uzantıya sahip implant destekli barlı tam protezler için istenmeyen eğilme momentlerini minimize edecek yeni bir prostetik koping/abutment/implant bağlantısı tasarlanmıştır. Yeni bağlantı tasarımı, kantilever uzantısız uygulamalarda yaygın olarak kullanılan iki ticari ürüne adapte edilmiş ve iki yeni protez modeli oluşturulmuştur. Aynı tereddütler özellikle kantilever uzantılı için olmak üzere doğal diş-implant destekli kısmi protezler için de söz konusu olduğundan çalışmaya bu protezler de dahil edilmiş ve yeni bağlantı tasarımı uygulanmıştır. Çalışmanın birinci bölümünde klasik protez modelleri ile yeni bağlantı tasarımını içeren modifiye protez modellerinin sonlu eleman modelleri oluşturulmuş ve posterior bölgedeki çiğneme kuvvetlerinden dolayı meydana gelecek gerilme dağılımlarını elde etmek için gerilme analizleri gerçekleştirilmiştir. Çalışmanın ikinci bölümünde mekanik testler için kantilever uzantılı implant destekli barlı tam protez modellerinin prototipleri gerçek ölçülerinde imal edilerek alt çeneyi temsilen poliüretan köpük (ASTM F1839-8 Sınıf 40 Standardına uygun) bloklara monte edilmiştir. Modellerin her biri posterior bölgede çiğneme kuvvetlerini temsil edecek şekilde kantilever uzantının uç noktasına kuvvet uygulanarak yorulma deneyine (0-100 N, 2Hz ve 105 çevrim) tabi tutulmuştur. Yorulma deneyi sonrasında poliüretan köpük bloklara monte edilmiş ve yorulma deneyi uygulanmış modellere ait implantlara çekip çıkartma ve burma testleri uygulanmıştır. Gerilme analizlerinden elde edilen Von-Mises gerilme değerleri, tasarlanmış olan yeni bağlantının klasik modellerdeki bağlantı şekline kıyasla avantaj oluşturduğunu, kantilever uzantılı ve kantilever uzantısız implant destekli tam protezler ile kantilever uzantısız diş-implant destekli kısmi protezler için yeni bağlantı tasarımının rahatlıkla kullanılabileceğini göstermiştir. Yeni bağlantı tasarımı, kantilever uzantılı doğal diş-implant destekli kısmi protezler için gerilme değerlerinde azalmaya yol açmasına rağmen özellikle implantı çevreleyen kortikal kemikte meydana gelen gerilme değerlerinin yüksekliği göz önüne alındığında doğal diş-implant destekli kısmi protezlerde diş tarafında kantilever uzantıdan kaçınılması gerektiği, implant tarafında bir diş miktarı kantilever uzantı eklemenin ise bar yapı-diş arayüzeyinde çekme etkisi oluşturacağı göz önünde bulundurularak kullanılabileceği sonucuna varılmıştır. Yorulma deneyi sonrası mekanik testlerden elde edilen maksimum çekip çıkarma ve burma değerleri karşılaştırıldığında klasik ve modifiye protez modelleri arasında bir fark görülmemiştir. İmplantların poliüretan köpük ile bağlantı derecesini gösteren çekip çıkarma ve burma değerlerinde modeller arasında bir fark görülmemiş olması gerilme analiz sonuçları ile de örtüşen bir durumdur. Çünkü sonlu elemanlar yöntemi ile gerçekleştirilen gerilme analiz sonuçlarına göre uygulanan yükten dolayı implantı çevreleyen kortikal ve trabeküler kemik dokuda meydana gelen gerilme değerlerinde kantilever uzantılı tam protez modelleri arasında sonuçları olumlu ya da olumsuz anlamda etkileyecek derecede bir fark görülmemiş ve gerilme değerleri elastik sınırlar içinde kalmıştır. Implant supported prostheses have been recognized as a significant improvement in the treatment of partial and complete tooth loss. However, some patients who have partial or complete teeth loss and require treatment continue to hesitate to undergo implant-supported prostheses treatment rather than treatment with conventional prostheses. These hesitations are shared by dentists for implant or natural tooth-implant supported prostheses with cantilever extension and it is thought that the cantilever extension will increase the rate of damage experienced in the prosthesis. As is known from the basic mechanics, larger forces and moments occur in the supports of beams with cantilever extension compared to the simple supported beams without cantilever extension. The same rule applies to the implant-supported dental prosthesis with cantilever extension which is inevitable in some cases. Therefore, studies on how to eliminate or reduce the negative effects of the forces and moments occurred by adding cantilever extension to the implant-supported prosthesis take the first place in this area. In this study, a new prosthetic coping/abutment/implant connection was designed to minimize undesired bending moments for implant-supported full prosthesis with cantilever extension to be used in the mandible. The new connection design has been adapted to two commercial products that are widely used in applications without cantilever extension and two new prosthetic models were created. Since the similar hesitations apply to natural tooth-implant supported partial prosthesis, especially with cantilever extension, a new connection design has also been adapted to these prostheses and they were included in this study. In the first part of the study, the finite element models of the implant supported full prosthesis and the natural tooth-implant supported partial prosthesis with the classical and new connection designs were created. Then, the finite element stress analysis was performed to obtain the stress distribution in the components of the prosthesis models that would occur due to the occlusal forces in the posterior region. In the second part of the study, real scale prototypes of the implant supported full prosthesis models with cantilever extension were manufactured for mechanical tests and mounted on polyurethane foam blocks (ASTM F1839-8 Grade 40) representing the mandible. Each prosthesis model was subjected to a fatigue testing (0-100 N, 2 Hz, 105 cycles) by applying a vertical force to the end point of the cantilever extension to represent the occlusal forces in the posterior region. After the fatigue testing, the implants of the models mounted on polyurethane foam blocks were subjected to the pull-out and detorque tests. Von Mises stress values obtained by the finite element stress analysis show that the new connection design is advantageous compared to the connection types in the conventional models and so it can certainly be used for the full implant-supported prostheses with or without cantilever extension and tooth-implant supported partiral prostheses without cantilever extension. Although the new connection design leads to a reduction in the stress values for the components of the natural tooth-implant supported partial prostheses with the cantilever extension, the cantilever extension on the tooth side should be avoided due to the high stress values occurred in the natural tooth and the cortical bone surrounding the implant. However, a short cantilever extension on the implant side can be used by considering the stress in the tooth mostly occurs as a result of pull-out effect on the tooth-bar interface. When comparing the maximum pull-out and detorque values obtained from the post-fatigue mechanical tests, no difference was observed between the conventional and modified prosthetic models. This result is also consistent with the finite element analysis results. Because, the maximum von Mises stress values in the cortical and trabecular bone surrounding the implant are not so different as to affect the test results to one's favor for the full prosthesis models with cantilever extension and they were all within the elastic limits.
