Kronik böbrek yetmezliğinin prevalans ve insidansının giderek artması, bireylerin ve yakınlarının yaşam kalitesini olumsuz yönde etkilemesi, yüksek tanı ve tedavi maliyeti ile ülkelerin bütçelerine yük getirmesi bu hastalığın tedavisinde alternatif tedavi yöntemlerinin ekonomik değerlendirilmesini zorunlu kılmaktadır. Bu araştırmanın amacı, son dönem böbrek yetmezliği tedavisinde uygulanan hemodiyaliz, periton diyalizi ve böbrek transplantasyonu tedavi yöntemlerinin Sosyal Güvenlik Kurumu perspektifinden maliyet etkililiğini ve bu yöntemlerinin bütçeden ne kadar kaynak tükettiğini tespit etmektir. Araştırmanın maliyet boyutunda hemodiyaliz 29.592 TL, periton diyalizi 29.061 TL, böbrek transplantasyonu ise naklin gerçekleştiği birinci yılda, 51.279 TL, ikinci ve diğer yıllarda 8.654 TL maliyet oluşturduğu tespit edilmiştir.Araştırmanın etkililik boyutunda ise EQ-5D-5L ölçeği kullanılarak hemodiyalizde 0.6832,periton diyalizinde, 0.7092 ve böbrek transplantasyonunda 0.8929 HRQQL etkililik değerleri hesaplanmıştır.Araştırmada Markov modeli kullanılarak bu tedavi yöntemlerinin maliyeti, yaşam yılı ve kaliteye ayarlanmış yaşam yılı (QALY) hesaplanmış ve hesaplanan sonuçlarına %3 indirgeme yapılmıştır. Çalışmanın kapsadığı zaman dilimi ve iskonto oranının sonuçlara etkisini değerlendirmek için tek yönlü duyarlılık analizi yapılmıştır.Araştırma sonuçlarına göre Markov modelinde belirlenen zaman diliminde hasta başına maliyet hemodiyaliz, periton diyalizi ve böbrek transplantasyonunda sırasıyla 305.591 TL, 322.895,99 TL ve 217.259 TL; QALY değerleri ise hemodiyaliz 6.63, periton diyalizi 8.02 böbrek transplantasyonu ise 12.99 olarak saptanmıştır.Araştırmada Markov modelinden elde edilen sonuçlara göre bir yıllık yaşam süresinde hemodiyalizde 36.892 TL/YL, periton diyalizinde 29.921 TL/YL ve böbrek transplantasyonunda 14.401 TL/YL maliyet tespit edilirken QALY başına maliyet ise hemodiyalizde 46.105 TL/QALY, periton diyalizinde 40.274 TL/QALY ve böbrek transplantasyonunda 16.714 TL/QALY olarak hesaplanmıştır. Maliyet etkililik analizi sonucuna göre böbrek transplantasyonu hemodiyaliz ile karşılaştırıldığında 6.37 QALY ve (-) 88.332 TL ilave maliyet sağlamaktadır. Böbrek transplantasyonu tedavisinin İlave Maliyet Etkililik Oranı (İMEO), kazanılan QALY başına (-) 13.865 TL hesaplanmıştır.Böbrek transplantasyonu periton diyalizi ile karşılaştırıldığında 4.98 QALY ve (-) 105.637 TL ilave maliyet sağlamaktadır. Böbrek transplantasyonu tedavisinin İMEO, kazanılan QALY başına (-) 21.207 TL olarak hesaplanmıştır. Maliyet etkililik analizinde İMEO’nun eşik değerin çok altında olması nedeni ile böbrek transplantasyonu hem hemodiyaliz hem de periton diyalizine göre maliyet etkili olarak bulunmuştur. Markov modelinden elde edilen verilere göre bütçe etkisi analizinde bu üç tedavi yönteminin yıllık bütçeye finansal yükü ise 2.047.633.644 TL olarak hesaplanmıştır. Economic evaluation of chronic renal failure’s alternative treatment methods have become compulsory because of its prevalence and incidence rising, contagion of patients’ and their relatives’ life quality negatively and high cost of diagnosis and treatment. The purpose of this study is to confirm the cost effectiveness of hemodialysis, peritoneal dialysis and kidney transplantation which are used for renal failure treatment with perspective of Social Security Instution as well as confirm the alternative treatment methods’ amount consumed in budget.In this study confirmed that a patient’s cost of hemodialysis is 29.592 TL, cost of peritoneal dialysis is 29.061 TL, cost of kidney transplantation is 51.279 TL in first year of transplantation and 8.654 TL in second and the other years. Alternative treatment methods’ effectiveness values were evaluated with EQ-5D-5L scale.Hemodialysis effectiveness value was confirmed as 0.6832, peritoneal dialysis value was confirmed as 0.7092 and kidney transplantation value was confirmed as 0.8929.These treatment methods’ cost, life year and quality adjusted life year (QALY) were calculated by using Markov model and results were reduced by 3%. One-way sensitivity analysis was made for evaluating the effect of discount rate and study’s time frame.According to the Markov model’s results; cost per patient of hemodialysis is 305.591 TL cost of peritoneal dialysis is 322.895,99 TL and cost of kidney transplantation is 217.259 TL. Hemodialysis QALY value is 6.63, peritoneal dialysis QALY value is 8.02 and kidney transplantation QALY value is 13.00.According to Markov model results; in one-year life time cost of hemodialysis was found as 36.892 TL/YL, cost of peritoneal dialysis was found as 29.921 TL/YL and cost of kidney transplantation was found as 14.401 TL/YL. Meanwhile per QALY cost of hemodialysis was found as 46.105 TL/QALY, cost of peritoneal dialysis was found as 40.274 TL/QALY and cost of kidney transplantation was found as 16.714 TL/QALY.According to cost-effectiveness analysis result when kidney transplantation was compared with hemodialysis, kidney transplantation provides incremental cost as 6.37 QALY and (-) 88.332,44 TL. Incremental Cost-Effectiveness Ratio (ICER) of kidney transplantation was calculated as (-) 13.865 TL per acquired QALY. When kidney transplantation was compared with peritoneal dialysis, kidney transplantation provides incremental cost as 4.98 QALY and (-) 105.637 TL. ICER of kidney transplantation was calculated as (-) 21.207 TL per acquired QALY. Because of ICER is below the threshold in Cost-Effectiveness Analysis, kidney transplantation is cost-effective both regarding to hemodialysis and peritoneal dialysis. These three treatment methods’ financial charge is 2.047.633.644 TL in annual budget.
