Meme kanseri, dünyada milyonlarca kadını etkileyen, en sık görülen ve en çok ölüme neden olan önemli bir halk sağlığı problemidir. Meme kanserinin erken evrede tespit etmek ve meme kanserine bağlı ölümleri azaltmak için mamografi sağlık teknolojisi altın standart olarak kabul edilmektedir. Ancak mamografi sağlık teknolojisi ile meme kanseri taramasının meme kanserine bağlı ölümleri azaltmadığı, maliyet etkili olmadığı ve hatta bu teknoloji kullanmanın zararlı olduğu yönünde tartışmalar bulunmaktadır. Bu araştırmanın amacı; mamografi sağlık teknolojisi ile meme kanseri taramasının kadınlarda mortalite hızını ne kadar düşürdüğünü meta analizi yöntemiyle tespit etmektir. Araştırmanın örneklemini vaka grubunda meme kanseri taraması yapılan 4.886.684, kontrol grubunda ise meme kanseri taraması yapılmayan 4.332.659 kadın oluşturmaktadır. Araştırmada mamografi sağlık teknoloji ile meme kanseri taramasının etkinliğini tespit edebilmek için odds oranı (OR), relatif risk (RR) ve risk farkı (RD) hastalık risk (etki) ölçütleri olarak alınmıştır. Araştırmada meme kanseri taramasında sabit ve rastgele etki modelinde; mantel-haenszel, peto ve ters varyans istatistiki yöntemleri kullanılarak 39-49; 50 ≥; 40-74 yaş grubu ve kadınların 7 yıl boyunca takip edildiği araştırmaların alt grup bazında meta analizi yapılmıştır. Araştırmada meme kanseri taramasının her üç yöntemde risk etki büyüklüğü, 39- 49 yaş grubunda 0.98; 50 ≥ yaş grubunda 0.62; 40-74 yaş grubunda 0.83 ve 7 yıl boyunca takip edilen kadınlarda ise 0.80 olarak tespit edilmiştir. Araştırma sonuçlarının birleştirmesi sonucunda odds oranı her üç yöntemde etki büyüklüğü 0.82 (OR=0.82; %95 G.A: 0.78, 0.86) olup, istatiksel olarak anlamlı olduğu (Z=7.30; p < 0.00001) ve heterojen (X²=26.01, df = 28 (P = 0.57); I² = 0%) olmadığı tespit edilmiştir. Risk oranı her iki yöntemde de etki büyüklüğünün 0.82 (RR=0.82; %95 G.A: 0.78, 0.87), isttiksel olarak anlamlı olduğu (Z = 7.24; p < 0.00001) ve heterojen (X²= 26.81, df = 28 (P = 0.53); I² = 0%) olmadığı tespit edilmiştir. Risk farkının ise etki büyüklüğü -0.00 olarak tespit edilmiştir. Ayrıca araştırmada yayın yanlılığı tespit edilmemiştir. Sonuç olarak araştırmaya dahil edilen çalışmaların bulguları meta analizi yöntemi ile birleştirildiğinde mamografi sağlık teknoloji ile meme kanseri taramasının; 39-49 yaş grubunda % 2, 50 ≥ yaş grubunda % 38, 40-74 yaş grubunda % 17, kadınların 7 yıl boyunca takip edildiği araştırmalarda % 20 ve bu alt grupların birleştirilmesi sonucu % 18 oranında mortalite hızını düşürdüğü tespit edilmiştir. Breast cancer is a major public health problem affects millions of women in the world, the most common and leading to death. Mammography health technology is regarded as a gold standard. However thre are arguments about the breast cancer screening with mamography do not reduce the breast cancer related death cases, are not cost-effective and even it is harmfull to use this technology. The purpose of this research is to determine with meta-analysis method how breast cancer screening with mammography health technology reduce the women's mortality rate. The study sample consisted of 4.886.684 women have breast cancer screening with mammography health technology in case group and 4.332.659 women have not breast cancer screening with mammography health technology in control group. Odds ratio (OR), relative risk (RR) and risk difference (RD) were taken as a disease risk measurement in the study. Meta-analysis which 39-49; 50 ≥, 40-74 age group and research that women were followed for 7 years were made with mantel-haenszel, peto and reverse variance statistical methods in fixed and random effect model and close values were determined in all three methods. In the study, the effect size was approximately 0.98 in the 39-49 age group; 50 ≥ 0.62 in the age group; 0.82 in the 40-74 age group and 0.80 in the women who were followed for 7 years. As a result of combining research results odds ratio effect size in all three methods is 0.82 (OR=0.82; %95 G.A: 0.78, 0.86), statistically significant (Z=7.30; p < 0.00001) and is not heterogeneous (X²=26.01, df = 28 (P = 0.57); I² = 0%). Effect size in all two methods are used calculate the risk ratio is 0.82 (RR=0.82; %95 G.A: 0.78, 0.87), statistically significant (Z = 7.24; p < 0.00001) and is not heterogeneous (X²= 26.81, df = 28 (P = 0.53); I² = 0%). Effect size of risk difference is -0.00. As a result, when the findings of the studies included in the study were combined with the meta-analysis method, it was determined that the mortality rate of breast cancer screening by mammography health technology decreased by 2% in the 39-49 age group; 38% in the 50 ≥ age group; 18% in the 40-74 age group, 20% in the women followed for 7 years, and 18% in the combination of these subgroups.
