Sağlık harcamaları üzerinden günümüzde ülkelerin gelişmişlik düzeyleri ve sosyal devlet anlayışları görülebilmektedir. Bu açıdan ülke yönetimleri, ekonomileri geliştikçe refah seviyesini artırmak amacıyla temel kamu hizmetlerinden sayılan sağlık gibi erdemli mallara daha fazla kaynak ayırmaktadır. Türkiye İstatistik Kurumu'nun son on yıllık verileri incelendiğinde; ülkemizde sağlık harcamalarının gayrı safi milli hasılasına oranının ortalama % 5,8 olduğu görülmektedir. 2014 yılı verileri incelendiğinde toplam sağlık harcamaları tutarı yaklaşık 94.750.000.000 (doksandörtmilyaryediyüzellimilyon) türk lirası(TL) olduğu görülmektedir. Bu harcamaların %77,4' ünün kamu harcaması olarak yapıldığı görülmektedir. Kişi başı sağlık harcamasının ise 1.232 TL olduğu görülmektedir. Bu derece büyük kaynağın aktarıldığı sağlık sektöründe maliyet kontrolünün öneminin günden güne arttığı görülmektedir. Etkin maliyet kontrol yöntemlerinden biri de sorumluluk muhasebesi yöntemidir. Sorumluluk Muhasebesi; karmaşık yapıdaki işletmelerin yönetim kontrolünü sağlamak amacıyla oluşturulmuş yönetim muhasebesi ve maliyet muhasebesiyle birlikte kullanılan yönetim kontrol sistemidir. Bu sistem, karmaşık yapıdaki sağlık işletmelerinde merkezkaç yönetim sisteminin benimsenmesini ve alt düzey birimlerin yöneticilerine sorumluluk vererek performans artışını ve maliyetin azaltılmasını amaçlamaktadır. Bu sistemin sağlık işletmelerinde uygulanmasıyla; hastane içerisinde maliyet, gelir, kar ve yatırım merkezleri oluşturulur ve birim yöneticisinin sorumluluğunda etkin bir kontrol süreci sağlanabilinmektedir. Sorumluk Muhasebesi her ne kadar işletmeler açısından maliyet kontrolü sağlayan bir sistem olsa da hastanelerin asıl amacı olan insan sağlığının korunması ilkesiyle çelişmemelidir. Bu sistemin tam olarak uygulanması, özellikle ülkemizde sürekli kamu bütçe açığı yaratan kamu hastanelerinde maliyet kontrolü sağlamak ve verimi artırmak açısından yardımcı olacaktır. Bu çalışmada; ülkemiz kamu hastaneleri içerisinde büyük ölçekli sayılan bir "Şehir Hastanesinde" bir yıllık mali veriler, finansal tablolar, mizanlar ve muhasebe kayıtları incelenerek tüm birimlerin sorumluluk raporları hazırlanmıştır. Sorumluk raporları hazırlanmadan önce sorumluluk merkezleri belirlenirken hastane muhasebe sisteminde gelir kalemi görülmeyen Radyoloji ve Laboratuvar birimleri yardımcı üretim gelir merkezi olarak değil esas üretim gelir merkezi olarak değerlendirilmiştir. Radyoloji ve Laboratuvar birimlerinin gelirleri muhasebe sisteminde talepte bulunan birimine yansıtılmaktadır. Bu birimlere ait gelirler "Transfer Fiyatlaması" yöntemiyle tüm birimlerin ödenmeyen istemler toplamı bulunarak aktarılmıştır. Bu sayede Radyoloji ve Laboratuvar birimleri kar merkezi olarak değerlendirilmekte ve diğer tüm birimlere sorumluluk raporlarının daha şeffaf şekilde incelenebilmesi imkanı verilmektedir. Kar merkezleri TFRS 8: Faaliyet Bölümleri standardı kapsamında oluşturulmuştur. Gerekli durumlarda Faaliyet Bölümleri standardı çerçevesinde bölüm birleştirme kriterleri uygulanmıştır. Kamu hastane maliyetleri değerlendirilirken, direkt işçilik giderleri hastane gelirlerinden bağımsız olarak kamu ödemesi olduğu için sorumluluk raporlarını etkilemektedir. Bu konuda hem direkt işçilik giderleri sorumluluk merkezlerine gider olarak yansıtılmış raporlar hem de yansıtılmamış raporlar hazırlanmıştır. The development levels of countries and understanding of social state can be seen today through health expenditures. In this respect, state governments allocate more resources to virtuous property such as health, which is considered as one of the main public services, to increase the level of wealth as economies develop. When the last ten years' statistics of the Turkish Statistical Institute are examined; it is seen that the ratio of health expenditures to gross national product is 5.8% on average in our country. When the year 2014 data is analyzed, it is seen that total health expenditures are about 94.750.000.000 (nine and four billion seven hundred and fifty million) turkish liras (TL). It is seen that 77.4% of these expenditures are made as public expenditures. Per person health expenditure is 1.232 TL. It seems that the importance of cost control in the health sector, where such a large source has been transferred, has increased day by day. One of the effective cost control methods is the responsibility accounting method. Responsibility Accounting is a management control system used together with management accounting and cost accounting established to provide management control of complex structured businesses. This system is aimed to performance improvement and cost reduction in complex health company by giving responsibility to the managers of the lower-level units and adoption to the centrifugal management system. The implementation of this system in health businesses; in hospital costs, revenue, profit and investment centers are created and unit managers are provided under the responsibility of an effective control process. Responsibility Accounting System though the main purpose of providing control hospital costs for businesses but it must not contradict the principle of protecting human health. Full implementation of this system especially in our country creates cost control and efficiency in public hospitals which provide permanent public budget deficit, will help to improve the terms. In this study; one of the large-scale public hospital called "City Hospital"'s annual financial data, trial balance reports and accounting records are examined and responsibility reports for all departments are prepared. While responsibility reports are being prepared, Radiology and Laboratory units are not considered as production cost center but considered as income center which has not seen income in hospital accounting system. Revenues of Radiology and Laboratory units is recognized by the requesting unit in the accounting system. These unit's revenues are transposed by "Transfer Pricing" as the sum of payment transactions. In this way, radiology and laboratory units are evaluated as a profit center and It is given the opportunity to examine more transparent accountability report to all other units and it is given the opportunity to examine more transparent accountability report to all other units. Profit centers were created as part of the IFRS 8: Operating Segments standard. If necessary, section merge criteria are applied in the context of Operating Segments standard. In assessing the public hospital costs, direct labor costs are public payments and independent of hospital revenue which affects responsibility reports. In this regard, both direct labor costs were recorded as expense reports, and not recorded report are prepared to the responsibility centers.
