Bu çalışmanın temel amacı, bir üniversite hastanesinde faaliyet gösteren hemodiyaliz biriminde, öncelikle hemodiyaliz seansına ilişkin standart işlem aşamalarını, bu aşamalara ilişkin işlem sürelerini, kullanılan direkt ilk madde ve malzeme miktarlarını belirleyip hemodiyaliz seansının fiili ve standart maliyetini hesaplayarak, standart maliyet hesaplama modelinin sağlık kurumlarında da uygulanabilirliğini ortaya koymaktır. Tez konusu olan maliyet analizi çalışması, Pamukkale Üniversitesi Hastaneleri'nde yürütülmüştür. Yapılan çalışma sonucunda, hemodiyaliz seansı fiili maliyeti ₺219,37, hemodiyaliz seansı standart maliyeti ise ₺167,91, hesaplanmış olup fiili maliyetin standart maliyetten, ₺51,46 ve %30,65 oranında fazla olduğu belirlenmiştir. Kamu sübvansiyonu olarak değerlendirilen memur maaş ödemeleri hariç yapılan hesaplama sonucunda hemodiyaliz seans maliyeti ₺170,55 (₺219,37 - ₺48,83) ve standart maliyeti ise ₺138,64 (₺167,91 - ₺29,27)' dir. Hemodiyaliz ünitesinde 2014 yılı için verimlilik sapması ₺157.181 ve bütçe sapması ₺23.532 olarak hesaplanmıştır. İşletme yönetimi açısından, yöneticilere karar alma süreçlerinde ihtiyaç duyacakları her türlü bilgiye ulaşma imkânı sağlayacağından, sağlık kurumlarında standart maliyet hesaplama yönteminin kullanılması önerilmektedir. Çalışma ayrıca, pahalı teknoloji kullanımı gerektiren hemodiyaliz ünitelerinde, diyaliz sürelerine ilişkin standartların oluşturulmasına, hemodiyalize girecek olan hastaların tedaviye erişimde bekleme sürelerinin kısaltılmasına, maliyet artışlarına neden olabilecek sağlık personelinin tutum ve davranışlarından kaynaklanan etken olmayan sürelerin azaltılmasına ve biyomedikal imkânlardan maksimum düzeyde yararlanılmasına olumlu etki edecektir. The main objective of this study is to determine the standard treatment steps for the hemodialysis session, the treatment times for these steps, the actual and standard costs of the hemodialysis session by determining the amount of the first raw materials and materials used in the hemodialysis unit operating in a university hospital and the standard cost calculation model applicability. The cost analysis study, which is the thesis subject, was carried out at Pamukkale University Hospitals. As a result of the study, it was determined that the actual cost of the hemodialysis session was ₺219,37, the standard cost of the hemodialysis session was ₺167,91, and the actual cost was found to be ₺51,46 and 30,65% higher than the standard cost. As a result of the calculation made except for salary payments for civil servants who are considered as public subsidies, the hemodialysis session cost is ₺170,55 (₺219,37 - ₺48,83) and the standard cost is ₺138,64 (₺167,91 - ₺29,27) . The productivity deviation for the year 2014 in the hemodialysis unit was calculated as ₺ 157.181 and the budget deviation ₺ 23.532. In terms of business management, it is suggested to use standard cost calculation method in health institutions as it will provide managers with access to any information they need in decision making process. The study will also have a positive impact on establishing standards for dialysis durations in hemodialysis units that require costly technology use, shortening waiting times for access to treatment for hemodialysis patients, reducing ineffective periods resulting from health staff attitudes and behaviors that may lead to cost increases, and maximizing biomedical opportunities.
