Melez iş kimliği, kişilerin eğitimini aldıkları meslek dolayısıyla kazandıkları profesyonel kimlikleri ile çalıştıkları kurum içerisindeki görevleri vasıtasıyla kazandıkları iş kimliklerinin bir araya gelmesiyle oluşan kimliklerin melezleşmesi durumudur. Sağlık alanının doğal lideri olarak görülmeleri sebebiyle hekimliklerinin yanı sıra yönetim kademelerinde de görevlendirilen hekimlerin durumu da melez iş kimliği olarak değerlendirilebilir. Aynı şekilde bu durumdaki hekimler de melez hekim yöneticiler olarak adlandırılmaktadır. Bu araştırma, yöneticilik yapmakta olan hekimlerin, mevcut durumları hakkındaki görüşlerinin değerlendirildiği, kendi mesleki kimlikleri için yaptıkları tanımların ve onları bu tanımlamaya götüren sebeplerin araştırıldığı, mevcut sağlık yönetimi düzeni ve geleceğe yönelik önerilerinin yer aldığı, bu kapsamda melez hekim yöneticilerin yaşadıkları sorunlar tespit edilerek, ideal sağlık yöneticisi kimdir sorunsalına cevap aranmaya çalışılan niteliksel bir çalışmadır. Araştırmanın çalışma grubu, amaçlı örnekleme yöntemlerinden ölçüt örnekleme yöntemiyle belirlenmiş ve bu çerçevede İstanbul, Ankara ve Isparta illerindeki 3 başhekim, 4 başhekim yardımcısı, 3 eski başhekim ve 1 hastane yöneticisi hekim ile görüşmeler yapılmıştır. Araştırmada nitel araştırma yöntemlerinden derinlemesine görüşme tekniği kullanılmıştır. Bu kapsamda yarı yapılandırılmış görüşme formu kullanılmıştır. Görüşme formunda hekim yöneticilerin yönetici oluş süreçlerinin, mesleki kimlikleri hakkındaki görüşlerinin, hekimlik ve yöneticiliğe bakışlarının, hekim yöneticilik sürecinde yaşadıkları sorunların ve akıllarındaki ideal sağlık yöneticisi tasvirlerinin sorgulandığı sorular yer almaktadır. Bu sorular haricinde görüşmenin seyri ve araştırmacının gördüğü lüzum üzere yeni sorular eklenerek görüşmeler gerçekleştirilmiştir. Yapılan görüşmeler katılımcıların izin vermesi durumunda ses kayıt cihazı kullanılarak ya da araştırmacı tarafından not alınarak kayıt altına alınmıştır. Çalışma kapsamında yapılan görüşmelere ait ses kayıtları yazıya geçirilerek araştırmacı tarafından defaatle okunmuş ve çalışmanın temel soruları doğrultusunda kodlamalar yapılmıştır. Araştırmanın analiz sürecinde nVivo 12 nitel araştırma programı kullanılmış ve tematik analize tabi tutulmuştur. Yapılan araştırma ve analiz sonucunda melez hekim yöneticilerin kendilerini öncelikle hekim olarak tanımladıkları, yöneticiliği ise yabancı oldukları, farklı ve zor bir alan olarak değerlendirdikleri, uzman olan hekim yöneticilerin mesleki ikilem yaşadığı, pratisyen hekim yöneticilerin ise hekimlikten tamamen kopma noktasına geldikleri görülmüştür. Bu çerçevede hekim yöneticiliğin mesleki körelme ve hekimlikten uzaklamaya sebep olduğu sonuçlarına ulaşılmıştır. Elde edilen sonuçlar çerçevesinde herbiri potasiyel yönetici olan hekimlerin tıp eğitimi sırasında yöneticiliğe yönelik eğitimler alması, sağlık yönetim sistemine profesyonel sağlık yöneticilerinin entegre edilmesi, meslekten kopma ve mesleki körelmelerin önüne geçmek adına hekimlerin uzun sürelerle yöneticilik yapmaması ve rotasyona tabi tutulması yönünde öneriler sunulmuştur. The hybrid work identity is the hybridization of the identities that are formed by the combination of the professional identities that the people earned for their profession and the work identities they earn through their duties in the institution they are working with. Because of the fact that they accepted as the natural leaders of healthcare sector, physicians are assigned as administrative staff as well as medical staff. So, situation of physicians is also called as the hybrid physician managers. This research is a study that evaluates opinions about the current situation of physicians who are managing, and of their definitions of their professional identities and the reason for these definitions, of current health management practice and future proposals and also is a qualitative study which seeks to find answers to the question of "who is the ideal health manager" by identifying the problems that hybrids are experiencing in their own context. The study group of the study was determined by using the sampling method of purposeful sampling methods. In this framework, interviews were made with 3 chief physicians, 4 chief physician assistants, 3 former chief physicians and 1 hospital manager physician in Istanbul, Ankara and Isparta provinces. In-depth interviewing technique was used in qualitative research methods. In this context, semi-structured interview form is used. In the interview form there are questions about the managerial process of physician managers, their opinions about their professional identities, the views of physicians and administrators, the problems they have experienced in physician management process and the images of ideal healthcare managers in their minds. Except from these questions, interviews were conducted by adding new questions to the interviewer and the need of the investigator. The interviews were recorded by the researcher using a voice recorder with permission of participants. The voice recordings of the interviews carried out in the scope of the study were written. These have been read many times by the researcher. The decryption and codification process were done in accordance with the basic questions of the study. The nVivo 12 qualitative research program was used in the analysis of the study and subjected to thematic analysis. As a result of the research and analysis, the following results have been determined. Hybrid physician directors define themselves primarily as physicians. They regard the management as a different and difficult area that they foregin. It has been seen that the administrators who are specialist physicians have a professional dilemma and the administrators who are general practitioners have come to the point of being completely disconnected from the profession of medicine. Within the framework of the results obtained, suggestions are presented in the following direction. Physicians, each of which is a potential manager, should receive training in management during medical education. Professional health care managers should be integrated into the health management system. Physicians should not be a manager for a long time and should be subjected to rotation.
