AMAÇ: Klimizde ortognatik cerrahi tekniklerden lefort 1 cerrahisi, bilateral sagital split cerrahisi ve her iki cerrahi geçiren hastaların kan serum RAN,RANKL ve OPG değerlerinin cinsiyet, zaman ve ameliyat tipi olarak karşılaştırılmasını değerlendirdik. Amacımız cerrahi gereksinimi olan ve ameliyat uygulanan hastalarda kan serum sitokin değerlerinin ortognatik cerrahide kemik iyileşme hızını ve remodelingi ne denli etkilediğini araştırmaktır. GEREÇ VE YÖNTEM: Ortognatik cerrahiye gereksinim duyan 17 hasta ortodontist tarafından değerlendirilerek muayene edildi. Operasyondan önce kan örneğinden (kontrol grubu) alınan kandaki RANK, RANKL ve OPG değerlerinin belirlenmesi amaçlanır. Operasyondan sonra yine örnek (çalışma grubu). Hastalardan alınan kan örnekleri ependorf tüplerinde -80C'de serumize edilerek saklanmıştır. Bu tüplerdeki serum örneklerinde RANK, RANKL, OPG miktarların ELISA cihazları yardımıyla saptanmıştır. BULGULAR: Hem OPG hem de sRANKL için, lokal ve sistemik düzeyler arasında anlamlı bir fark bulunmadı ve değerleri, ameliyat edilen zaman noktalarında cerrahi sonrası kırık iyileşme süreci ile değiştirilmedi. RANK, RANKL ve OPG molekülleri arasında kırık onarımı sürecinde ana rol sRANK tarafından oynanmaktadır. RANK osteoklast aktivitesinde önemli bir rol oynadığından, cerrahi sonrası bu sitokinin düzeylerinin hızlı ve lokal olarak artması, onarım sürecindeki osteoklastik aktivitenin artışına işaret edebilir.Elde edilen sonuçlarda istatistiksel olarak anlamlı bir sonuç olmasına rağmen, ameliyattan 1 ve 10 gün sonra ameliyat öncesi serum örneklerinde OPG değerinin hafif arttığı belirlendi.RANKL değerlerinin, RANKL değerlerinin cerrahi öncesi azalmasına rağmen, RANKL değerlerine ters orantılı olduğu ancak istatistiksel olarak anlamlı olmadığı bulundu. SONUÇ: Bu biyobelirteçleri kemik iyileşme sürecini izlemek için olası bir araç olarak değerlendirmek amacıyla, çalışmamız ortognatik cerrahi sırasında RANK / RANKL / OPG sisteminin ilk kapsamlı değerlendirmesi olarak düşünülebilir. OBJECTIVES: We compared the blood serum RAN, RANKL and OPG values of orthognathic surgery techniques in terms of gender, time, and type of surgery for left 1 surgery, bilateral sagital split surgery and both surgical patients. Our aim is to investigate the effect of blood serum cytokine levels on orthognathic surgery on bone remodeling and remodeling in surgically treated patients. MATERIALS AND METHODS:Seventeen patients who need orthognathic surgery will be identified after being evaluated and examined by the orthodontist.İt is aimed to determine the values of RANK,RANKL and OPG in the blood be taken from the blood sample (control group) before the operation and the blood sample(study group) again after the operation.Blood samples taken from patients will be serumized and stored at -80C in ependorf tubes.İt is aimed to determine the amounts of RANK,RANKL,OPG in serum samples in these tubes with the help of ELISA devices. RESULTS:For both OPG and sRANKL there were no significant differences between local and systemic levels and their values were not altered by the fracture healing process after surgery at the considered time points. Among the RANK, RANKL and OPG molecules, the main role during the fracture repair process is played by sRANK. Since RANK has a major role in osteoclast activity, the rapid and local increase of the levels of this cytokine after surgery could indicate an increase of the osteoclastic activity involved in the repair processAlthough there was a statistically significant result in the obtained results, it was determined that OPG value was slightly increased in the serum samples before surgery from the patients on days 1 and 10 after ameliyat.RANKL values were found to be inversely proportional to the values of RANKL values despite the decrease in RANKL values before surgery, but not statistically significant CONCLUSIONS:Our study can be considered the first comprehensive evaluation of the RANK/RANKL/OPG system during orthognathic surgery, in order to evaluate these biomarkers as a possible tool to monitor the bone healing process. Further investigation will be performed to improve the correlation of these parameters with clinical evaluation of surgery and post surgery recovery.
