Bu çalışmanın amacı; dört farklı cam iyonomer içerikli restoratif materyalin yüzey morfolojilerinin taramalı elektron mikroskobu ile, yüzey pürüzlülüklerinin atomik kuvvet mikroskobu ile incelenmesi ve materyallerin yüzeyinin Streptococus Mutans tutulumu açısından karşılaştırılmasıdır. Çalışmamızda; bir rezin modifiye cam iyonomer siman, bir pre-reacted cam iyonomer/Giomer, bir seramik ilaveli cam iyonomer/amalgomer, bir karbomer ve floridapatit ile güçlendirimiş cam iyonomer/cam karbomer olmak üzere dört adet cam iyonomer içerikli restoratif materyal kullanıldı. Çalışmada kullanılan materyallerin yüzey morfolojilerinin, pürüzlülüklerinin ve biyofilmin incelenmesi amacı ile; standart 6 mm çapında ve 2 mm kalınlıkta disk şeklinde örnekler hazırlandı. Restoratif materyallere ait 3'er örneğin yüzey morfolojisi; taramalı elektron mikroskobu ile X 500, X 1000, X 2000 büyütmelerde düşük vakum, yüksek voltaj tekniği ile incelendi. Materyallere ait örneklerin yüzey pürüzlülükleri AFM cihazının profilometre modunda değerlendirildi. Çalışmamızda ölçüm uzunluğu 2 cm olarak ayarlandı. Her örnekten üç ölçüm yapıldı. Elde edilen 3 ölçümün aritmetik ortalaması alınarak, o örneğin yüzey pürüzlülük değeri olarak Ra cinsinden kaydedildi. Çalışmanın biyofilm aşamasında, referans bakteri olarak Streptococcus mutans ATCC 25175 suşu kullanıldı. Hazırlanan 4 farklı plaka S. mutans ATCC 25175 suşunun test edilen disk materyallerine adezyonun sağlanması için 1, 7, 21 ve 30 gün süreyle 37 °C'de inkübe edildi. Adhere olan hücre sayıları, alınan örneklerin FTS ortamı kullanılarak yapılan seri dilüsyonlarının BHI agar besiyeri ortamlarına damlatılması ve petri kutularının 37 °C'de 48 saat inkübasyonu sonucu gelişen kolonilerin sayımı sonucu tespit edildi. Taramalı elektron mikroskobu analizi sonucunda; rezin modifiye cam iyonomer ve Giomer gruplarının yüzeyinin cam karbomer ve amalgomere kıyasla daha pürüzsüz ve homojen olduğu görüldü. Bununla birlikte amalgomere ait örneklerin yüzeyinde çok sayıda sığ ve kısa mikro çatlak tespit edilirken, cam karbomere ait örneklerin yüzeyinde daha derin ve devamlılık gösterir tarzda mikro çatlaklar olduğu belirlendi. Yüzey pürüzlülüğü ölçümleri sonucunda, en yüksek pürüzlülük değerleri amalgomer ve cam karbomer grubunda elde edilirken; pürüzlülük değerlerinin en düşük olduğu materyaller Giomer ve rezin modifiye cam iyonomer olarak belirlendi. Materyallerin yüzey pürüzlülüğü değerleri arasındaki fark istatistiksel olarak anlamlı idi (p<0,05). İkili karşılaştırmalar sonucunda ise, amalgomer ile cam karbomer arasında (p=1,00) ve rezin modifiye cam iyonomer siman ile Giomer arasında (p=1,00) istatistiksel olarak anlamlı bir fark bulunmadı. Hem rezin modifiye cam iyonomer siman hem de Giomer'in amalgomer ve cam karbomer ile arasında ise anlamlı bir farklılık bulundu (Bonferroni, p=0,00). Biyofilm oluşumuna ait sonuçlarda, istatistiksel değerlendirme sonucunda; 1. gün adhere olan bakteri sayısı açısından, gruplar arasında anlamlı bir farklılık bulundu (Kruskal Wallis, p=0,013). Analiz sonuçlarına göre 1. gün sonunda sadece rezin modifiye cam iyonomer ve Giomer arasında anlamlı bir fark olduğu (Mann Whitney U, p=0,008) ve rezin modifiye cam iyonomerin Giomere oranla yüzeyinde istatistiksel olarak anlamlı derecede daha düşük bakteri sayısına sahip olduğu belirlendi. İstatistiksel olarak arasında anlamlı fark bulunmayan gruplar ise; amalgomer ve rezin modifiye cam iyonomer, amalgomer ve cam karbomer, amalgomer ve Giomer, rezin modifiye cam iyonomer ve cam karbomer ile cam karbomer ve Giomer olarak tespit edildi. Materyallerin 7. gün sonunda yapılan değerlendirmelerinde, adhere olan bakteri sayısı açısından gruplar arasında istatistiksel olarak anlamlı bir fark olmadığı tespit edildi (Kruskal Wallis, p=0,106). Gruplar arasında 21. gün sonunda da adhere olan bakteri sayısı açısından istatistiksel olarak anlamlı bir fark olmadığı belirlendi (Kruskal Wallis, p=0,095). Sonuç olarak; çalışmamızda kullanılan cam iyonomer içerikli restoratif materyallerden, rezin modifiye cam iyonomer ve Giomerin, cam karbomer ve amalgomere kıyasla yüzey pürüzlülüğünün daha düşük olduğu ve ayrıca yüzeylerinin daha homojen ve düzgün bir yapı sergilediği belirlendi. Biyofilm oluşumu testi sonuçlarına göre sadece 1. günün sonunda ve sadece rezin modifiye cam iyonomer ve Giomer arasında anlamlı bir fark olduğu ve rezin modifiye cam iyonomerin Giomere oranla yüzeyinde istatiksel olarak anlamlı derecede daha düşük bakteri sayısına sahip olduğu belirlendi. Elde ettiğimiz sonuçlara göre materyalin yüzey pürüzlülüğü ile bakteri birikimi açısından doğrudan bir ilişki kurulamayacağı söylenebilir.
The aim of this study is; to analyse the surface morphology of four different glass ionomer containing restorative materials by scanning electron microscopy, the surface roughness by atomic force microscopy and to compare the material surfaces in terms of Streptococus Mutans adherence. Four glass ionomer-containing restorative materials such as a resin-modified glass ionomer cement, a pre-reacted glass ionomer / giomer, a ceramic reinforced glass ionomer / amalgomer and flourapatite reinforced glass ionomer / glass carbomer were used in our study Standard 6 mm diameter and 2 mm thick disk shaped specimens were prepared to evaluate the surface morphology, the surface roughness and biofilm of the materials used in the study. Surface morphologies of 3 samples from restorative materials were viewed with scanning electron microscope by low vacuum high voltage technique at X 500, X 1000, X 2000 magnifications. The surface roughness of the< samples were evaluated at the profilometer mode of the AFM instrument. The measurement length was set to 2 cm in our study. Three measurements were taken from each sample. The arithmetic average of the 3 measurements was recorded in terms of Ra value as the surface roughness value of that sample. Streptococcus mutans ATCC 25175 strain was used as a reference bacterium during the study's biofilm phase. The prepared four different plates of S. mutans ATCC 25175 strains were incubated at 37 ° C for 1, 7, 21 and 30 days to provide adhesion to the tested disk materials. Adhered cell counts were determined as a result of the counting the number of colonies which produced by dropping the number of serial dilutions of the samples taken using FTS medium to the BHI agar media and incubating the petri dishes at 37 ° C for 48 hours. As a result of scanning electron microscopy analysis; the surface of the resin-modified glass ionomer and giomer groups was found more smooth and homogenous than the glass carbomer and amalgomer surfaces. However; while a large number of shallow and short microcracks were found on the surface of amalgomer specimens, the deeper and continuous micro-cracks on the surface of glass carbomers samples was determined. As a result of the surface roughness measurements, while the highest roughness values were obtained for the amalgomer and glass carbomer groups; the materials with the lowest roughness values were identified as giomer and resin-modified glass ionomer. The difference between the surface roughness values of the materials was statistically significant (p <0,05). As a result of pairwise comparisons, it was found that there was no statistically significant difference between amalgomer and glass carbomer (p = 1,00) with resin modified glass ionomer cement and giomer (p = 1,00). Both resin-modified glass ionomer cements and giomers showed significant difference between amalgomer and glass carbomer (Bonferroni, p = 0.00). As a result of statistical evaluation of biofilm formation; there was a significant difference between the groups in terms of the number of adhered bacteria on the first day (Kruskal Wallis, p = 0.013). According to the results of the analysis, it was determined that there was a significant difference between only resin-modified glass ionomer and giomer at the end of the 1st day (Mann Whitney U, p = 0,008) and that the resin-modified glass ionomer had a statistically significant lower number of bacteria on its the surface than giomer. It was found that there is no statistically significant difference between some groups; such as amalgomer and resin modified glass ionomer, amalgomer and glass carbomer, amalgomer and giomer, resin modified glass ionomer and glass carbomer and glass carbomer and giomer. At the end of the 7th day, no statistically significant difference was observed between the groups in terms of number of adhered bacteria (Kruskal Wallis, p = 0,106). And also there was no statistically significant difference between the groups in terms of number of adhered bacteria (Kruskal Wallis, p = 0,095) at the end of 21 days. Consequently, it was determined that the resin-modified glass ionomer and giomer had lower surface roughness values and also their surfaces had more homogeneous and smooth structure compared to glass carbomer and amalgomer among the glass ionomer containing restorative materials used in our study surfaces have a more homogeneous and smooth structure. According to the results of the biofilm formation test, it was determined that there was a significant difference between only resin-modified glass ionomer and giomer only at the end of day 1 and that the resin-modified glass ionomer had a statistically significant lower number of bacteria on the surface than the giomer. According to the results obtained, it can be said that the bacterial accumulation and the surface roughness of the material are not directly related.
