Ağır Obstruktif Uyku Apne Sendorumlu ve REM ilişkili Obstruktif Uyku Apne Sendromlu Hastalarda Diyabet İlişkisi Bu çalışmada ağır OSAS ve REM ilişkili OSAS' lı hastalar ile diyabet ilişkisini araştırmayı amaçladık. Çalışmamızda, Ocak 2018 ile Mayıs 2018 tarihleri arasında Süleyman Demirel Üniversitesi Tıp Fakültesi Göğüs Hastalıkları Anabilim Dalı Uyku Bozuklukları Merkezi ve Isparta Şehir Hastanesi Uyku Merkezine başvuran ve tek gece polisomnografi (PSG) tetkiki yapılan hasta verileri prospektif olarak incelendi. AHI> 30 olay/saat olan, ağır OSAS tanılı 26 hasta, AHI>5 olay/saat olan, ve REM AHI değerinin NREM AHI değerinden 2 kat ve üzeri olan REM ilişkili OSAS tanılı 10 hasta ve diyabet polikliniğine başvuran 287 kişiden STOP BANG testi 2 ve üzeri olan 12 hastadan çalışmayı kabul eden 8 diyabetli hasta çalışmaya dahil edildi. Çalışmada yaş, cinsiyet VKİ gibi demografik özellikler, açlık kan şekeri, en az 2 saat arayla alınan 2. kan şekeri değeri ve HbA1c değerleri, toplam uyku süresi, uyku latansı, REM latansı, uyku mimarisi ( Evre 1, Evre 2, Evre 3 ve REM) , apnehipopne indeksi (AHİ) oksijen desatürasyon indeksi (ODI) gibi polisomnografi parametreleri incelendi. Çalışmaya alınan gruplar 3 grup halinde incelendi. Grup1; ağır OSAS tanılı hastalar, Grup 2; REM ilişkili OSAS tanılı hastalar, Grup 3; diyabet tanılı olup PSG ile OSAS tanısı alan hastalardan oluşturuldu. Çalışmaya alınan hastalar; 26 ağır OSAS (%59.1) 10 REM ilişkili OSAS (%22,7) ve diyabet polikliniğine başvuran 287 hastadan STOP-Bang testi sonucu >2 üzeri olan 12 hastanın 8'ine tek gece PSG yapılarak OSAS tanısı (%18.2) aldı. Cinsiyet açısından gruplar arasından anlamlı fark yoktu. Vücut ağırlığı olarak bakıldığında kadınlar erkeklere göre daha kiloluydu. REM ilişkili OSAS tanısı alan hastaların çalışma popülasyonu ile kıyaslandığında daha yaşlı, VKİ daha yüksek ve kadın cinsiyette oldukları saptandı. Ağır OSAS'lı kadın hastalarda REM/NREM AHI, REM AHI incelendiğinde anlamlı olarak fark saptandı. REM ilişkili OSAS ve ağır OSAS'ın diyabet ile ilişkili olduğu, ağır OSAS hastalarındaki glukoz ve HbA1c değerlerinin REM ilişkili OSAS hastalarına göre daha yüksek olduğu saptandı. REM ilişkili OSAS tanılı hastalarda Evre 1 ve Evre 2 süreleri kısalmış, Evre 3 değeri ise artmış olarak bulundu. Sonuç olarak REM ilişkili OSAS ve ağır OSAS'ın diyabet ile ilişkili olduğu, diyabet tanısı almış olan hastaların OSAS açısından değerlendirilmesi gerektiği, OSAS'ın tedavi edilmediği takdirde diyabet gibi komplikasyonlara yol açabileceği düşünülmelidir.. Diyabet tanısı almış olan hastaların polisomnografik incelenmesinde REM ilişkili OSAS sıklığının daha yüksek olması diyabetin REM ilişkili OSAS'lı hastalarda daha sık olduğunu düşündürmektedir
Diabetes Correlation in Patients with Severe Obstructive Sleep Apnea Syndrome and REM related Obstructive Sleep Apnea Syndrome We aimed to examine the diabetes correlation in patients with severe OSAS and REM related OSAS in this study. In our study, data of the patients who applied to Suleyman Demirel University Medical Faculty Department of Chest Diseases Somnipathy Center between the dates of January 2018 and May 2018 and were administered over-night polysomnography (PSG) were prospectively examined. 26 patients, diagnosed with severe OSAS, at which AHI> 30 instance/hour, 10 patients, diagnosed with REM related OSAS, at which AHI> 5 instance/hour and REM AHI values are more than twice the NREM AHI values and 8 of 12 diabetes patient who agreed be a part of the study, whose STOP BANG test results were 2 or higher, out of the 287 people who applied to diabetes polyclinic were included in the study. In this study, demographic features such as age, sex, BMI. Blood test such as pre-prandial blood glucose, 2nd blood glucose taken at least 2 hours after and HbA1c values, polysomnographic parameters such as entire sleep period, sleep latency, REM latency, sleep architecture (Stage 1, Stage 2, Stage 3 and REM), apnea- hypopnea index (AHI), oxygen desaturation index (ODI) were examined. Groups taking part in the study were examined in 3 groups. Group1; consists of patients diagnosed with severe OSAS, Group2; consists of patients diagnosed with REM related OSAS, Group3; consists of patients who are already diagnosed with diabetes and were also diagnosed with OSAS via PSG. Patients who took part in the study were diagnosed as follows; 26 patients with severe OSAS (59.1%), 10 patients with REM related OSAS (22.7%) and 8 of 12 diabetes patient, whose STOP BANG test results were 2 or higher, out of the 287 people who applied to diabetes polyclinic (18.2%). In relation to sex, there were no significant difference among the groups. When body mass was considered, females were heavier compared to males Compared to the study population, it was observed that patients diagnosed with REM related OSAS were older, had higher BMI and were female. When REM/NREM AHI and REM AHI were examined in female patients with severe OSAS, a significant difference was observed. It was determined that REM related OSAS and severe OSAS were correlated with diabetes and glucose and HbA1c values of patients with severe OSAS were higher compared to patients with REM related OSAS. In patients diagnosed with REM related OSAS, it was observed that Stage 1 and Stage 2 periods were shortened and Stage 3 values were increased. In conclusion, it is determined that REM related OSAS and severe OSAS are correlated with diabetes and patients diagnosed with diabetes should be evaluated in terms of OSAS and unless treated, OSAS can lead up to complications such as diabetes. High frequency of REM related OSAS in patients diagnosed with diabetes, revealed by the polysomnographic examination, leads to the conclusion of; diabetes occurring more frequently in patients with REM related OSAS.
