Bu çalışmanın amacı; çalışma gurubunda KBY ve non KBY'li hastalarda diyabet, diyaliz durumu, KBY hastalık süresi ve ilaç kullanımı karşısında, ADC değişimleri ve kimyasal şift MR bulgularının karşılaştırılması ve ayrıca diyabetik nefropatideki yağlı dejenerasyon ve lipotoksite tezini kimyasal şift sekanslarını içeren MR tekniklerini kullanarak değerlendirmektir. MRG çekimleri 1.5 Tesla manyetik alan gücüne sahip cihaz ile (MAGNETOM Avanto, Simens Medical Solitions, Erlangen, Germany) yapıldı. Hastaların üst batın MRG tetkikleri sedasyon gerekmeksizin supin pozisyonda ve 16 kanallı ''body coil'' kullanılarak elde edildi. Hastaların görüntüleri klinik iş istasyonunda (iMac Pro retina, Apple) Osirix DİCOM Viewer kullanılarak değerlendirildi. ADC haritası ve infaz görüntülerde, her iki böbrek hilus düzeyinden geçen kesitlerden birinci yöntemde tüm renal parankimi kapsayacak şekilde elle bir ROİ çizilerek ve ikinci yöntemde ise yine bu kesitte medulla-korteks ayrımını en net şekilde değerlendirebildiğimiz düzeyden, (medulla ve korteksi içerecek şekilde) küçük bir ROİ çizilerek sinyal intensite hesaplamaları yapıldı. Bu çizilen ROİ'ler aynı kesimden ölçümler yapabilmek amacıyla kopyalanıp diğer kimyasal şift sekanslarına yapıştırılmıştır. Yapılan istatiksel analiz sonucunda KBY' li gurupta, sağlıklı guruba göre ADC değerleri ve dalak-böbrek infaz out-of-faz oranları istatiksel olarak anlamlı şekilde düşük bulunmuştur. Hastalık süresi, diyabeti olup olmaması, insülin kullanımı ve diyalize girip girmemesine göre sınıflandırmalar yapıldı. Bunlardan KBY'li popülasyonda sadece diyaliz durumuna göre yapılan sınıflandırmada, diyalize girenlerde girmeyenlere göre ADC değerlerinde anlamlı düşüklük tespit edilmesine karşın dalak-böbrek CSI oranlarında diyaliz giren ve girmeyen gurupta anlamlı farklılık saptanmamıştır. FF ölçümlerinden elde edilen değerlerde hiçbir sınıflandırmada anlamlı fark tespit edilememiştir. İlginç bir bulgu olarak birinci yöntem ve ikinci yöntem sonuçları karşılaştırıldığında, KBY hastası olan gurupta anlamlı fark izlenmezken, sağlıklı gurupta birinci yöntemde, ikinci yönteme göre ADC değerleri anlamlı derecede düşük izlenmiştir. Sonuçta diyabetik nefropatide renal steatoz ve lipotoksite tezini MRG'de yağ görüntüleme teknikleriyle destekleyecek bulgular tespit edemedik. Fakat yapılan diğer çalışmalara paralel olarak, KBY'li hasta gurubunda sağlıklı guruba göre ADC değerlerini anlamlı olarak düşük tespit edildi. Ayrıca ölçülen ADC değerleri ile böbrek-dalak CSI ratio değerleri arasında pozitif korelasyon tespit edildi. Ölçüm metodları arasından birinci yöntemin daha sağlıklı veri sunabileceğini tezi sunuldu. Findings of Diffusion MRI And Chemical Shift MRI in Choronic Kidney Disease The aim of this study is to compare diabetes and dialysis status, the duration of CKD (Chronic Kidney Disease), medication-induced changes in ADC (Apparent Diffusion Coefficient) and chemical shift MRI findings in study groups comprised of CKD and non CKD patients, while evaluating the thesis of fatty degeneration and lipotoxicity in diabetic nephropathy using MRI techniques featuring chemical shift and Dixon sequences. MRI monitoring was made using a machine with 1.5 Tesla magnetic field power (MAGNETOM Avanto, Siemens Medical Solutions, Erlangen, Germany). MRI presentations of the upper abdominal area were acquired in supine position using a 16 channel "body coil". Presentations were evaluated on a clinical workstation (iMac Pro retina, Apple) using Osirix DICOM Viewer. Two different methods were employed to measure signal intensity on ADC map and in-phase images: In the first method, a ROİ was drawn by hand over sections on renal hilus to include the whole renal parenchyma; while in the second, a short ROİ was drawn over the same section to include medulla and cortex at a level where we can most clearly differentiate the two. These ROİ's were copied and pasted on other Dixon sequences with a view to making measurements on the same section. The statistical analysis finally showed significantly low ADC values and in phase out-of-phase rates of spleen and kidney in CKD group compared to non-CKD group. Classifications were made based on the duration of disease, diabetes status, use of insulin, and dialysis status (whether the patient receives dialysis or not). Under the classification based on dialysis status, significantly low values were noted in patients who receive dialysis compared to those who don't. No significant difference under any classification was noted in values acquired from FF measurements. An interesting finding was that when results from the first and second method were compared, no significant difference was observed in CKD patients; while the healthy group showed significantly low ADC values in the first method compared to the second. As a result, we couldn't come up with any finding that supports the thesis of renal steatosis and lipotoxicity in diabetic nephropathy. However, in parallel with other studies carried out in the past, significantly low ADC values were detected in CKD patients compared to the healthy group. Moreover, a positive correlation was found between ADC values and kidney-spleen CSI ratio values. It was propounded that the first method could produce more healthy data than the second.
