Pediatrik popülasyonda morbiditeye sebep olan patolojiler arasında ilk sırada enfeksiyon hastalıkları gelmektedir. Üst solunum yolu enfeksiyonları arasında en sık karşılaşılan tablo akut tonsillofarenjittir. Üç yaş altındaki tonsillofarenjitlerin büyük çoğunluğu virüslere bağlıdır. A grubu beta hemolitik streptokoklar akut tonsillofarenjite sebep olan ve tonsillofarenjit etkeni olarak virüslerden sonra en sık karşılaşılan mikroorganizmalardır, 5-15 yaş grubunda görülen tüm bakteriyel tonsillofarenjitlerin %40-50'sini oluşturur. Özellikle 5-15 yaş arası okul çocuklarının akut tonsillofarenjitinde, AGBHS %30-37 oranında etken iken bu oran 5 yaş altında %5-10'a düşer. AGBHS sadece insanda enfeksiyon yapar ve hasta olan kişiden damlacık yolu ile bulaşır. AGBHS tanısında boğaz kültürü altın standarttır. Eğer klinik olarak AGBHS düşünülen bir olguda hızlı test pozitifse boğaz kültürü istenmez. Eğer hızlı test negatif kalırsa boğaz kültürü alınmalı ve sonucu beklenmelidir. Difteri, tularemi ve Vincent Anjini gibi nadir görülen hastalıkların dışında antibiyotik tedavisinin gerekli olduğu bakteriyel tonsillofarenjitlerde en sık etken AGBHS olduğu için tanıda asıl amaç AGBHS tonsillofarenjitlerinin saptanmasıdır. Bu amaçla McIsaac ve Centor'un yapmış olduğu klinik çalışmalar doğrultusunda streptokoksik tonsillofarenjiti öngörmek için bazı klinik skorlama sistemleri geliştirilmiştir. AGBHS tonsillofarenjitlerinin tedavisinin başarısız olması halinde, akut eklem romatizması ve glomerülonefrit gibi ciddi sorunlarla karşılaşılabilmektedir. Bu çalışmada 5-15 yaş grubu çocuklarda, AGBHS tonsillofarenjit oranı %22 olarak bulundu. AGBHS tonsillofarenjiti tanısı alan hastalarda oral penisilin (10 gün) veya depo penisilin (tek sefer) reçete edilmelidir. Alerjisi olanlarda ise makrolid grubu antibiyotikler tercih edilen ajanlar olup, eritromisin 10 gün süre ile ya da azitromisin 5 gün süre ile kullanılmalıdır. Tedaviye 9 gün içerisinde başlanması akut romatizmal ateş gelişmesini önler. Literatür ışığında 3 yaş üzeri semptomatoloji ve fizik muayene bulguları bakteriyel enfeksiyonla uyumlu tonsillofarenjitlerde, kültür testi uygulanıp sonucu beklenerek antibiyoterapiye karar verilmesi gerektiği sonucuna varılmıştır. The most common table that encountered among the upper respiratory infections is acute tonsillopharyngitis. A great majority of tonsillopharyngitis' below age 3, depends on viruses.Group A beta-haemolytic streptococci (AGBHS) are the most common microorganisms, which cause acute tonsillopharyngitis and is a factor of tonsillopharyngitis, after viruses. This microorganisms create %40-50 of all bacterial tonsillopharyngitis' which is seen between ages 5-15. Especially on school childs'(five-fifteen) acute tonsillopharyngitis, AGBHS is active add the rate of %30-37 but below the age 5, this rate declines to %5-10. AGBHS causes infection just on human being and infects from the sick (or case) by using the way of droplet On the diagnosis of AGBHS, throat culture is the golden standart. If the quick test is positive in a case which clinically considered as AGBHS, there is no need to throat culture. If the quick test stays negative, then throat culture must be taken and be examined. Because the AGBHS is the sharpest factor on bacterial tonsillopharyngitis' which antibiotic treatment is a must, except for the rare a disease like diphteria, tularemia and Vincent's Angina, original purpose, in diagnosis is the determining of the AGBHS tonsillopharyngities. For this purpose, in line whith the clinical trials of McIsaac and Centor, clinical scoring methods were developed for predict streptococcic tonsillopharyngitis. In case of the failure of AGBHS tonsillopharyngitises' treatments, can be encountered with serious ailments like acute articular rheumatism and glomerulonephritis. In this study, the rate of AGBHS tonsillopharyngitis among the children 5-15 was found as %22. On the patients who get the diagnosis of AGBHS tonsillopharyngitis, it must be prescribed oral penicillin (10 days) or repository penicillin (single use). On allergic patients, for macrolide antibiotics are favoured agents, erithromycin must be used for a period of 10 days or azithromycin must be used for a period of 5 days. To begin the treatment in 9 days, prevent the acute rheumatic fever. Under the lights of literatüre, it concluded that in tonsillopharyngities which symptomatology and physicial findings are competible with bacterial infection, above 3 years of age; it must be tested out the culture test, and according to its result it must be decided to antibiotherapy.
