Amaç: Bu çalışmada, oral izotretinoin kullanan akneli ergenlerde yaşam kalitesi, depresyon, anksiyete, intihar, sosyal kaygı ve obsesif kompulsif belirti düzeylerinin tedavi başlangıcı ve tedavinin 3. ayı sonunda sistemik antibiyotik kullanan kontroller ile karşılaştırılarak değerlendirilmesi amaçlandı. Yöntem: Çalışmaya Süleyman Demirel Üniversitesi Dermatoloji Polikliniğine 1 yıllık süre içinde başvuran ve orta veya şiddetli aknesi olan 15-18 yaş arası toplam 102 hasta dahil edildi. Bu hastalar oral izotretinoin ve sistemik antibiyotik tedavisi açısından değerlendirilerek izotretinoin (n=60) ve antibiyotik (n=42) olmak üzere iki gruba ayrıldı. Her iki gruba da Global Akne Derecelendirme Sistemi (GADS), Görsel Analog Skala (GAS), Akne Yaşam Kalitesi Ölçeği (AYKÖ), Hastane Anksiyete ve Depresyon Ölçeği (HADÖ), İntihar Olasılığı Ölçeği (İOÖ), Liebowitz Sosyal Kaygı Ölçeği (LSKÖ), Maudsley Obsesif Kompulsif Soru Listesi (MOKSL) tedavi başlangıcı ve tedavinin 3. ayı sonunda olmak üzere toplamda 2 kez uygulandı. Akneye eşlik edebilecek komorbid psikiyatrik bozuklukları olan olguları çalışma dışında tutmak için her iki grup çalışma başlangıcında Okul Çağı Çocukları için Duygulanım Bozuklukları ve Şizofreni Görüşme Çizelgesi-Şimdi ve Yaşam Boyu Versiyonu (ÇGDŞ-ŞY) ile değerlendirildi. Sonuçlar: Yaş ortalaması ve cinsiyet dağılımı açısından izotretinoin ve antibiyotik grupları arasında anlamlı bir farklılık saptanmadı. Çalışmamıza dahil edilen akne vulgarisli olguların yaklaşık %60’ını kız ergenler oluşturmaktaydı. Ortalama akne süresi ve çalışmaya dahil edilmeden önce ortalama tedavi alma süresinin izotretinoin grubunda antibiyotik grubuna göre anlamlı olarak daha uzun olduğu saptandı. Ayrıca izotretinoin grubundaki akneli ergenlerin antibiyotik grubundaki akneli ergenlere göre tedavi takiplerine anlamlı olarak daha düzenli geldikleri belirlendi. Her iki grupta da tedavi öncesine göre tedavinin 3. ayı sonunda ortalama GADS ve GAS puanlarında anlamlı bir düşüş gözlendi. Her iki grupta da tedavi öncesi ve sonrası depresyon, anksiyete ve intihar düzeyleri arasında anlamlı bir farklılık yoktu. Diğer yandan her iki grupta da tedavi öncesine göre tedavinin 3. ayı sonunda yaşam kalitesi, sosyal kaygı ve obsesif kompulsif belirtilerde anlamlı bir düzelme olduğu görüldü. Bunlara ek olarak izotretinoin grubunda tedavi öncesinde yaşam kalitesi, sosyal kaygı/kaçınma ve ruminasyon düzeyleri arasında anlamlı bir korelasyon olduğu tespit edildi. Tartışma: Çalışma sonucunda akneli ergenlerde izotretinoinin depresyon, anksiyete ve intihar gelişimi ile ilişkili olmadığı düşünüldü. Ayrıca etkin izotretinoin tedavisinin yaşam kalitesi, sosyal kaygı ve obsesif kompulsif belirtilerde iyileşme sağladığı gösterildi. Çalışmamızın ve literatürdeki diğer çalışmaların verileri göz önüne alındığında, izotretinoinin karmaşık merkezi sinir sistemi etkilerinin anlaşılabilmesi için olgu sayısının daha fazla olduğu ve takip süresinin daha uzun olduğu geniş kapsamlı çalışmalara ihtiyaç vardır. Purpose: In this study, we aimed to evaluate the levels of quality of life, depression, anxiety, suicide, social anxiety, and obsessive-compulsive symptoms at baseline and 3 months in adolescents receiving oral isotretinoin by comparing adolescents receiving systemic antibiotics. Methods: This study included a total of 102 patients between the ages of 15 and 18 who were admitted to the Dermatology Polyclinic of Suleyman Demirel University within the 1-year period and who had moderate or severe acne. These patients were divided into two groups as isotretinoin (n=60) and antibiotic (n=42) after evaluating for oral isotretinoin and systemic antibiotic treatment. The Global Acne Grading System (GAGS), Visual Analogue Scale (VAS), Acne Quality of Life Scale (AQLS), Hospital Anxiety and Depression Scale (HADS), Suicide Probability Scale (SPS), Liebowitz Social Anxiety Scale (LSAS), and Maudsley Obsessive-Compulsive Question List (MOCQL) were administered to both groups at baseline and 3 months. In order to exclude cases with comorbid psychiatric disorders that can accompany acne, both groups were evaluated with the Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version (K-SADS-PL) at the beginning of the study. Results: There were no significant differences in the mean age and gender distribution between the two groups. Female adolescents accounted for about 60% of cases of acne vulgaris included in our study. Both mean duration of acne and mean duration of previous acne treatment were found to be significantly longer in the isotretinoin group than in the antibiotic group. It was also determined that those in the isotretinoin group more regularly attended appointments than those in the antibiotic group. In both groups, there were significant decreases in the mean GAGS and VAS scores at 3 months compared with baseline. In both groups, there were no significant differences between pre- and post-treatment levels of depression, anxiety, and suicide. On the other hand, there were significant improvements in the quality of life, social anxiety, and obsessive-compulsive symptoms at 3 months compared with baseline in both groups. In addition, it was found that there was a significant correlation between baseline levels of quality of life, social anxiety/avoidance, and rumination in the isotretinoin group. Discussion: Consequently, it was thought that isotretinoin was not associated with the development of depression, anxiety, and suicide in adolescents with acne. Moreover, effective isotretinoin treatment was shown to improve the quality of life, social anxiety, and obsessive-compulsive symptoms. When considering the data obtained from our study and other studies in the literature, there is a need for extensive studies with a larger number of cases and with a longer follow-up period for understanding the complex effects of isotretinoin on the central nervous system.
