GİRİŞ: KML, 100.000 yetişkinde 1-2 vaka insidansı olan myeloproliferatif bir neoplazm olup erişkin lösemilerinin %15’ini oluşturur. KML, miyeloproliferatif neoplazm olan bir hastada Philadelphia kromozomunun varlığı ile tanımlanır. KML tedavisi, tirozin kinaz inhibitörlerinin BCR-ABL1 onkoprotein ve adenosin trifosfat arasındaki etkileşimi bloke ederek malign klonun proliferasyonunu engelleyerek değişmiştir. Bu hedeflenmiş yaklaşım, KML’nin doğal tarihini değiştirdi ve 10 yıllık sağ kalım oranını yaklaşık %20 ile %80-90 arasında arttırdı. KML hastalarında beklenen uzun sağ kalım süresi ve ömür boyu tedavi uygulanması nedeniyle tedavide maliyeti hasta yönetiminde göz önünde bulundurulmalı dirençli olgularda etkin tedavi değişikliği zamanında yapılması gerekmektedir. AMAÇ: TKİ ile tedavi altında olan KML hastalarımızda etkin tedavi değişikliği zamanında yapılıp yapılmadığı, tedavi yanıt oranları sağ kalım verilerinin güncel literatür ile benzer olup olmadığının araştırılması amaçlanmıştır. YÖNTEM: Süleyman Demirel Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı’nda 2000-2018 yılları arasında KML tanısı ile takip edilen 18 yaş üzeri KML hastaları çalışmaya alındı.. Veriler retrospektif olarak dosya ve sistem kayıtlarından toplandı. Periferik kan yayması ve kemik iliği incelemesi ile birlikte karyotip analizinde Ph kromozomu varlığının ve/veya polimeraz zincir reaksiyonu yöntemi ile BCR-ABL pozitifliğinin saptandığı hastalar çalışmaya alındı. Klinik ve laboratuar verileri yeterli olan 58 KML hastası çalışmaya alınmıştır. SONUÇLAR: Çalışmamızda 58 hastanın 32 tanesi (% 55) erkek, 26 tanesi (% 45) kadındı. Hastaların yaş dağılımı 18-89 arasında değişmekte olup ortalama yaş 52 idi. Toplam 58 hastanın 18’inde (% 31) 2. Basamak TKI’ye geçildi. Ortalama takip süresi 88 aydır. (6-264) Takipte hastaların 11 tanesi (% 19) eksitus olmuştur. Çalışmamızda GS % 81 olarak saptandı. Hastaların 13 tanesinde (% 22) progresyon gelişmiş bu hastalardan 3 tanesi blastik evre KML’ye bağlı eksitus olmuştur. Progresyonsuz sağkalım % 78 olarak saptandı. Hastalıksız sağkalım % 95 olarak saptandı. Sokal risk skorunun düşük, orta ve yüksek olanlarda GS sırasıyla % 92,9 , % 78,4 ve % 80; progresyonsuz sağkalım sırasıyla % 78 % 85 ve % 60; hastalığa bağlı sağkalım % 100, % 100 ve % 80 olarak saptanmıştır. Çalışmamızda retiküler lif derecesine göre genel sağkalım grade 0=90.9 grade 1=%75 grade 2= % 75 grade 3= % 77.8 grade 4 = % 100 olarak saptandı. TARTIŞMA VE SONUÇ: Çalışamamızda literatürle benzer yaş ortalaması, erkek/kadın oranı saptandı. 2. basamak TKİ’ye geçişin en sık nedeni yan etkiydi. OS tahminleri, CCAİ’si 0, 1 -2, 3 -4 , ≥5 olan hastalarda yüzde 100, 84, 73 % 20 olarak saptandı. Bu sonuçlar, KML'nin genellikle mevcut tedavilerle iyi yönetildiğini ve ölümlerin diğer tıbbi problemler nedeniyle daha sık meydana geldiğini göstermektedir. Sokal risk skorunun düşük risk olanlarda sağkalımın arttığını ve risk skorunun artmasının sağkalımı azalttığını göstermiştir. Sokal risk skorunun hasta sayımızın az olması veya günümüzde TKI tedavileri nedeniyle istatistiksel olarak anlamlı etki etmediği düşünülmüştür. Hastalarda retiküler lif derecesinin genel sağkalım, hastalıksız sağkalım ve progresyonsuz sağkalıma literatürle benzer şekilde istatistiksel olarak anlamlı etki etmediği saptanmıştır. Retiküler lif derecesinin KML’li hastalar arasındaki olumsuz prognostik etkisi TKİ tedavisiyle ortadan kalktığı düşünülmüştür. Çalışmamızda literatürle benzer şekilde sağkalım oranları saptanmıştır. Çalışmamızda yaş dağılımının (p=0,001) Charlson komorbidite İndeksi’nin (p=0,005) ve Charlson komorbidite-yaş İndeksi’nin (p=0,000) genel sağkalıma istatistiksel olarak anlamlı etki ettiği görülmüştür. Çalışmamızda yaş dağılımının (p = 0.029), Charlson komorbidite yaş İndeksi (p = 0,001 ) ve 12. ayda MMR alınmasının (p = 0,028 ) hastalıksız sağkalıma istatistiksel olarak anlamlı etki ettiği görülmüştür. Çalışmamızda sadece 12. Ayda MMR (p=0,006) progresyonsuz sağkalıma istatistiksel olarak anlamlı etki ettiği görülmüştür. Sonuç olarak KML hastalarında sitogenetik ve moleküler izlemlere uyularak, zamanında ve hastanın komorbid durumlarını da gözeterek yapılan tedavi değişiklikleri ile literatüre destekler düzeyde uzun dönem sağ kalım ve hastalıksız sağkalım elde edilebilmektedir. Ancak iyi bir hasta izlemi için sitogenetik ve moleküler izleme özen gösterilmelidir. INTRODUCTION: Chronic myeloid leukemia is a myeloproliferative neoplasm with an incidence of 1–2 cases per 100 000 adults. It accounts for approximately 15 % of newly diagnosed cases of leukemia in adults. Central to the pathogenesis of CML, is the fusion of the Abelson murine leukemia (ABL1) gene on chromosome 9 with the breakpoint cluster region (BCR) gene on chromosome 22. The CML therapeutic landscape changed dramatically with the development of the small molecule tyrosine kinase inhibitors (TKIs) that potently interfered with the interaction between the BCR-ABL1 oncoprotein and adenosine triphosphate (ATP), blocking cellular proliferation of the malignant clone. This “targeted” approach altered the natural history of CML, improving the 10-year survival rate from approximately 20% to 80%-90%. Because of long expected survival in cml patients and needs to be treated for life; the cost of treatment should be considered and in resistant cases, effective treatment changes must be done at the right time. PURPOSE: This study aims to investigate that effective management of tki treatment and overall survival in CML patients. Additional aim is to discuss the results with current literature. METHOD: In this study, 58 patients who were diagnosed as CML between 2000 and 2018 by Süleyman Demirel University Department of Internal Medicine, Hematology Department were evaluated. This study included 58 cml patient who are treating with tyrosine kinase inhibitors the clinical, demographıc and treatment response properties of these patients were evaluated. With this retrospective study, patients’clinical and laboratory characteristics, , treatment options, side effects and responses were evaluated. All the patients evaluated due to demografical and clinical features. Risk assesment and staging applied with World Health Organization criteria and Sokali Hasford and Eutos risk scoring system. We determined hematological, cyotgenetic and moleculer response according to European Leukemia Network criteria. RESULTS: In this study, 32 (55%) of 58 patients were male and 26 (45%) were female. The mean age of the patients ranged from 18 to 89 years and the mean age was 52 years. In 18 of the 58 patients (31%), TKI was performed in step 2. The mean follow-up period was 88 months. (6-264) During follow-up, 11 patients (19%) died. In our study, GS was found to be 81%. Of these patients, 13 (22%) developed progressive progression and 3 of them died due to blastic stage CML. Progression-free survival was 78%. The disease-free survival was 95%. The low risk, moderate and high risk of GS were 92.9%, 78.4% and 80%, respectively; progression-free survival was 78% 85% and 60%, respectively; disease-related survival was 100%, 100% and 80%. In our study, overall survival according to reticular fiber grade was 0 = 90.9 grade 1 = 75% grade 2 = 75% grade 3 = 77.8% grade 4 = 100%. DISCUSSION AND CONCLUSION: In our study, the average age and male / female ratio were found similar to the literature. The second most common cause of the transition to TKI was side effects. OS estimates were 100, 84, 73, 20% in patients with CCAI 0, 1 -2, 3 -4, ≥5. These results suggest that CML is generally well managed with existing treatments and that deaths occur more frequently due to other medical problems. It has been shown that survival of patients with low risk of Sokal risk score increases and the increase of risk score decreases survival. Sokal risk score was considered not to be statistically significant due to the low number of patients or TKI therapies. The overall survival, disease-free survival and progression-free survival of patients with reticular fiber were not statistically significant. The negative prognostic effect of retinal fiber grade between CML patients was thought to have disappeared with TKI treatment. In our study, survival rates were found similar to those in the literature. In our study, it was observed that the age distribution (p = 0.001) and Charlson comorbidity index (p = 0.005) and Charlson comorbidity-age index (p = 0.000) had a statistically significant effect on overall survival. In our study, age distribution (p = 0.029), Charlson comorbidity age index (p = 0.001) and MMR at 12 months (p = 0.028) were found to have a statistically significant effect on disease-free survival. In our study, it was observed that MMR (p = 0.006) had a statistically significant effect on progression-free survival only at 12 months. As a result, CML patients long- term survival and disease-free survival can be obtained as a current literature data, with effective monitoring cytogenetic- molecular response and treatment changing at the rigt time with considering the patient co-morbidity.
