İş kazaları önemli bir mortalite ve morbidite sebebi olmasının yanı sıra, meydana getirdiği tedavi giderleri ve iş gücü kayıpları sebebi ile ekonomik açıdan ciddi yüklere sebep olmaktadır. Bu çalışmanın amacı, hastanemize başvuran iş kazalı hastalarda demografik bilgiler, yaralanmaların özellikleri, iş kazasının hangi çalışma saati diliminde olduğu, yatış durumları, acil servis maliyetleri ve bunlar arasındaki ilişkiyi incelemektir. 01.01.2017 – 31.12.2017 tarihleri arasında hastanemiz acil servisine başvuran 18 yaş ve üzeri iş kazalı 323 hasta çalışma kapsamına alındı. Veriler Hastane Yönetim Bilgi Sistemi (Enlil) ve hasta dosyalarının geriye dönük olarak incelenmesiyle elde edildi. Çalışma grubunun yaş ortalaması 34,03 ± 9,8 (18-63) idi. Olguların %89,8’i erkekti. Yaralanmalar en sık 28-47 yaş grubunda (%62,9) görüldü. Acil Servis başvurularında yıl içinde aralık ayında, hafta günlerine dağılımda Salı günleri, gün içinde ise, 08.00-16.00 saatleri arasında artış görüldü. Olguların %24,1’i inşaat alanında çalışmaktaydı. Yaralanmaların %66,6’sının işe başladıktan sonraki ilk dört saat içerisinde olduğu ve en fazla üst ekstremitenin etkilendiği belirlendi. Çalışma grubunda taburculuk oranı %69,0 idi. En fazla konsültasyon ve yatış işlemi Plastik ve Rekonstrüktif cerrahi ile yapıldı. Ortalama Acil Servis maliyeti 119,40 ± 115,38 idi. Eğitim durumu azaldıkça kazanın maliyetinin anlamlı olarak arttığı saptandı. Yaralanan vücut kısmı, acil servis sonlanımı ve hasarın BTM (basit tıbbi müdahale) ile giderilme durumu da maliyet ile ilişkili olarak belirlendi. İş kazalarının ve kaza neticesinde oluşacak kayıpların azaltılabilmesi için koruyucu faktörlerin yanı sıra, yaralanmaya yatkınlık oluşturacak etmenlerin de sorgulanması ve kaydedilmesi gereklidir. İş kazalarına karşı önlem almanın daha ucuz ve daha insani olduğu açıkça gözlenmektedir. Anahtar Kelimeler : İş Kazası, Acil Tıp, Maliyet.
Besides being an important cause of mortality and morbidity, occupational accidents lead to serious economic burdens because of treatment costs and labor losses it causes. The aim of this study is to investigate the demographic information of the patients admitted to our hospital because of occupational accidents, features of the injuries they had, the time of the occupational accident, hospitalization situations, emergency department costs, and the relations among these variables. In the study, 323 patients who were 18 or older and who were admitted to the emergency department of our hospital between 01.01.2017-31.12.2017 because of occupational accidents were included. The data was obtained from the Hospital Management Information System (Enlil) and through a retrospective investigation of the patient folders. Mean age of the study group was 34,03 ± 9,8 (18-63). 89,8% of the cases involved men. Most of the injuries were in the 28-47 age-group (62,9%). There was an increase in the number of admissions to the Emergency Service each year in December, each week on Tuesday, and between 8 am and 4 pm during the day. 24,1% of the cases were related to construction work. 66,6% of the injuries occurred during the first four hours of work; and most often the injuries were in the upper extremities. 69,0% of the study group were discharged from the hospital. Consultation and hospitalization were highest in Plastic and Reconstructive surgery. Average Emergency Service cost was 119,40 ± 115,38. Oy was observer that there was a significant increase in costs of accidents as education level decreased. Injured part of the body, emergency department outcome, and SMT (simple medical treatment) were also found to be related to the cost. To be able to decrease the number of occupational accidents and the losses happening as a result of them, along with the protective factors, the factors which create a tendency to get injured should be investigated and recorded as well.
