Bu çalışmanın amacı Isparta ve Antalya illerinde geleneksel ürün satılan market ve pazarlardan temin edilen toplam 100 çiğ süt (10) ve süt ürününde (70 peynir, 10 tereyağ, 10 yoğurt) yüksek seviyede aminoglikozid dirençli (YSAD) Enterococcus yaygınlığı ve aminoglikozid-modifiye edici enzim (AME) genleri varlığının araştırılmasıdır. Ayrıca, izolatların antibiyotik direnç profilleri ile gentamisin ve streptomisin minimum inhibisyon konsantrasyonları (MİK) da belirlenmiştir. Tez çalışması kapsamında 24 örnekten (2 çiğ süt, 22 peynir) toplam 59 YSAD Enterococcus suşu izole edilmiştir. 59 YSAD izolatının 20'si yüksek seviyede gentamisin dirençli (YSGD) ve 39'u yüksek seviyede streptomisin dirençli (YSSD) dir. Moleküler yöntemler ile (türe özgü PZR ve 16S rDNA dizi analizi) izolatların 26'sı E. faecalis (% 44.07), 18'i E. faecium (% 30.51), 13'ü E. durans (% 22.03) ve 2'si E. gallinarum (% 3.39) olarak tanımlanmıştır. Disk difüzyon testi sonucu YSAD Enterococus izolatlarının en duyarlı olduğu antibiyotiğin teikoplanin, en dirençli oldukları antibiyotiğin ise tetrasiklin olduğu saptanmıştır. İzolatlar arasında antibiyotiklere en dirençli türün E. faecium olduğu tespit edilmiştir. İzolatların % 89.83'ü (53/59) çoklu antibiyotik direncine (en az üç antibiyotiğe karşı dirençli) sahip bulunmuştur. YSAD Enterococcus izolatlarının streptomisin ve gentamisin antibiyotiklerine karşı MİK değerlerinin 64 ile >4096 µg/mL arasında değiştiği belirlenmiştir. 59 YSAD Enterococcus izolatının 47'sinin (% 79.66) hem YSSD hem de YSGD oldukları tespit edilmiştir. YSAD Enterococcus izolatlarında en sık rastlanan aminoglikozid-modifiye edici enzim (AME) geninin % 94.92 (56/59) ile aph(3ʹ)-IIIa olduğu tespit edilmiştir. Bu geni sırasıyla % 45.76 (27/59) ile ant(6ʹ)-Ia, % 20.34 (12/59) ile aph(2ʹʹ)-Ic ve % 10.17 (6/59) ile ant(4ʹ)-Ia'nın izlediği belirlenmniştir. İzolatlarının hiçbirinde aac(6ʹ)-Ie-aph(2ʹʹ)-Ia, aph(2ʹʹ)-Ib ve aph(2ʹʹ)-Id genlerine rastlanılmamıştır. PCR denemeleri sonucu E. durans RG20.1, E. faecalis RG22.4 ve RG26.1 izolatlarında AME geni varlığı tespit edilmemiştir. Sonuç olarak bu çalışma, çiğ süt ve peynir örneklerinde YSAD Enterococcus varlığını göstermiştir. YSAD Enterococcus izolatları yüksek seviyede aminoglikozid direnci ile ilişkili genlerin yayılmasında rezevuar görevi görebilir. The aim of this study is to investigate the prevalence of high-level aminoglycoside resistant (HLAR) Enterococcus in 100 raw milk (10) and dairy products (70 cheese, 10 butter, 10 yoghurt) obtained from traditional markets and bazaars in Isparta and Antalya; and to investigate the presence of aminoglycoside-modifying enzyme (AME) genes. In addition, the antibiotic resistance profiles of the isolates and gentamicin and streptomycin minimum inhibition concentrations (MIC) were also determined. Within the scope of thesis study, a total of 59 HLAR Enterococcus strains were isolated from 24 samples (2 raw milks, 22 cheeses). 20 of 59 HLAR isolates are high level of gentamicin resistant (HLGR) and the rest 39 is high level streptomycin resistance (HLSR). Of the isolates with molecular methods (species-specific PCR and 16S rDNA sequence analysis), 26 were defined as E. faecalis (44.07%), 18 as E. faecium (30.51%), 13 as E. durans (22.03%) and 2 were defined as E. gallinarum (3.39%). As a result of disc diffusion test, teicoplanin is the antibiotic to which HLAR Enterococus isolates are the most sensitive; and tetracycline was the most resistant antibiotic. Among the isolates, E. faecium was found to be the most resistant species to antibiotics. 89.83% (53/59) of the isolates had multiple antibiotic resistance (resistant to at least three antibiotics). MIC value of HLAR Enterococcus isolates against streptomycin and gentamicin antibiotics ranged from 64 to >4096 µg/mL. It was determined that 47 (79.66%) of 59 HLAR Enterococcus isolates were both HLSR and HLGR. The most common aminoglycoside-modifying enzyme (AME) gene in Enterococcus isolates was aph(3ʹ)-IIIa with 94.92% (56/59). None of the isolates contained the aac(6ʹ)-Ie-aph(2ʹʹ)-Ia, aph(2ʹʹ)-Ib and aph(2ʹʹ)-Id genes. As a result of PCR experiments, the presence of AME gene in E. durans RG20.1, E. faecalis RG22.4 and RG26.1 isolates was not detected. In conclusion, this study showed the presence of HLAR Enterococcus in raw milk and cheese samples. HLAR Enterococcus isolates can act as resorbers in the dissemination of genes associated with high levels of aminoglycoside resistance.
