Amaç: Bu çalışmanın amacı akut travmatik el yaralanmasının erken iyileşme döneminde hastaların uyku kalitesinin araştırılması ve uyku bozukluğu ile ilişkili faktörlerin saptanmasıdır. Metod: Travmatik el yaralanması geçirip cerrahi sonrası akut dönemde rehabilitasyon polikliniğine başvuran 69 hasta çalışmaya alındı. Psikiyatrik hastalık öyküsü ve uykuya etkisi bilinen ilaç kullanımı olanlar çalışmaya dahil edilmedi. Hastalar sosyodemografik veriler, yaralanma özellikleri, operasyon bilgileri, uykuyu etkileyebilecek kişisel alışkanlıklar, eşlik eden hastalıklar, orteze bağlı nedenler, ağrı, elini eskisi gibi kullanamama endişesi ve psikiyatrik faktörlerin uyku kalitesine etkisi açısından değerlendirildi. Yaralanma şiddeti Modifiye El Yaralanması Ciddiyet Skoru (MEYCS) ile belirlendi. Uyku kalitesi Pittsburgh Uyku Kalitesi İndeksi (PUKİ) ile değerlendirildi. Depresyon, anksiyete ve travma sonrası stres bozukluğu (TSSB) semptomları sırasıyla Beck Depresyon Ölçeği (BDÖ), Beck Anksiyete Ölçeği (BAÖ) ve TSSB-Soru Listesi-Sivil Versiyonu ile değerlendirilerek takip edildi. Hastalar cerrahi sonrası ilk üç gün içinde ve cerrahi sonrası birinci ve ikinci aylarda prospektif olarak değerlendirildi. Bulgular: Hastaların PUKİ ortalamaları tüm değerlendirmelerde normal sınırlarda idi. Uyku kalitesi bozukluğu sıklığı ilk başvuruda %16,7 yaralanmadan sonraki birinci ayda %21,6, ikinci ayda %19,5 idi. Cerrahi sonrası ilk üç günde ağrı (p=0,030), birinci ayda depresyon (p=0,008) ve K-teli ile fiksasyon varlığı (p=0,027), ikinci ayda depresyon (p=0,003), anksiyete (p=0,028), ortezin uykuyu bozduğunu düşünme (p=0,028), ortez nedeniyle uyanma (p=0,031) ve elini eskisi gibi kullanamama endişesinin (p=0,023) uyku kalitesi bozukluğu için risk faktörleri olduğu saptandı. Regresyon analizi sonrasında birinci ayda depresyon varlığının uyku kalitesi bozukluğu gelişme riskini 9 kat arttırdığı saptanırken (p=0,014, OR=9,20, %95 CI=1,56-54,15), diğer risk faktörleri anlamlılıklarını devam ettiremedi (p>0,05). Sonuç: Akut travmatik el yaralanmalarından sonraki birinci ayda depresyon varlığı uyku kalitesini bozmaktadır. Anahtar Sözcükler : El yaralanması, uyku kalitesi, travma.
Purpose: The aim of this study is to evaluate the sleep quality and the factors related to sleep quality in patients with acute traumatic hand injury. Methods: A total of 69 patients with traumatic hand injury who presented to the rehabilitation outpatient clinic after surgical repair in the acute period were included in the study. Patients who were diagnosed of any psychiatric disorder and those who used any drug known to affect sleep were excluded. Patients were evaluated for sociodemographic data, injury characteristics, operative information, personal habits that may affect sleep, concomitant diseases, orthosis related problems, pain, anxiety of inability to reuse hand functionally, psychiatric factors and their effects on sleep quality. The severity of injury was determined by Modified Hand Injury Severity Score (MHISS). Sleep quality was evaluated with the Pittsburgh Sleep Quality Index (PSQI). Symptoms of depression, anxiety and posttraumatic stress disorder were evaluated with Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI) and Posttraumatic Stress Disorder Question List-Civilian Version, respectively. Patients were assessed prospectively within the first three days post operatively and at the first and second months after surgical repair. Results: Total scores of PSQI were within normal range at all assesments. The prevalence of sleep quality impairment was 16.6% at baseline, 21.6% at the first month, and 19.5% at the second month. Pain (p=0.030) within the first three days after surgical repair, presence of fixation with K-wire (p=0.027) and depression (p=0.008) at the first month, depression (p=0.003), anxiety (p=0.028), subjective orthosis related wakefulness after sleep onset (p=0.031), subjective orthosis related sleep disturbance (p=0,028) and anxiety of inability to reuse hand functionally (p=0.023) at the second month were found to be risk factors for sleep quality impairment. The presence of depression at the first month revealed a 9-fold increase in the risk of developing sleep quality impairment (p=0.014, OR=9.20, 95% CI=1.56-54.15), while other risk factors could not maintain their significance after regression analysis (p>0,05). Conclusion: Presence of depression in the first month after acute traumatic hand injuries disrupts sleep quality. Keywords : Hand injury, sleep quality, trauma.
