Enfeksiyon Servisinde Yatan Hastalarda İdrar Yolu Enfeksiyonu Etkenlerinin Prognoza Etkisi: Bir Klinik Çalışma Uygulaması Çalışmamızda, idrar yolları enfeksiyonu (İYE) tanısı ile enfeksiyon servisinde yatan ve tedavi gören hastalarda enfeksiyona etken olan mikroorganizmaların belirlenerek, uygulanan tedaviye bağlı olarak hastalığın sonucunu tahmin edebilme ve buna bağlı olarak da hastanede kalış süresine etki eden parametrelerin univariate, multivariate istatistik yöntemleri gibi kullanılarak sonuçların analiz edilmesi ve bir sonuca varılması amaçlandı. Retrospektif olarak yapılan bu çalışmada yetişkin grubu hedef alınıp kliniğe yatışı yapılan hastalardan alınan kan, idrar kültürü tetiklerine göre antibiyotik tedavisi yapılarak hastanın prognozu ve dolayısı ile hastanede kalış sürelerine olan etkiler incelenmiştir. Bu amaçla Isparta Şehir Hastanesi Enfeksiyon Hastalıkları Servisi'nde 21/03/2017-21/03/2018 tarihleri arasında İYE tanısıyla yatış yapılan ve araştırma kurallarına uygun 18-95 yaş arasında 124 olgunun verileri değerlendirildi. Araştırmamız sonucunda İYE'nin kadınlarda daha fazla gözlendiği (%54,8), yaş olarak 76-96 yaş döneminde daha sık rastlandığı (%44,4) ve en sık izole edilen mikroorganizmanın E. Coli (%37,4) olduğu görüldü. Hastanede kalış süresinin azalması üzerine kullanılan antibiyotik grubu etkisi incelendiğinde ise en fazla tercih edilen antibiyotik Karbopenemler (%55,6) olarak kullanılmasına rağmen Sefolosporin grubu antibiyotiklerle tedavi edilenlerin hastane kalış gün süresinin daha az olduğu gözlemlendi (5,46±3,30). Bu çalışma sonucunda elde edilen verilerin idrar yolu enfeksiyonları tanılarında hangi tedavi protokolünün uygun olduğunun daha iyi anlaşılması ve yapılan istatistik analizleriyle tedavi için geliştirilebilecek yöntemlere katkıda bulunabileceği düşünülmüştür.

The effect of different antibiotics use on prognosis of patients who hospitalized with urinary tract infection: a retrospective study The objective of this study was to determine the microorganisms that are responsible for patients with urinary tract infection (UTI), and to compare the parameters that affecting the prognosis and the duration of stay in hospital with several univariate, multivariate statistical methods, and finalize the conclusion. In this study, an adult group was targeted and they were given different antibiotics after collection of their blood and urine cultures during the hospital stay. We thus, investigated prognoses of the patients and their hospital stays via antibiotic usage. For this purpose, there were 124 people, who were aged between 18 and 95 year-old and hospitilized with the diagnosis of UTI between March 21, 2017 and March 21, 2018 in the Infectious Disease Service Department of Isparta City Hospital, Isparta. Our result showed that the UTI was more common in women (54.8%) and was more frequent in ages between 76 and 95 years (44.4%). E. Coli pathogenic strains were dominant among the other genera. We found that the most effective antibiotics were realized as Cephalosporins among the others for the shorter length of stay in hospital (5.46±3.30), although the Carbapenems (55.6%) were the most commonly prescribed by the physicians. As a result, we retrospectively identifed which type of antibiotics usage was appropriate for a better treatment of UTI, and therefore determined the length of hospitilization with statistical analyses.
