Amaç, lomber spinal stenoz (LSS)'un direk grafilere yansıyan nesnel parametrelere etkisini araştırmak, LSS tanısı için varolan pratik radyografik indeksleri uyarlamaktır. LSS, lumbosakral MRG'de aksiyel T2 ağırlıklı görüntülerde orta hat anteroposterior (AP) spinal kanal çapı ölçülerek saptandı. LSS'si olan (hasta grubu) 50 ve olmayan (kontrol grubu) 50 olgu şeklinde sınıflandı. LSS grubundaki ve kontrol grubundaki hastaların optimal şekilde çekilmiş olan lateral lumbosakral grafileri retrospektif olarak değerlendirildi ve grafilerde; pelvik insidans (Pİ), pelvik tilt (PT), sakral slope (SS), lomber lordoz (LL), L5 insidans (L5İ), L5 slope (L5S), sakral table açısı (STA), sagittal vertebra korpus yüksekliği (SBH), sagittal vertebra korpus genişliği (SBW), pedinkül genişliği (PW), foraminal genişlik (FW), arka pedinkül kenarı (PPM) ölçüldü, SBW/PW ve SBW/PPM oranları hesaplandı. LSS grubundaki kadın hastaların oranı (%72) erkeklerin oranından (%28) yüksek bulundu. LSS'li hastalarda yaş ortalaması anlamlı düzeyde yüksek bulundu (p<0,001). LSS grubunda Pİ anlamlı şekilde daha yüksek (p=0,029) bulundu. FW (p<0,001) ve PW (p<0,005) değerleri her seviye için dar kanal saptanan grupta anlamlı düzeyde düşük bulundu. LSS grubunda, yalnızca L4 seviyesinde SBH değeri anlamlı düzeyde düşük (p=0,041), SBW değeri L2-5 seviyelerinde anlamlı düzeyde yüksek (p<0,005), PPM değeri yalnızca S1 seviyesinde anlamlı düzeyde düşük bulundu (p<0,005). LSS grubunda, SBW/PW oranı her bir seviye için, SBW/PPM oranı L4 ve L5 seviyelerinde anlamlı düzeyde daha yüksek bulundu (p<0,005). LSS ve kontrol grubu arasında anlamlı farklılık bulunan parametrelere yapılan ROC analizinde, ROC eğrisi anlamlı bulunan parametre ve seviyelerin cut-off değerleri hesaplandı ve özellikle PW, FW değerleri ile SBW/PW oranının duyarlılık, özgüllük ve doğruluk yüzdeleri nispeten yüksek bulundu. Çalışmamızın sonuçları, belirtilen radyografik ölçümler spinal dar kanalı olan ve olmayan hastaları sınıflandırmak için yararlıdır, ancak bu konuyla ilgili daha fazla çalışma yapılması gerekmektedir. The aim of this study was to investigate the effect of lumbar spinal stenosis (LSS) on objective parameters reflected on radiographs and to adapt existing radiographic indexes to the diagnosis of LSS. LSS was determined by measuring the midline anteroposterior (AP) spinal canal diameter on axial T2-weighted images in lumbosacral MRI. 50 patients with LSS (patient group) and 50 patients (control group) were classified as. The optimally drawn lateral lumbosacral radiographs of the patients in the LSS group and in the control group were evaluated retrospectively and on radıographs, pelvic incidence (PI), pelvic tilt (PT), sacral slope (SS), lumbar lordosis (LL), L5 incidence (L5I), L5 slope (L5S), sacral table angle (STA), sagittal vertebral corpus height (SBH) , sagittal vertebral corpus width (SBW), peduncular width (PW), foraminal width (FW), posterior pedicle margin (PPM) were measured, SBW / PW and SBW / PPM ratios were calculated. The rate of female patients in the LSS group (72%) was higher than that of men (28%). The mean age was significantly higher in patients with LSS (p <0.001). In the LSS group, PI was significantly higher (p = 0.029). FW (p <0.001) and PW (p <0.005) values were significantly lower in the narrow channel group for each level. In the LSS group, SBH levels were significantly lower (p = 0.041) at L4 level, SBW levels were significantly higher at L2-5 levels (p <0.005) and PPM values were significantly lower at S1 level only (p <0.005). In the LSS group, the SBW / PW ratio for each level the SBW / PPM ratio at L4 and L5 levels were significantly higher(p <0.005). In the ROC analysis of parameters with significant differences between LSS and control groups, the cut-off values of significant parameters and levels were calculated and the sensitivity, specificity and accuracy percentages of PW, FW and SBW / PW ratios were found to be relatively high. The results of our study are useful for classifying patients with and without spinal narrow canals, but further studies are required.
