Akut miyokard infarktüsü (AMI), dünya genelinde yüksek ölüm oranlarına sahiptir. Ancak erken tanı ve tedavi ile prognozun yüksek oranda düzelmesi sağlanabilmektedir. Bu nedenle AMI ve akut koroner sendrom (AKS) acil serviste erken tanınması önemlidir. HEART skoru acil serviste göğüs ağrılarını değerlendirmek için geliştirilmiş, yüksek riskli hastaları olduğu kadar düşük riskli hastaları da ayırt edebilen, bir skorlama sistemidir. AKS gibi hayatı tehdit eden durumlarda periferik kan akımında azalmaya neden olur. Bu azalma dolaşım halindeki kan ile durgun halindeki kanın oranı olan perfüzyon indeksi (PI) ile gösterilir. Bu çalışmanın amacı göğüs ağrısı ile acil servise başvuran hastaların nedenlerini tesbit etmekte PI ve PVI nın HEART Skoru ile karşılaştırarak kullanılabileceğini araştırmaktır. Hastanemiz acil servise 01.04.2019-01.10.2019 tarihleri arasında göğüs ağrısı ile başvuran 154 hasta çalışmaya dahil edildi. Hastalar cinsiyet, yaş, göğüs ağrısı tipi, bilinen kronik hastalıkları ve sigara kullanım durumuna göre gruplandırıldı ve başvuru anındaki vital bulguları, PI değeri, troponin değeri, acil servis ve hastaneye yatanların sonlanımları (taburculuk, eksitus) çalışma formuna işlendi. 154 hastanın 58 i kadın hasta 96 tanesi erkek hastaydı. Hastaların 55'inin tipik göğüs ağrısı (13'ü kadın, 42'si erkek), 99'unın atipik göğüs ağrısı (48'ü kadın, 51'i erkek) mevcuttu. 154 hastanın 88'i acil servisten taburcu olurken 66 tanesi hastaneye yatırıldı. Yatan 66 hastanın 3 tanesi hastane içinde eksitus olurken, 63 hasta servisten taburcu edildi. HEART Skoru ile PI değerleri arasında istatistiksel olarak anlamlı fark saptanmıştır (p=0,003). HEART Skoru ile PVI değerleri karşılaştırıldığında istatistiksel olarak anlamlı fark saptanmamıştır (p=0,756). HEART Skoru ile troponin değerleri karşılaştırıldığında istatistiksel olarak anlamlı fark saptanmıştır(p<0,001). Göğüs ağrısı ile başvuran hastaların AKS açısından değerlendirmede HEART Skoru kullanımı acil servisten taburculuğu karar vermede etkili olabileceği görülmüştür. Ancak acil servise en sık başvuru nedenlerinden biri olan göğüs ağrısı ayırıcı tanısının ayrımında PI ve PVI değerlerinin yeterince aydınlatıcı olmadığı anlaşılmaktadır. Acute myocardial infarction (AMI) has high mortality rates worldwide. However, early diagnosis and treatment can reform the prognosis. For this reason, early diagnosis in the emergency department is very important for AMI and acute coronary syndrome (ACS). The HEART score was developed to evaluate chest pain in the emergency department. However, it is a scoring system that can distinguish among high-risk patients as well as low-risk patients. Life-threatening conditions such as ACS cause a decrease in peripheral blood flow. This decrease is indicated by the perfusion index (PI), that is the ratio of circulating blood to stagnant blood. The aim of this study was to determine the causes of patients applying to the emergency department with chest pain. In addition, we aimed to investigate whether PI and PVI can be compared with HEART Score. 154 patients admitted to our emergency department with chest pain between 01.04.2019 and 01.10.2019 were included in the study. Patients were grouped according to gender, age, type of chest pain, known chronic diseases and smoking status. The vital signs, PI value, troponin value, emergency department and hospitalization outcomes (discharge, death) were recorded in the study form. Of the 154 patients, 58 were female and 96 were male. 55 patients had typical chest pain (13 female, 42 male) and 99 had atypical chest pain (48 female, 51 male). 88 out of 154 patients were discharged from the emergency department and 66 were hospitalized. Of the 66 hospitalized patients, 3 died in the hospital and 63 patients were discharged from the hospital. There was a statistically significant difference between HEART score and PI values (p = 0.003). There was no statistically significant difference between HEART score and PVI values (p = 0.756). When HEART Score and troponin values were compared, as statistically significant difference was found (p <0.001). The use of the HEART Score in the evaluation of patients with chest pain for ACS was found to be effective in deciding on discharge from the emergency department. However, in the differential diagnosis of chest pain, which is one of the most common reasons for admission to the emergency department, PI and PVI values are not sufficiently illuminating.
