İnsanları etkileyen tüm ağrıların içinde baş ağrısı en sık olanıdır. Primer baş ağrısı tanısı genellikle baş ağrısının özellikleri ve eşlik eden semptomlar gözönüne alınarak konur. Kesin bir tanı yöntemi yoktur. Görüntüleme yöntemleri ve laboratuar tetkikleri sıklıkla diğer baş ağrısı nedenlerini dışlamak için kullanılır. Allopregnanolon insan beyninde de novo ürerilen progesteron kaynaklı bir steroidtir. Nörosteroidlerin nöronlar üzerinde koruyucu etkiye sahip olduğu iyi bilinmektedir. Allopregnanolon nükleer steroid reseptörleri etkisi olmasının yanı sıra GABA reseptörleri üzerinden hızlı nongenomik etki sergilemektedir. Daha önceden yapılan birçok çalışma Alzheimer hastalığında, posttravmatik stres bozukluğunda, depresyonda, iskemik beyin hastalıklarında, epilepsi gibi organik beyin hastalıklarında allopregnanolonun beyinde etkinliğinin artmasının hastalıkları olumlu yönde etkilediği gösterilmiştir. Bu çalısmanın amacı primer baş ağrısı olan hastalarla sağlıklı kontrol grubu arasında serum bazal allopregnanolon düzeylerinde farklılık olup olmadığı, varsa bu farklılığın primer baş ağrısı olan hastalarda akut atak döneminde sapmalar gösterip göstermediği incelemek; böylelikle allopregnanolonun primer baş ağrısı olan hastalarda tanı ve tedavide rol oynayabilecek bir madde olup olmadığını belirlemektir. Bu şekilde gerek yeni tedavi stratejilerinin geliştirilmesine katkıda bulunmak, gerekse primer baş ağrısı ayırıcı tanısı ve prognozunun tayinine yardımcı olabilecek biyolojik bir belirteç bulmak amaçlanmıştır. Süleyman Demirel Üniversitesi Tıp Fakültesi acil servisine başvuran primer baş ağrısı olan atak döneminde olan 103 hasta grubu ile sağlıklı kontrol grubundan oluşan 103 kişilik kontrol grubu alınarak çalışma yapılmıştır. Çalışma için sözel bilgilendirme sonrası 103 hasta yazılı onam vererek atak dönemi için venöz kan örneği verdi. İlk başvurularından en az iki hafta sonra atak dönemi için kan veren hastalar farklı zamanlarda ataksız dönemde tekrar venöz kan örneği verdi. Cinsiyet ve yaş olarak benzer 103 sağlıklı kontrol hastasının venöz kan örneği alındı. Biyokimya tüpüne alınıp, örneklerin serumu ayrıştırıldıktan sonra ELİSA yöntemi ile allopregnanolon düzeyi incelenmiştir. Hastaların yaş, cinsiyet, tetikleyici faktör varlığı, ağrı tipi, atak sıklığı, atak süresi, atak şiddeti, ek şikayet varlığı, tetkileyici faktör varlığı gibi demografik özellikler kaydedildi. Hastalar migren, gerilim, küme tipi başağrısı ve nonkategorize primer baş ağrısı olarak dört alt gruba ayrılmıştır. Çalışmamızda primer başağrısı ile başvuran olgularda ağrılı dönemde Allopregnanolon düzeyleri ataksız döneme ve kontrol grubuna kıyasla daha düşük olarak saptanmıştır (p<0,05). Bu sonuçlara bakılarak allopregnanolonun primer baş ağrısı patogenezinde rol oynayan bir hormon olduğu, ancak daha geniş tabanlı bir çalışma yapılarak primer baş ağrısı tanı ve prognozuna daha anlamlı katkı sağlayacağı düşünüldü. Headache is undouptedly the most common of all the pains affecting people. Primary headache is usually diagnosed by taking cognizance of the headache features and the associated symptoms. There is no definitive diagnostic method. Laboratory tests and imaging methods are often used to exclude other causes of serious headache. Allopregnanolon is a progesterone based steroid in human brain where de novo is produced apart from peripheral production. Neurosteroids are well known to have a protective effect on neurons. Allepregnanolone has effect through nuclear steroid receptors besides rapid non-genomic effect through GABA receptors. Lots of studies previously done, in Alzeihmer disease, postraumatic stress disorder, depression, ischemic brain disease and in organic brain diseases such as epilepsy, have shown that increasing of activity of allopregnanolone in the brain has a positive impact on diseases. This study aims to examine if there is serum basal allopregnanolone level difference between primary headache patients and healthy control group, if so, to examine the relation of this difference and clinical manifestations of this disease and whether acute exacerbation periods showing deviations in primary headache patients; and thus to determine whether allopregnanolone is a substance, playing a role in the diagnosis and treatment of primary headache. In this way we aim to contribute to the development of new therapeutic strategies as well as to find biological markers that may help to assess the differential diagnosis and prognosis of primary headache. 103 patients previously admitted to the Suleyman Demirel University Faculty of Medicine emergency department who diagnosed with primary headache and who are in the attack period wil be taken as patient group with another control group consisting of 103 healty people and a study was carried. After verbal information, 103 patients gave a written consent and a venous blood sample for the episode. At least 2 weeks after the first application, 103 of the patients who gave blood for the wanted episode gave repeat venous blood sample at different times. Venous blood samples of 103 healthy control patients with similar characteristics as gender and age were taken. After the samples were taken into biochemistry tube and the serum of the samples were separated, allopregnanolone level was investigated by ELISA method. Demographic characteristics such as age, gender, frequency of attack, duration of attack, pain character, severity of attack, presence of additional complaints and presence of trigger factor were recorded in primary headache patients. Patients were divided into four subgroups as migraine, tension-type headache, cluster-type headache and non-categorized primary headache. In our study, allopregnanolone levels were lower in the patients with primary headache compared to the control gruop and painless period group (p<0,05). Based on all these results, allopregnanolone is a hormone that plays a role in the pathogenesis of primary headache, but it is thought that a larger base study will contribute to the diagnosis and prognosis of primary headache more significantly.
