Çalışmamızın amacı; Total Diz Protezi (TDP) sonrası akut postoperatif ağrı ve fiziksel performansın preoperatif faktörler ile olan ilişkisini araştırmaktır. Süleyman Demirel Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Servisi'nde yatan, çalışmanın dahil edilme ve edilmeme kriterlerine uyan 70 hasta çalışmaya alındı. Hastalar ameliyattan 1 gün önce ve ameliyattan 3 gün sonra aynı fizyoterapist tarafından değerlendirildi. Ameliyat öncesi; hastaların fiziksel ve demografik bilgilerini içeren Hasta Ön Değerlendirme Formu, bilateral diz fonksiyonunu değerlendiren Hospital for Special Surgery (HSS) Diz Skorlaması, Vizüel Analog Skalası (VAS), Tampa Kinezyofobi Ölçeği (TKÖ), Hastane Anksiyete ve Depresyon Ölçeği (HADÖ), Uluslararası Fiziksel Aktivite Anketi Kısa Formu (UFAA-KF) ve Vücut Farkındalığı Anketi (VFA) kullanıldı. Ameliyat sonrası; son 24 saatte hissedilen ağrıyı değerlendirmek için Kısa Ağrı Anketi (KAA) ve fiziksel performansı değerlendirmek için Süreli Kalk ve Yürü Testi (SKYT) kullanıldı. Preoperatif kinezyofobi, anksiyete, depresyon ve ağrı şiddeti akut postoperatif ağrıyla ilişkili bulunurken (p<0.05), preoperatif kinezyofobi akut postoperatif fiziksel performansla ilişkili bulunmuştur (p<0.05). Preoperatif vücut farkındalığı ile akut postoperatif ağrı ve fiziksel performans arasında ise ilişki olmadığı bulunmuştur (p>0.05). Ayrıca preoperatif kronik diz ağrı süresi; '<5 yıl' ve '≥ 6 yıl' olanlar arasında akut postoperatif ağrı ve fiziksel performans açısından anlamlı fark bulunmuştur (p<0.05). Preoperatif fiziksel aktivite düzeyi inaktif olanlar ile inaktif olmayanlar arasında akut postoperatif fiziksel performans açısından anlamlı fark bulunmuştur (p<0.05). Akut postoperatif dönemde ağrı ve fiziksel performansı etkileyebilecek preoperatif faktörler dikkate alınarak; hasta memnuniyeti, ağrı yönetimi, taburculuk planlaması, ek fizyoterapi desteğinin verilmesi ve psikolojik destek gibi konularda bu faktörlerin hasta üzerindeki sonuçlarından yararlanılabilir.

The aim of our study is to investigate the relationship between acute postoperative pain and physical performance with preoperative factors after TDP. Seventy patients who were hospitalized in the Süleyman Demirel University Medicine Faculty Orthopedics and Traumatology Service and complied with the study's inclusion and exclusion criteria were included in the study. The patients were evaluated by the same physiotherapist 1 day before surgery and 3 days after surgery. Before surgery; The Patient Pre-Evaluation Form containing the physical and demographic information of the patients, Hospital for Special Surgery (HSS) Knee Score evaluating bilateral knee function, Visual Analogue Scale (VAS), Tampa Kinesiophobia Scale (TKS), Hospital Anxiety and Depression Scale (HADS), International Physical Activity Questionnaire-Short Form (IPAQ-SF) and Body Awareness Questionnare (BAQ) were used. After surgery; Brief Pain Inventory (BPI) was used to evaluate the pain in the last 24 hours and Time Up and Go Test (TUG) was used to evaluate physical performance. Preoperative kinesiophobia, anxiety, depression and pain severity were associated with acute postoperative pain (p<0.05), preoperative kinesiophobia was associated with acute postoperative physical performance (p<0.05). There was no relationship between preoperative body awareness and acute postoperative pain and physical performance (p>0.05). In addition, there was a significant difference in terms of acute postoperative pain and physical performance between the preoperative chronic knee pain duration; '<5 years' and ' ≥ 6 years'. There was a significant difference in terms of acute postoperative physical performance between the inactive and non-inactive ones (p<0.05). Considering the preoperative factors that may affect pain and physical performance in the acute postoperative period, the results of these factors on the patient can utilized in subjects such as patient satisfaction, pain management, discharge planning, providing additional physiotherapy support and psychological support.
