Bipolar Bozukluk (BB) ve Şizofreni, büyük oranda yeti yitimine yol açan ve yıkıcı seyri sırasında yüksek mortalite riski taşıyan kronik, nöropsikiyatrik hastalıklardandır. Dünya Sağlık Örgütü tarafından en çok işgücü kaybına neden olan hastalıklar arasında kabul edilmektedir. Literatürde eser elementlerin ölçümü ile tiyol/disülfit dengesinin bir arada yorumlandığı ve BB atak-remisyon dönemi ile şizofreninin bir arada değerlendirildiği bir çalışma bulunmamaktadır. Çalışmamızda BB atak ve remisyon dönemlerinde ve şizofreni hastalarında her üç metal elementin düzeylerini ve total, redükte tiyol seviyelerini değerlendirerek olası oksidatif denge değişikliğini protein oksidasyonu üzerinden saptamayı ve bu metal elementlerle ayrı ayrı ilişkilerini değerlendirmeyi amaçladık. Çalışmaya DSM-V tanı kriterlerine göre 32 bipolar bozukluk (atak ve remisyon dönemi ayrı ayrı değerlendirilmek üzere) tanısı alan ve 32 şizofreni tanısı almış toplam 64 hasta ve 32 sağlıklı gönüllü dahil edilmiştir. Serum total ve native tiyol düzeyleri otomatize kolorimetrik yöntem ile belirlenmiştir. Serum bakır, çinko ve alüminyum düzeyleri ise atomik absorbsiyon spektrometri (AAS) yöntemi ile ölçülmüştür. Hem BB remisyon grubunda hem de şizofreni grubunda kontrol grubuna göre, ayrıca BB manik atak döneminde remisyon dönemine göre native tiyol düzeyleri ve total tiyol düzeyleri anlamlı derecede düşük bulunmuştur. Disülfit düzeyleri açısından ise şizofreni grubu ile hem kontrol grubu hem de BB remisyon grubu arasında istatistiksel olarak anlamlı farklılık bulunmazken, BB remisyon grubunda kontrol grubuna göre ve BB manik atak döneminde remisyon dönemine göre disülfit düzeyleri anlamlı derecede düşük bulunmuştur. Her üç metal element serum seviyeleri her grupta normal aralıkta bulunmuştur fakat hem şizofreni grubunda hem de BB manik atak grubunda çinko seviyeleri daha yüksek, bakır seviyeleri ise daha düşük bulunmuştur. Bu nedenle BB manik atak dönemi ve şizofreni grubunda serum Zn/Cu oranı optimal kabul edilen aralığın oldukça üzerinde bulunmuştur. Serum Al düzeyleri açısından ise gruplar arası anlamlı farklılık saptanmamıştır. Metallotioneinler gibi tiyol (-SH, sülfidril) grupları açısından zengin olan proteinlerin rolü ve çalışmamızda total ve native tiyol düzeylerinde saptanan anlamlı düşüklük birlikte değerlendirildiğinde eser element dengesindeki (Zn/Cu) bozulmanın ve tiyol-disülfit homeostazisinin kaybının BB manik dönem ve şizofreni tablolarının etyopatogenezinde yer alabileceği söylenebilir. Fakat tüm bu veriler düşünüldüğünde oksidatif stresi değerlendiren çalışmalarda, hastalığın evresini, beslenme alışkanlıklarını, kullanılan duygudurum düzenleyiciler ve antipsikotik türünü ve yaşam tarzı değişikliklerini dikkate almak gerekmektedir. Çalışmamızdan elde edilen sonuçlar, ileride yapılacak çalışmalarda bu bulguların farklı bir tasarım ve daha büyük örneklem büyüklüğüyle doğrulanması ihtiyacını ortaya koymaktadır. Bu da hem eser elementlerin hem de tiyol-disülfit homeostazisinin şizofreni ve bipolar bozukluk etyopatogenezindeki rolünün netliğe kavuşturulmasını sağlayabilecektir. Bipolar Disorder (BD) and Schizophrenia are chronic neuropsychiatric diseases that cause disability to a great extent and carry a high mortality risk during their destructive course. It is accepted by the World Health Organization among the diseases that cause the most labor loss. In the literature, there is no study in which the measurement of trace elements and thiol/disulfide homeostasis are interpreted together and bipolar attack-remission period and schizophrenia are evaluated together. In our study, we aimed to determine the possible oxidative balance changes through protein oxidation by evaluating the levels of three metal elements, total-native thiol levels in bipolar disorder attack and remission periods and in schizophrenia patients and to evaluate their relationship with metal elements separately. A total of 64 patients with a diagnosis of 32 bipolar disorder (attack and remission period to be evaluated separately) and 32 schizophrenia according to DSM-V diagnostic criteria and 32 healthy volunteers were included in the study. Serum total and native thiol levels were determined by automated colorimetric method. Serum copper, zinc and aluminum levels were measured by atomic absorption spectrometry (AAS) method. Native thiol levels and total thiol levels were found to be significantly lower in both the BD remission group and the schizophrenia group compared to the control group, and also during the bipolar manic episode compared to the remission period. In terms of disulfide levels, there was no statistically significant difference between the schizophrenia group and both the control group and the BD remission group while the disulfide levels were significantly lower in the BD remission group compared to the control group and in the BD manic episode period compared to the remission period. Serum levels of three metal elements were in the normal range in each group, but zinc levels were higher and copper levels were lower in both the schizophrenia group and the BD manic episode group. Therefore, the serum Zn/Cu ratio was found to be above the optimal range in the BD manic episode and schizophrenia group. There was no significant difference between the groups in terms of serum Al levels. Impairment in trace element balance (Zn/Cu) and loss of thiol-disulfide homeostasis may be included in the etiopathogenesis of BD manic episode and schizophrenia when the role of proteins rich in thiol (-SH, sulfhydryl) groups such as metallothioneins and the significant low levels of total thiol and native thiol in our study are evaluated together. However, it is necessary to consider the stage of the disease, eating habits, the type of mood stabilizers and antipsychotics used, and lifestyle changes in studies evaluating oxidative stress. The results obtained from our study reveal the need to confirm these findings with a different design and larger sample size in future studies. This will enable clarification of the role of both trace elements and thiol-disulfide homeostasis in the etiopathogenesis of schizophrenia and bipolar disorder.
