Bu çalışmanın amacı opere edilmiş tibia plato kırığı mevcut hastalarımızın izlemlerinde elde ettiğimiz MRG bulgularını, cerrahi onarım sonrası fonksiyonel ve radyolojik değişiklikler ile birlikte değerlendirmektir. Bu çalışmada 2010-2019 yılları arasında kliniğimize tibia plato kırığı tanısı ile başvurup ameliyat edilmiş hastalar hastane bilgi sistemi üzerinden belirlenerek toplamda 91 hasta tarandı. Bu hastalardan 18 yaş altı olanlar, ölçümlere uygun radyografileri olmayanlar, MRG çekimine uygun olmayanlar ve telefonla ulaşılamayanlar çalışma dışı bırakıldı. Çalışmamız 49 hastanın 49 ameliyatlı ve 49 sağlam dizi ile tamamlandı. Hastaların demografik özellikleri, KSKS, KSFS, KOOS fonksiyonel ve klinik skorlamaları, Rasmussen, Kellgren&Lawrence ve WORMS radyolojik skorlamaları, takip MRG ile menisküs ve bağ patolojileri incelendi. Hastalarımızın 39'u erkek, 10'u kadın, ortalama yaşı 45,9±11,89 ve ortalama takip süresi 58,2±30,7 aydı. Kırıkların %49'u (n=24) lateral plato, %6,1'i (n=4) medial plato ve %42,9'u (n=21) her iki platoyu içeren bikondiler kırıktı. KSKS diz skoru %85,7 hastada, KSFS fonksiyonel skoru %89,7 hastada, Rasmussen radyolojik skoru %93,8 hastada tatmin ediciydi. KOOS subjektif anketinde hastalar 430,7 puan aldı. WORMS puanlamasında ortalama total puan 34,2 idi. Kellgren evrelemesine göre %18,3 (n=9) hastada şiddetli osteoartrit (evre 3-4) geliştiği görüldü. Tüm hastalar MRG takiplerinde patolojik değişiklikler gösterdi. Tibia plato kırıkları tatmin edici klinik ve radyolojik sonuçlar gösterseler dahi orta vadeli sonuçlarında bile ciddi kıkırdak hasarı, marjinal osteofit oluşumu, kemik iliği ödemi, kemikte yıpranma ve çökme gibi osteoartrite ilerleyen süreçler beraberinde gelmektedir. Uzun vadeli sonuçlar elimizde yok ancak ilerleyen süreçte klinik ve fonksiyonel sonuçların da bozulabileceği ve radyolojik olarak da osteoartrit gelişebileceğini öngörüyoruz. The aim of this study is to evaluate the MRI findings we obtained during the followup of our patients with operated tibial plateau fractures together with functional and radiological changes after surgical repair. In this study, 91 patients who were admitted to our clinic with the diagnosis of tibial plateau fracture and operated between 2010 and 2019 were identified through the hospital information system. Among these patients, those under the age of 18, those who did not have radiographs suitable for measurements, those who were not suitable for MRI and those who could not be reached by phone were excluded from the study. Our study was completed with 49 operated and 49 intact knees of 49 patients. The demographic characteristics of the patients, KSKS, KSFS, KOOS functional and clinical scores, Rasmussen, Kellgren&Lawrence and WORMS radiological scores, and meniscus and ligament pathologies with follow-up MRI were examined. 39 of our patients were male, 10 of them were female, the meanage was 45.9 ± 11.89 and the mean follow-up period was 58.2 ± 30.7 months. 49% (n = 24) of the fractures were lateral plateau, 6.1% (n = 4) medial plateau and 42.9% (n = 21) bicondylar fractures including both plateaus. The KSKS knee score was satisfactory in 85.7% of the patients, the KSFS functional score in 89.7% of the patients, and the Rasmussen radiological score in 93.8% of the patients. In the KOOS subjective questionnaire, the patients scored 430.7 points. The mean total score on the WORMS score was 34.2. According to the Kellgren staging, 18.3% (n = 9) patients developed severe osteoarthritis (stage 3-4). All patients showed pathological changes in MRI follow-up. Even if tibial plateau fractures show satisfactory clinical and radiological results, even in the medium-term results, serious cartilage damage, marginal osteophyte formation, bone marrow edema, bone we ar and collapse are accompanied by processes that progress to osteoarthritis. We do not have long-term results, but we anticipate that clinical and functional results may also deteriorate and osteoarthritis may develop radiologically in the future.
