Üriner sistem taş hastalığı, ülkemizde sık görülen hastalıklardandır Üreter taşları böbrekte oluşup, üretere yerleşen taşlardır. Üreter taşlarının tedavisinde pek çok yöntem vardır ancak noninvaziv tedavi yöntemleri daha sık tercih edilmektedir. Bitkisel tedavilerde sık başvurulan yöntemdir ancak literatürde bu konuyla ilgili klinik çalışma azdır. Toplumumuzda da Kayseri ve çevresinde yetişen gilaburu (viburnum opulus) olarak bilinen meyve ekstresi böbrek taşının düşürülmesi için yaygın olarak kullanılmaktadır. Bu çalışmada gilaburunun üreter taşlarının medikal ekspulsiyonunda etkinliğini araştırmayı amaçladık. YÖNTEM: Çalışmamıza Mart - Ekim 2020 arasında Süleyman Demirel Üniversitesi Tıp Fakültesi Üroloji Anabilim Dalı polikliniğine yan ağrısı, idrarda kanama gibi şikayetlerle başvuran ve üreter taşı tanısı konulan, üreterde 4- 10 mm arasında taşı olan 18- 65 yaş arası yetişkin 60 hasta dahil edilmiştir. Hastalar gilaburu kullananlar ve kullanmayanlar olarak iki gruba ayrıldı. Hastalar zarf yöntemiyle randomize edildi. Hastalar en fazla 4 hafta takip edildi. Bütün hastalara diyet, sıvı alımı, günlük yürüyüş önerilerinde bulunuldu ve ağrı kontrolü için diklofenak sodyum verildi. Grup1' i oluşturan 30 hastaya günde 3 defa yemeklerden sonra 200 cc (1 bardak) olacak şekilde gilaburu meyve suyu içirildi. Grup 2' yi oluşturan 30 hastaya standart öneriler dışında herhangi bir tedavi uygulanmadı. Gilaburu içen ve içmeyen hastaların demografik verileri, taş düşürme süreleri, düşürme sırasındaki nonsteroid antiinflamatuar kullanım miktarları, VAS skor değişimleri, spot idrarda sodyum, potasyum, kalsiyum, kreatin, idrar dansite ve idrar Ph düzeyleri karşılaştırılmıştır. BULGULAR: Hastaların yaş ortalaması 44,43±13,61 yıl olarak belirlendi. Ortalama taş boyutları 7, 14±1,57 mm olarak belirlendi. Ekstre kullanan grupta taşların boyut ortalaması 6,63±1,34 mm olurken kullanmayan grupta 7,75±1,65 mm olarak hesaplandı. Ekstre kullanan hastalarda taş düşürme süresi 6,58±2,37 gün olurken kullanmayan hasta grubunda taş düşürme süresi 9,60±4,24 gün olarak anlamlı düzeyde daha uzun bulundu. Tüm hastaların % 73,3' ü taşını düşürdüğü gözlendi. Gilaburu kullanan grupta taş düşürme oranı %80 bulundu ancak istatistiksel anlamlı değildi (p=0,247). İki grup arasında istatistiksel anlam saptanmadı. Gilaburu kullanan ve kullanmayan hastaların taş lokalizasyonuna göre düşürme süreleri karşılaştırıldığında proksimal hasta grubunda gilaburu içenlerin süreleri istatistiksel anlamlı düzeyde daha kısa bulundu (sırasıyla 8,00±1,67-13,33±4,32 gün). Gilaburu kullanan ve kullanmayan hastaların taş boyutlarına göre düşürme süreleri karşılaştırıldığında 6-8 mm arasında taşı olan ve gilaburu kullanan hastaların taş düşürme süreleri 6,86±1,34 gün ile istatistiksel anlamlı ve düşüktü. Gilaburu kullanan hastaların kullanmayanlara göre VAS skorlarının takipler süresince istatistiksel anlamlı düştüğü gözlendi (sırasıyla 2,92±1,21-4,60±1,53). Spot idrarda biyokimya verilerinde de gilaburu içen hastaların idrar sodyum, potasyum, kalsiyum, idrar dansite düzeylerinin daha düşük, idrar Ph'larının daha yüksek olduğu saptandı. Spot idrardaki bu biyokimyasal veriler gilaburu kullananlar lehine istatistiksel anlamlı raporlandı. TARTIŞMA ve SONUÇ: Çalışmamızda gilaburunun üreter taşlarının, özellikle proksimale lokalize taşların düşürülme oranında artışa neden olduğu ve bu süre içerisinde hastaların ağrı skorlarında iyileşme ve nonsteroid antiinflamatuar kullanımında azalmaya neden olduğu gözlemlenmiştir. Hastaların bu sürenin başindaki ve sonundaki spot idrardaki taş oluşumuna etkili olabilecek biyokimyasal verilerini gilaburu kullanımının olumlu etkilediği saptanmıştır. Bu veriler ışığında çalışmamız literatürde gilaburunun ureter taşı düşürmeye etkisini gösteren ilk klinik çalışma olması açısından önemlidir. Urinary system stone disease is one of the common diseases in our country. Ureteral stones are stones that form in the kidney and settle in the ureter. There are many methods in the treatment of ureteral stones, but noninvasive treatment methods are preferred more frequently. It is a frequently used method in herbal treatments, but clinical studies on this subject are few in the literature. In our society, the fruit extract known as gilaburu (viburnum opulus) that grows in Kayseri and its surroundings is widely used to reduce kidney stones. In this study, we aimed to investigate the efficacy of gilaburun in medical expulsion of ureteral stones. METHOD: Sixty adult patients aged 18- 65 years who were diagnosed with ureteral stones and who presented with complaints such as flank pain and bleeding in the urine between March and October 2020 in the outpatient clinic of the Department of Urology, Faculty of Medicine, Süleyman Demirel University, were included in our study. The patients were divided into two groups as those using gilabu and those who did not. Patients were randomized by the envelope method. The patients were followed for a maximum of 4 weeks. All patients were advised on diet, fluid intake, daily walking, and diclofenac sodium was given for pain control. 30 patients in Group 1 were given 200 cc (1 glass) gilaburu fruit juice three times a day after meals. No treatment other than standard recommendations was applied to 30 patients in Group 2. The demographic data of the patients who drank gilabur and those who did not smoke, the duration of stone descent, the amount of nonsteroidal anti-inflammatory use during abortion, VAS score changes, spot urine sodium, potassium, calcium, creatine, urine density and urine Ph levels were compared. RESULTS: The mean age of the patients was determined as 44.43 ± 13.61 years. Average stone dimensions were determined as 7.44 ± 1.57 mm. While the average size of the stones was 6.63 ± 1.34 mm in the group using the extract, it was calculated as 7.75 ± 1.65 mm in the group not using the extract. The time for stone dropping was found to be 6.58 ± 2.37 days in patients using the extract, while the stone dropping time was found to be significantly longer in the patient group, which was 9.60 ± 4.24 days. It was observed that 73.3% of all patients dropped stones. The stone dropping rate in the group using gilaburu was 80%, but it was not statistically significant (p = 0.247). There was no statistical significance between the two groups. When the reduction times of the patients using and not using gilabur were compared according to the stone location, the duration of the gilaburu drinkers in the proximal patient group was found to be statistically significantly shorter (8.00 ± 1.67-13.33 ± 4.32 days, respectively). When the reduction times of the patients who used and did not use gilaburu were compared according to stone size, the stone reduction times of patients with stones between 6-8 mm and using gilaburu were statistically significant and lower with 6.86 ± 1.34 days. It was observed that the VAS scores of the patients using gilaburu decreased statistically significantly during the follow-up compared to those who did not (2.92 ± 1.21-4.60 ± 1.53, respectively). In the biochemical data of spot urine, it was determined that the urinary sodium, potassium, calcium, urine density levels of the patients who drank gilaburu were lower and their urine pH was higher. These biochemical data in spot urine were reported statistically significant in favor of gilaburu users. DISCUSSION and CONCLUSION: In our study, it was observed that gilaburu caused an increase in the rate of reducing ureteral stones, especially proximally localized stones, and during this period, it caused an improvement in the pain scores of the patients and a decrease in the use of nonsteroidal anti-inflammatory drugs. It was determined that the use of gilaburu positively affected the biochemical data of the patients, which may affect the formation of stones in the spot urine at the beginning and at the end of this period. In the light of these data, our study is important as it is the first clinical study showing the effect of gilaburu on ureteric stone reduction in the literature.
