Giriş ve Amaç: Meme kanseri dünyada kadınlarda en yaygın görülen kanserdir. Gelişen tedavilere rağmen hala yüksek morbitide ve mortalite oranlarına sahiptir. HER2 pozitif alt tipte bu kötü prognoz daha belirgindir. Yeni prognostik ve prediktif belirteçlere ihtiyaç vardır. Son yıllarda WBP2 expresyonunun meme kanserinin gelişiminde ve kötü prognozunda etkili olduğuna dair veriler artmaktadır. Bu çalışmada C-Erb B2 (Her2) Pozitif Trastuzumab Tabanli Tedavi Alan Meme Kanseri Hastalarinda WBP2 ekspresyonunun prognoza etkisi araştırıldı. Materyal ve Metod: Nisan 2008 ile Nisan 2019 tarihleri arasında HER2 pozitif invaziv meme kanseri tanısıyla takip edilen, postoperatif patoloji preparatlarına ulaşılabilen toplam 136 hastada WBP2 expresyon düzeyi immünhistokimyasal boyama yöntemi ile çalışıldı. Kaplan-Meier testi ve log-rank analizi progresyonsuz sağkalım (PFS) ve genel sağkalım analizi için ve sağkalımı bağımsız bir şekilde etkileyen faktörleri tespit etmek için Cox regresyon analizi uygulandı. Bulgular: Tek değişkenli analizde istatistiksel olarak anlamlı olan faktörler; evre, lenf nodu pozitifliği, Ki-67 proliferasyon indeksi ve WBP2 overekspresyonu olarak saptandı (p<0,05). Tek değişkenli analizde p değeri <0.05 olan faktörlere çok değişkenli model uygulandı. Invazif hastalıksız sağkalım üzerine WBP2 overekspresyonu (p=0,005) ve Ki-67 (p=0,030) proliferasyon indeksi bağımsız prediktif faktörlerdi. Genel sağkalım (OS) üzerine ise adjuvan trastuzamab tedavisi tamamlandıktan sonra beyin metastazı gelişmesi istatistiksel olarak bağımsız prognostik faktör olarak tespit edildi (p=0,002). WBP2'nin genel sağkalıma etkisi vardı, ancak istatistiksel olarak zayıftı (p=0,050). Sonuç: WBP2 overekspresyonu ve Ki-67 indexi invaziv hastalıksız sağkalımı olumsuz etkileyen prediktif faktörler olarak saptandı. Adjuvan trastuzumab tedavisi tamamlandıktan sonra beyin metastazı gelişmesi genel sağkalımı kötü yönde etkileyen bağımsız prognostik faktör olarak tespit edildi ve bu literatürde yeni bir bulgudur. Introduction and Aim: Breast cancer is the most common cancer in women in the world. Despite the advancing treatments, it still has high morbidity and mortality rates. This poor prognosis is more pronounced in the HER2 positive subtype. New prognostic and predictive markers are needed. In recent years, there has been an increase in data showing that WBP2 expression is effective in the development and poor prognosis of breast cancer. In this study, the effect of WBP2 expression on prognosis in Breast Cancer Patients Receiving C-Erb B2 (Her2) Positive Trastuzumab Based Treatment was investigated. Materials and Methods: WBP2 expression level was studied by immunohistochemical staining method in 136 patients who were followed up with a diagnosis of HER2-positive invasive breast cancer between April 2008 and April 2019 and whose postoperative pathology preparations could be accessed. Kaplan-Meier and log-rank analyses were employed on PFS and OS and multivariate Cox proportional hazard model analyzed clinical parameters for their prognostic relevance. Results: Statistically significant factors in univariate analysis; stage, lymph node positivity, Ki-67 proliferation index and WBP2 overexpression (p <0.05). Factors with a p value of <0.05 in univariate analysis, a multivariate model was applied. WBP2 overexpression (p = 0.005) and Ki-67 (p = 0.030) proliferation index were independent predictive factors on invasive disease-free survival. On the overall survival (OS), the development of brain metastasis after the completion of adjuvant trastuzamab treatment was found to be a statistically independent prognostic factor (p = 0.002). WBP2 had an effect on overall survival, but it was statistically weak (p = 0.050). Conclusion: WBP2 overexpression and Ki-67 index were determined as predictive factors negatively affecting invasive disease-free survival. After the completion of adjuvant trastuzamab treatment, the development of brain metastasis was identified as an independent prognostic factor that adversely affected overall survival, and this is a new finding in the literature.
