Tıbbi araştırmalarda üç boyutlu (3B) baskı teknolojileri ve tıbbi görüntü işleme yazılımları kullanılarak hastaya özgü implant, protez ve cerrahi operasyon planlama modelleri üretilerek hastalığın takibi ve tedavisine yön verilmektedir. Rutinde hastalara teşhis koymak amacı ile istenilen tıbbi görüntüleme tekniklerinin temelini oluşturan; Manyetik Rezonans (MR) ve Bilgisayarlı Tomografi (BT) yöntemleri kullanılarak elde edilen veriler iki boyutlu (2B) olarak bilgisayar ekranına görüntü sunmaktadır. Kemik ve kas deformitelerinin, hareket ve yönlerini 2B tıbbi görüntüleme yazılımlarında net bir şekilde ayırt etmek mümkün olamamakla birlikte uzun zaman almaktadır. Bu amaçla 3B faydalı katı model üretimi tedavi aşamasındaki engelleri ortadan kaldıracaktır. Cerrahi operasyonların planlaması faydalı 3B katı model üretimi büyük önem arz etmektedir. Ameliyat sonrası gerek defektin kapatılması gerekse simetrik bir görüntü elde etmek cerrahi işlem sayısını azaltmakta ve hastanın hayat kalitesini oldukça artıracağı ön görülmektedir. Doğuştan ya da edinsel sebeplere bağlı yüz bölgesini içeren ve cerrahi operasyon gerektiren deformiteler yumuşak doku veya kemik kaynaklı olabilmektedir. Özellikle kemik deformitelerin rekonstrüksiyonlarında cerrahi planlama büyük önem arz etmekte, osteotomi hatlarının seviyeleri ve kemik hareketlerinin yön ve hareket miktarlarının ne kadar olacağı önceden hesaplanması önem arz etmektedir. Hastaların preoperatif değerlendirilmesinde çekilen BT verilerinin kullanıldığı 3B modelleme sistemleri ile planlanarak yapılan cerrahi işlemlerin olumlu sonuçlar ortaya çıkacaktır. Ayrıca ameliyat öncesi model üzerinde yapılan planlamalar cerrahi işlemi uygulayacak ekibin işlemleri daha kısa sürede ve efektif yapmasına olanak sağlanarak cerrahi işlemi ve dolayısıyla hastanın aldığı anestezi süresi kısalmış olacaktır. Bu çalışmada; Süleyman Demirel Üniversitesi Diş Hekimliği Fakültesi Ağız, Diş ve Çene Cerrahi Anabilim Dalı tarafından tedavi görmekte olan hastaların tıbbi görüntü verileri 3B yazıcı teknolojisi kullanılarak 1/1 ölçekte somut bir katı modele dönüştürülmüştür. Elde edilen katı model üzerinde ameliyat öncesi cerrahi ekip tarafından planlamalar yapılmasına olanak sağlamıştır. Hastalara protezlerin implante edilmesi 3B model üzerinde denenerek ameliyat planlaması yapılmıştır. Yapılan ön çalışmalar ile cerrahi operasyon en optimal şekilde tamamlanmıştır. Bu doğrultuda cerrahi operasyonun süresi azalmıştır. Aynı zamanda hastaların ameliyat sırasında rutin olarak uygulanan anestezik kimyasallara maruz kalma süresi kısalmıştır. Bu tez çalışmasında; uzun süren rekonstrüktif operasyonlar öncesi en az dört hastadan alınan gerçek BT verileri üzerinde tıbbi görüntü işleme yazılımı kullanılarak görüntü işleme ve görüntü iyileştirme yöntemleri uygulanmıştır. Yüz bölgesi kemik deformitlerinin oluşturduğu kemik ve kıkırdak doku hatlarını karmaşık geometrilerin üretilmesindeki başarısı iyi bilinen 3B eklemeli imalat yöntemi kullanarak katı bir planlama modeli üretilecektir. Bu model üzerinde ışık patlamaları ve diğer artifaktlar giderilerek; hastaların tedavi süreci, cerrahi operasyon kalitesi ve cerrahi operasyon süresi incelenerek rutin sürece katkısı değerlendirilmiştir. In medical research, patient-specific implant, prosthetic and surgical operation planning models are produced using three-dimensional (3D) printing technologies and medical image processing software to guide the follow-up and treatment of the disease. It forms the basis of the desired medical imaging techniques in the routine with the aim of diagnosing patients; Data obtained using Magnetic Resonance (MR) and Computed Tomography (CT) methods provide images to the computer screen in two dimensions (2D). It is not possible to clearly distinguish bone and muscle deformities, movements and aspects in 2D medical imaging software, but it is taking a long time. For this purpose, the production of 3D useful solid models will eliminate obstacles in the treatment phase. The planning of surgical operations is of great importance in the production of useful 3D solid models. After surgery, both closing the defect and obtaining a symmetrical image reduces the number of surgical procedures and it is predicted to greatly improve the quality of life of the patient. Deformities involving the facial area due to congenital or acquired causes and requiring surgical operation may be caused by soft tissue or bone. Surgical planning is of great importance, especially in reconstructions of bone deformities, and it is important to pre-calculate the levels of osteotomy lines and the amount of direction and movement of bone movements. Positive results of surgical procedures planned with 3D modeling systems using CT data taken in preoperative evaluation of patients will be revealed. Furthermore, the planning made on the pre-surgical model will allow the team to perform the surgical procedure in a shorter time and effectively, and therefore the duration of the anesthesia received by the patient will be shortened. In this study; Medical image data of patients undergoing treatment by Suleyman Demirel University Faculty of Dentistry Department of Oral, Dental and Jaw Surgery has been converted into a concrete solid model on a 1/1 scale using 3D printer technology. The resulting solid model has been made possible by the pre-surgical surgical team. Implantation of prostheses was tested on 3D model and surgery planning was carried out. The surgical operation was completed optimally with preliminary studies. In this way, the duration of the surgical operation decreased. At the same time, the exposure time of the patients to anesthetic chemicals that are routinely applied during surgery has been shortened. In this thesis study; Image processing and image improvement methods were applied using medical image processing software on actual CT data from at least four patients before long-term reconstructive operations. A solid planning model will be produced using a well-known 3D insert manufacturing method, which is the success of bone and cartilage tissue lines formed by facial region bone deformites in the production of complex geometries. By de-activing light bursts and other artifactes on this model; the treatment process of the patients, the quality of the surgical operation and the duration of the surgical operation were examined and their contribution to the routine process was evaluated.
