Ağız ve diş sağlığı, genel sağlık ve yaşam kalitesinin önemli bir göstergesidir. Ağız ve diş hastalıkları en yaygın bulaşıcı olmayan hastalıklar arasında olup dünyada önemli bir küresel halk sağlığı sorunudur. Bu araştırmanın amacı Sağlık Bakanlığı'na bağlı Ağız ve Diş Sağlığı Merkezleri(ADSM)'nin 2014-2018 yıllarındaki etkinlik düzeyini tespit etmektir. Araştırmada yöntem olarak, ADSM'lerin teknik verimliliğini ölçmek için Veri Zarflama Analizi (VZA), VZA sonucunda verimli bulunan merkezlerin kendi aralarında üstünlük sıralamalarının yapılabilmesi için Süper Etkinlik, 5 yıllık zaman diliminde merkezlerin verimlilikteki değişimlerini tespit edebilmek için Malmquist Toplam Faktör Verimliliği Endeksi (MTFVE), 2014-2018 yılları içerisindeki verimliliklerin seyrini inceleyebilmek için VZA Window Analizi kullanılmıştır. Ayrıca değişkenlerin VZA etkinlik skorları üzerindeki etkisini ölçebilmek amacıyla regresyon analizi, etkinliğe etki eden faktörlerin tespit edilebilmesi için ise Tobit analiz yöntemi uygulanmıştır. Araştırmanın evrenini Türkiye'de, 2014-2018 yılları arasında faaliyetini aralıksız sürdürmüş, ünit sayısı 50 ve üzerinde olan Sağlık Bakanlığı'na bağlı ADSM'ler oluşturmaktadır. Araştırmada örneklem seçilmemiş olup, evrenin tamamına ulaşılmıştır. Veriler Sağlık Bakanlığı'ndan resmi yazı ile temin edilmiştir. Araştırmada 2 farklı model oluşturulmuş ve tüm analizler bu modellere göre yapılmıştır. Girdi olarak ünit sayısı ve toplam diş hekimi sayısı; çıktı olarak ise muayene sayısı, diş çekimi sayısı, konservatif tedavi sayısı, endodontik tedavi sayısı, sabit protez hasta sayısı, hareketli protez (total) hasta sayısı değişkenleri kullanılmıştır. Girdiler her iki modelde de aynı iken, 4 çıktısı olan Model 1'de protez sayıları çıktılar arasına alınmamıştır. CCR Model 1'de 2014-2018 VZA verimlilik skorları yıllara göre sırasıyla ortalama 0,750, 0,782, 0,759, 0,733, 0,716 olarak tespit edilmiştir. Yapılan süper etkinlik analizinde bütün yıllarda her iki modelde 100 ile 215,7 arasında verimlilik skorları bulunmuştur. MTFVE analizi incelendiğinde; Model 1'de ADSM'lerin 2014-2015 dönemi dışındaki diğer dönemlerde teknolojik değişimde yaşanan yükselişin toplam faktör verimliliğine pozitif yönde yansıdığı görülmüştür. Ölçeğe göre sabit getiri altında Model 2 VZA Window analiz sonuçlarına bakıldığında en yüksek ortalama etkinlik skoruna, pencere uzunluğu 1 iken 2015 yılında 0,844 ile ulaşılmıştır. En düşük ortalama etkinlik puanı, pencere uzunluğu 5 alındığında 0,748 olarak tespit edilmiştir. 2018 yılı CCR Model 1'de ünit sayısının; merkezlerin verimlilik skorları üzerinde negatif, muayene sayısı, diş çekim sayısı ve konservatif tedavi sayısının pozitif yönde anlamlı bir etkiye sahip olduğu saptanmıştır. VZA'da elde edilen etkinlik skorları bağımlı değişken olarak alınarak Tobit analizi yapılmış ve sonuçlar yorumlanmıştır. 2018 yılı Model 1 CCR sonuçlarıyla girdi değişkenlerinden ünit sayısının %21,81 ve toplam diş hekimi sayısının %24,17 oranında azaltılması gerektiği görülmektedir. Çıktı değişkenlerinden endodontik tedavi sayısının %34,59, diş çekimi sayısının %13,21, muayene edilen hasta sayısının %3,45 ve konservatif tedavi sayısının %2,77 oranında artırılması önerilmektedir. Oral and dental health is an important indicator of overall health and quality of life. Oral and dental diseases are among the most common non-infectious diseases and are a major global public health problem in the world. Purpose of this research was to determine efficiency level of the oral and dental health centers (ODHC) under the Ministry of Health in Turkey during 2014-2018. As a research method, to measure the technical efficiency of ODHC, data envelopment analysis (DEA) was used and as a result of DEA super efficiency was used to rank the centers among themselves. Malmquist Total Factor Productivity Index (MTFPI) was used to detect the changes in the efficiency of the centers in the 5-year timeframe and DEA Window Analysis was used to examine the course of the efficiencies during 2014-2018. In addition, regression analysis was used to measure the effect of variables on DEA efficiency scores, and Tobit analysis method was used to determine the factors affecting efficiency. The population of the research consisted of ODHCs affiliated with the Ministry of Health, which have continued their activities continuously between 2014 and 2018, and whose number of units was 50 and above. Entire population was used and sampling was not done. The data were obtained from the Ministry of Health with their permission. Two different models were created and all analyses were conducted according to these models. As input, number of units and total number of dentists; as output, number of patients examined, number of tooth extraction, number of conservative treatments, number of endodontic treatments, number of fixed prosthetic patients, number of removable prosthetic (total) patients were used. While the inputs were the same in both models, the number of prosthetics in Model 1, which has output of 4, was not included in the output. In CCR Model 1, the 2014-2018 DEA efficiency scores were found as 0.750, 0.782, 0.759, 0.733, 0.716, respectively, for years. Super efficiency analysis showed that in both models in all years, efficiency scores ranged between 100 and 215.7. When the MTFPI analysis was examined, it was observed that the increase in technological change in the Model 1's ODHC in other periods other than the 2014-2015 period was reflected positively on the total factor productivity. When it is looked at Model 2 DEA Window analysis results under constant returns to scale, the highest average efficiency score was reached with 0.844 in 2015 while window length was 1. The lowest average efficiency score of 0.748 was obtained when window length was 5. It was found that the number of units in CCR Model 1 for 2018 had a negative significant effect on the efficiency scores of the centers, whereas number of patients examined, number of tooth extractions and conservative treatment had a positive significant effect on the efficiency scores of the centers. Tobit analysis was performed by taking the efficiency scores obtained from DEA as dependent variable and the results were interpreted. Model 1 CCR results for 2018, suggested that the number of units of input variables and the total number of dentists be reduced by 21.81 and 24.17%, respectively. Output variables suggested that the number of endodontic treatments, number of tooth extraction, the number of patients examined and the number of conservative treatments be increased by 34.59, 13.21, 3.45 and 2.77%, respectively.
