Yaşa Bağlı Maküla Dejenerasyonu (YBMD) görme kaybı ile karakterize progresif bir hastalıktır. Patogenezinde vasküler endotelyal büyüme faktörü (VEGF) önemli yere sahiptir. Bu nedenle tedavide Anti-VEGF ajanlar kullanılmaktadır. Ancak literatürde bu ajanların makülada atrofi gelişimi üzerinde etkileri bulunduğuna dair bilgiler bulunmaktadır. Bu tez çalışmasında Anti-VEGF ajanlardan ranibizumab, aflibercept ve ziv-afliberceptin resveratrol ile kombinasyonunun ARPE-19 hücreleri üzerindeki etkilerinin in-vitro olarak araştırılması amaçlanmıştır. Bu amaç doğrultusunda sitozolik ve mitokondriyal reaktif oksijen türlerinin üretimi (SitROS ve MitROS), mitokondriyal membran depolarizasyonu (MitDep), kaspaz-3, kaspaz-8 ve kaspaz-9, hücre canlılığı, apoptozis ve VEGF-A düzeylerindeki değişimler konfokal mikroskop, mikro tabaka okuyucu (plate reader) ve ELISA yöntemleri kullanılarak ölçülmüştür. ARPE-19 hücreleri Kontrol, Kobalt Klorür (Co), Resveratrol (RES), Ranibizumab (RAN), Aflibercept (AFB), Ziv-aflibercept (ZAB), RAN+RES, AFB+RES, ZAB+RES, RAN+RES+Co, AFB+RES+Co ve ZAB+RES+Co şeklinde 12 gruba ayrıldı. Kontrol grubu dışındaki gruplardaki hücreler RES, RAN, AFB veya ZAB (yalnız veya kombine edilerek) ile 24 saat boyunca inkübasyon sonrasında ilgili gruplar kobalt klorür (CoCl2) ile 24 saat daha inkübe edildiler. Çalışmamızda Anti-VEGF'ler yalnız uygulandıklarında MitROS, MitDep ve SitROS'u artırarak oksidatif stresi artırdı. Kaspaz-3,8 ve 9 düzeyini ve apoptozisi artırıp hücre canlılığını azalttılar. Resveratrol ile Anti-VEGF'ler kombine uygulandığında resveratrol Anti-VEGF'lerin artırdığı oksidatif stresi azalttı. Kaspaz-3 ve 9 seviyelerini ve apoptozisi azaltarak hücre canlılığını artırdı. Sonuçlarımız in-vitro ortamda ranibizumab, aflibercept ve ziv-afliberceptin oksidatif stresi artırıcı ve hücre canlılığını azaltıcı etkileri olduğunu göstermiştir. Age-Related Macular Degeneration (AMD) is a progressive disease characterized by vision loss. Due to the Vascular Endothelial Growth Factor (VEGF) having a crucial role in the pathogenesis of AMD, Anti-VEGF agents are used for treatment. However, it is indicated that those agents may cause the progression of atrophy development in the macula. It is aimed to investigate the effects of Anti-VEGF agents ranibizumab, aflibercept and ziv-aflibercept combined with resveratrol on ARPE-19 cells in-vitro. In accordance with this purpose alterations in the levels of cytosolic reactive oxygen spices (CytROS), mitochondrial reactive oxygen spices (MitROS), mitochondrial membrane depolarization (MitDep), caspase-3, caspase-8 and caspase-9, cell viability, apoptosis and VEGF-A were measured by using confocal microscope, plate reader and ELISA methods in this study. The cells were divided into twelve groups described as follow: Control, Cobalt Chloride (Co), Resveratrol (RES), Ranibizumab (RAN), Aflibercept (AFB), Ziv-aflibercept (ZAB), RAN+RES, AFB+RES, ZAB+RES, RAN+RES+Co, AFB+RES+Co and ZAB+RES+Co. Except for the Control group (48 hours incubation with no treatment), all the other groups were treated with resveratrol, ranibizumab, aflibercept or ziv-aflibercept (alone or combined) for 24 hours then the cells in the merely related groups were incubated with Cobalt Chloride (CoCl2) for 24 hours more. It was revealed that alone treatment with an Anti-VEGF agent causes increment on cellular oxidative status level by prompting the MitROS, MitDep and SitROS levels. Thus, it increased the caspase-3, caspase-8 and caspase-9 levels resulting in enhancing apoptosis or on the contrary decreasing cell survival. Consequently, our findings indicate that ranibizumab, aflibercept and ziv-aflibercept may have a provoker effect on cellular oxidative stress and a suppressor effect on apoptosis in-vitro. Besides, it is concluded that combination with resveratrol has valuable and non-negligible potential for intercepting the devastating effects of Anti-VEGF agents.
