Tiroid tümörleri günlük pratikte en sık karşılaşılan endokrin tümörlerdir.(1) Klinikte tiroid nodülü popülasyonun yalnızca %5-10'unda gözlenmekte olup, bunların çoğunluğu benign foliküler neoplaziler iken yalnızca %2-5'i maligndir. Olguların %95'inden fazlası tiroid primer malignitesidir ve epitelyal orijinlidir.(1, 2). Anjiogenez büyüme aşamasında fizyolojik bir süreçtir ve önceden var olan damarlardan yeni damarların gelişmesi ve yenilenmesi anlamına gelir.(3) Tümör büyümesinde ve yayılmasında anjiogenezis kritik öneme sahiptir. Tümör ile ilişkili anjiogenik faktörler tümör hücrelerinden ve tümördeki inflamatuar hücre infiltrasyonundan sağlanır(4). Antianjiyogenik ilaçların kanser tedavisi için artan kullanımı, onlarca yıllık kapsamlı temel ve klinik araştırmalardan ortaya çıkmıştır. Bununla birlikte, bu tür ilaçların klinik yararları nispeten düşüktür. Gelişmelerin, tümör anjiyogenezini yöneten moleküler ve hücresel mekanizmaların ve antianjiyogenik tedavilere verilen yanıtın daha kapsamlı bir şekilde anlaşılmasından gelmesi muhtemeldir.(5) Tümör ile ilişkili bu neovasküler gelişim ilerleyen dönemde hem tanı açısından hem de hastalık seyri açısından dikkate değer bir bulgudur. Araştırmamızın amacı tümöral ve non-tümöral tiroid dokusunda izlenen vasküler yapının araştırılması ve ayrıcı tanı, klinikopatolojik seyir ve olası tedaviler açısından yol gösterici olmasıdır. Çalışmamıza Süleyman Demirel Üniversitesi Tıp Fakültesi Tıbbi Patoloji Anabilim Dalı'nda 2014-2020 tarihleri arasında retrospektif arşiv taraması yapılarak rastgele seçilen 47 tiroid papiller karsinom, 16 NİFTP, 9 Malignite potansiyeli belirsiz iyi diferansiye neoplazi, 9 foliküler karsinom, 11 tiroid medüller karsinom olmak üzere toplam 95 olgu çalışma grubunu oluşturmuştur. 9 adet olan malignite potansiyeli belirsiz iyi diferansiye neoplazi yeni sınıflamaya göre tekrar değerlendirilmiş olup NİFTP kategorisine dahil edilmiştir. Elde edilen sonuçlar ile PSMA ekspresyonunun tümöre ait yeni oluşan damarlarda ekspresyonu izlenmiş olup non-tümöral tiroid vasküler yatağında spesifik ekspresyon görülmemiştir. Ayrıca yeni bir antite olan nisbeten iyi seyirli NİFTP vakalarında da diğer tümörlere göre ekspresyon görülmemiş ve ayrıcı tanıda düşünülmesi ve üzerine çalışmalar eklenmesi gerekmektedir. Hastalık seyri ve lenfovasküler invazyon, kapsül invazyonu, metastaz ve tiroid dışı invazyonlar ile anlamlı ilişki saptanması ayrıca bu ekspresyonun kötü prognozla ilişkili olduğunu ifade etmektedir. PSMA'nın damar endotellerinde olan pozitifliği ile immünohistokimyasal olarak teşhiste, serum seviyeleri klinik izlemde, radyolojik görüntüleme ile klinik evrelemede ve immünterapötik ilaç olarak tedavide kullanılabilir. Gelişen teknoloji sayesinde ileriki yıllarda radyoligand yöntemiyle non-tümöral ve tümöral damarlanmayı takip ederek yeni tedavi yöntemleri sunabilirler. Thyroid tumors are the most common endocrine tumors in daily practice. (1) Thyroid nodules are observed in only 5-10% of the population in the clinic, the majority of which are benign follicular neoplasia, while only 2-5% are malignant. More than 95% of cases are thyroid primary malignancies and of epithelial origin. (1, 2). Angiogenesis is a physiological process at the stage of growth and means the development and renewal of new vessels from pre-existing vessels. (3) Angiogenesis is critical in tumor growth and spread. Angiogenic factors associated with the tumor are providedfrom tumor cells and inflammatory cell infiltration in the tumor (4). The increasing use of antiangiogenic drugs for cancer treatment has emerged from decades of extensive basic and clinical research. However, the clinical benefits of such drugs are relatively low. The developments are likely to come from a broader understanding of the molecular and cellular mechanisms governing tumor angiogenesis and the response to antiangiogenic therapies. (5) This neovascular development associated with the tumor is a remarkable finding both in terms of diagnosis and in terms of disease course in the future. The aim of our research is to investigate the vascular structure monitored in tumoral and non-tumoral thyroid tissue and to guide differential diagnosis, clinicopathological course and possible treatments. Our study included 47 thyroid papillary carcinomas, 16 NIFTPs, 9 Malignancy potentially unspecified good differential neoplasia,9 follicular carcinoma, 11 thyroid medullary carcinomas which were randomly selected by retrospective archival screening in the Department of Medical Pathology at Suleyman Demirel University Faculty of Medicine between 2014 and 2020. A total of 95 cases constituted the study group. 9 malignant potential unspecified good differential neoplasia was reassessed according to the new classification and included in the NIFTP category. With the results obtained, PSMA expression was observed in newly formed vessels belonging to the tumor and specific expression was not seen in the non-tumoral thyroid vascular bed. In addition, in cases of nifty good-moving NİFTP, which is a new antithesis, expression is not seen compared to other tumors and should be considered in discriminatory diagnosis and studies should be added. The course of the disease and the determination of a significant relationship with lymphovascular invasion, capsule invasion, metastasis and non-thyroid invasions also indicate that this expression is associated with poor prognosis. With the positivity of PSMA in vascular endotels, immunohistochemical diagnosis, serum levels can be used in clinical monitoring, radiological imaging, clinical staging and treatment as immunotherapy drugs. Thanks to the developing technology, they can follow non-tumoral and tumoral vascularization with radioligand method in the coming years and offer new treatment methods.
