Bu araştırmanın amacı, Multipl Skleroz (MS) hastalığının birinci ve ikinci basamak tedavisinde kullanılan hastalığı modifiye edici ilaç (Disease Modifying Drug- DMD) tedavileri ile semptomatik tedavi seçeneğinin geri ödeme kurumu perspektifinden maliyet etkililiğini ve bütçe etkisini analiz etmektir. Araştırmanın evrenini Süleyman Demirel Üniversitesi Araştırma ve Uygulama Hastanesi'nde 2019 yılında tedavi alan MS hastaları oluşturmaktadır. Araştırmada örneklem çekilmemiş ve evrenin tamamı araştırma kapsamına alınmıştır. MS tedavi seçeneklerinin maliyeti ve etkililiği Markov modeli ile tespit edilmiştir. Modelde yer alan sağlık durumları hasta değerlendirmeli EDSS ölçeği ile belirlenmiştir. Araştırmada kullanılan maliyet verileri hasta faturalarından, etkililik değerleri ise hastalara uygulanan EQ-5D-5L ölçeği ile tespit edilmiştir. Modelde hastaların sağlık durumları arasındaki geçişleri için döngü uzunluğu 1 yıl, zaman dilimi ise 10 yıl olarak belirlenmiştir. Araştırmada MS tedavi seçeneklerinin toplam maliyeti ve toplam kaliteye ayarlı yaşam yılı (QALY) hesaplanmış ve hesaplanan sonuçlara %3 indirgeme yapılmıştır. Sonuçlar ilave maliyet etkililik oranı (İMEO) olarak sunulmuştur. Ayrıca araştırmada ikinci sonuç ölçütü olarak net parasal fayda (Net Monetary Benefit-NMB) hesaplanmıştır. Maliyet-etkililik analizinde tek yönlü ve olasılıksal duyarlılık analizi (Monte Carlo simülasyonu) yapılmıştır. Araştırmada Markov modelinden elde edilen sonuçlara göre QALY başına ilave maliyet etkililik oranı (İMEO) sırası ile Interferon β-1a IM'nin 347.100,57 TL, Interferon β-1a SC'nin 314.811,38 TL, Interferon β-1b SC'nin 258.885,00 TL, Glatiramer Asetat'ın 249.169,88 TL, Teriflunomid'in 296.197,43 TL, Dimetil Fumarat'ın 305.498,22 TL, Fingolimod'un 279,858,88 TL, Natalizumab'ın 535.805,05 TL ve Okrelizumab'ın 480.487,77 TL'dir. Araştırma sonunda 156.948 TL'lik bir eşik değerde semptomatik tedavi seçeneği ile karşılaştırıldığında DMD'lerle tedavilerin maliyet etkili olmadığı tespit edilmiştir. MS tedavi seçenekleri arasında parasal açıdan en yüksek NBM değerine sahip seçeneğin 776.656,28 TL ile semptomatik tedavi olduğu tespit edilmiştir. Tek yönlü duyarlılık analizi, belirlenen parametre değişimlerinde kararlı bir sonuç göstermiştir. Olasılıksal duyarlılık analizinde DMD'lerle tedavilerin 156.948 TL eşik değerde karşılaştırmaların %100'ünde maliyet etkili olmadığı tespit edilmiştir. Bununla birlikte 313.896 TL'lik bir eşik değerde Glatiramer Asetat'ın %96,8 olasılıkla ve ardından Interferon β-1b SC'nin ise %3,2 olasılıkla maliyet etkililik karşılaştırmalarında optimal (maliyet etkili) seçim haline geleceği tespit edilmiştir. Bütçe etki analizinde 2019 yılındaki toplam sağlık harcamalarının % 0,48'inin MS tedavilerine harcandığı tespit edilmiştir. Sonuç olarak MS tedavisinde kullanılan DMD'ler yüksek sağlık kazanımlarına rağmen oldukça maliyetli tedavilerdir. Bu nedenle DMD tedavilerinin maliyetinin düşürülmesine yönelik sağlık politikalarının geliştirilmesi ve ilaç fiyatları döviz kurlarındaki artıştan etkilendiği için Türkiye'de QALY başına eşik değerin dövize endeksli olarak belirlenmesi önerilmektedir. The purpose of this study is to analyze the cost-effectiveness and budget effect of the disease-modifying drug (DMD) treatments used in the first and second-line treatment of Multiple Sclerosis (MS) and the symptomatic treatment option from the perspective of the reimbursement institution. The population of the research consists of MS patients who received treatment at Süleyman Demirel University Research and Application Hospital in 2019. No sample was drawn in the study and the entire population was included in the research. The cost and effectiveness of MS treatment options were determined by the Markov model. The health status in the model were determined by the patient-evaluated EDSS scale. The cost data used in the study were determined from the patient bills, and the effectiveness values were determined by the EQ-5D-5L scale applied to the patients. In the model, the cycle length for the transition of patients between health statuses is 1 year, and the time period is 10 years. In the study, the total cost of MS treatment options and the total quality-adjusted life years (QALY) were calculated and a 3% discount was made to the calculated results. The results are presented as additional cost effectiveness ratio (ICER). In addition, net monetary benefit (Net Monetary Benefit-NMB) was calculated as the second outcome measure in the study. In cost-effectiveness analysis, one-way and probabilistic sensitivity analysis (Monte Carlo simulation) was performed. According to the results obtained from the Markov model in the study, the additional cost effectiveness ratio (IMEO) per QALY were 347.100,57 TL for Interferon β-1a IM, 314.811,38 TL for Interferon β-1a SC, 258.885,00 TL for Interferon β-1b SC, 249.169,88 TL for Glatiramer Acetate, 296.197,43 TL for Teriflunomide, 305.498,22 TL for Dimethyl Fumarate, 279,858,88 TL for Fingolimod 535.805,05 TL for Natalizumab and 480.487,77 TL for Okrelizumab, respectively. At the end of the study, it was determined that treatments with DMDs were not cost effective when compared with the symptomatic treatment option at a threshold value of 156.948 TL. Among the MS treatment options, it was determined that the option with the highest NBM value in terms of money was symptomatic treatment with 776.656,28 TL. One-way sensitivity analysis showed a stable result in the specified parameter changes. In the probablistic sensitivity analysis, it was found that treatments with DMDs were not cost effective in 100% of comparisons with a threshold value of 156.948 TL. However, at a threshold value of 313.896 TL, it was determined that Glatiramer Acetate with 96.8% probability followed by Interferon β-1b SC with 3.2% probability would become the optimal (cost effective) choice in cost effectiveness comparisons. In the budget impact analysis, it was determined that 0.48% of the total health expenditures in 2019 was spent on MS treatments. As a result, DMDs used in the treatment of MS are very costly despite their high health gains. It is recommended to develop health policies to reduce the cost of DMDs and to determine the threshold value per QALY in foreign currency indexed in Turkey, since drug prices are affected by the increase in exchange rates.
