Karotis web görülme sıklığını belirlemeyi, karotis web olgularının klinik verilerini, web morfolojik özelliklerini ve vasküler risk faktörlerini değerlendirerek iskemik inme/geçici iskemik atak ile ilişkisini belirlenmeyi ve alınabilecek önlemler ile gelecekteki en uygun tedavi yaklaşımlarına katkıda bulunmayı amaçladık. Süleyman Demirel Üniversitesi Araştırma ve Uygulama Hastanesi ve Isparta Şehir Hastanesi'ne 2016-2022 tarihleri arasında serebrovasküler hastalık ön tanısı ile başvuran, Karotis BTA ve/veya DSA yapılan toplam 1520 hastanın anjiyografik görüntülemelerini inceledik. Bu olgular içerisinden karotis web saptanan 31 hastayı çalışmamıza dahil ettik. Bu hastaların hastane yatış/poliklinik kayıtları, radyolojik görüntülemeleri, laboratuvar bulguları ve klinik bilgileri retrospektif olarak değerlendirildi. Çalışmamızda karotis web genel prevalansı %2,03 olarak bulundu. Karotis web saptanan olguların %35,5'i semptomatikti. Karotis web hastalarında en sık görülen vasküler risk faktörü hipertansiyondu. Karotis web yeri olguların %74,2'sinde ICA bulbustaydı. Web hastalarının %74,2'sinde oblik yönde karotis web saptadık ve semptomatik hastaların tamamında web yarık yönünün oblik olduğunu gördük. Web uzunluğu ortalaması 2,7±0,9 mm'di. Web uzunluğu açısından semptomatik hastalarda optimal eşik değeri ≥ 2,86 mm'di. Web uzunluğu ile web darlık derecesi arasında orta düzeyde pozitif yönde doğrusal bir ilişki saptadık. Webe bağlı darlık derecesi ortanca değeri %31 olarak bulundu. Tüm hastaların %87,1'inde hafif derecede(%30-49) darlık saptandı. Web darlık derecesi açısından semptomatik hastalarda optimal eşik değeri ≥ %31,33 bulundu. Sonuçlarımız toplumumuzda karotis web prevalansı ve semptomatik olma sıklığının incelenen diğer popülasyonlar ile benzer olduğunu, semptomatik olan ve olmayan gruplar arasında KAH dışında risk faktörlerinde farklılık olmadığını göstermiştir. Çalışmamız karotis webin bazı morfolojik özelliklerinin inme riskini arttırdığını ve bu özelliklere göre geliştirilecek karotis web sınıflamasının ve risk skorlama sistemlerinin inme riskini öngörmede ve tedavi düzenlenmesinde etkili olabileceğini ortaya koymaktadır. We aimed to determine the frequency of carotid web, determine its relationship with ischemic stroke/transient ischemic attack by evaluating the clinical data, web morphological features and vascular risk factors of carotid web cases. With these, we aimed to contribute to the most appropriate treatment approaches in the future with the precautions that might be taken. We examined the angiographic images of a total of 1520 patients who applied to Süleyman Demirel University Research and Application Hospital and Isparta City Hospital between 2016 and 2022 with an early diagnosis of cerebrovascular disease and who underwent Carotid CTA and/or DSA. Among these cases, we included 31 patients with carotid web in our study. Hospitalization/polyclinic records, radiological imagings, laboratory findings and clinical information of these patients were evaluated retrospectively. In our study, the general prevalence of carotid web was found to be 2.03%. 35.5% of the cases in which carotid web was detected were symptomatic. The major vascular risk factor for carotid web patients was hypertension. The carotid web location in 74.2% of the cases was in the bulb of the ICA. We detected an oblique carotid web in 74.2% of the web patients and found that the web cleft direction was oblique in all symptomatic patients. The average web length was 2.7±0.9 mm. In terms of web length, the optimal cut-off value in symptomatic patients was ≥ 2.86 mm. We detected a moderately positive linear relationship between web length and the degree of stenosis in the web. The median value of web-related stenosis degree was found to be 31%. Mild degree (30-49%) stenosis was detected in 87.1% of all patients. In terms of the degree of web stenosis, the optimal cut-off value in symptomatic patients was found to be ≥ 31.33%. Our results showed that the prevalence of carotid web and the frequency of being symptomatic in our population were similar to other examined populations. Furthermore there weren't any differences in terms of risk factors, other than coronary artery disease, between symptomatic and non-symptomatic groups. Our study shows that some morphological features of the carotid web increase the risk of stroke and that carotid web classification and risk scoring systems which developed according to these features may be effective in predicting the risk of stroke and regulating treatment.
