Amaç: Çalışmanın amacı; eksudatif plevra sıvılarının ayrımında, serum ve plevral sıvı adenozin deaminaz (ADA), yüksek duyarlıklı C-reaktif protein (Hs-CRP) ve alfa-1- asit glikoprotein (AAG) düzeyleri ölçümünün tanısal verimliliğini araştırmaktı. Materyal ve metod: Çalışmaya tanıları PPE, TBP ve MPE olan hastalar dahil edildi. Serum ve plevral sıvı örneklerinde ADA, CRP ve AAG düzeyleri ölçüldü. Tüm hasta gruplarında serum ve plevral sıvı CRP, ADA ve AAG düzeylerinin ortalamałSS.ları hesaplandı; her bir parametre için ölçülen serum ve plevral sıvı düzeyleri arasında korelasyon olup olmadığı araştırıldı. Eksudatif plevral efüzyonların ayrımında biyomarkırların duyarlılık ve özgüllüğü, ROC eğrisi ve AUC (area under the curve) analizleri ile değerlendirildi. Tüm parametrelerin aynı anda değerlendirilmesinin tanıya katkısı ise multinominal logit regresyon analizi ile incelendi. Bulgular: Çalışmaya 37 PPE, 40 MPE ve 16 TBP.li hasta hasta dahil edildi. PPE grubu, tipik PPE (TPPE) (n=18) ve komplike PPE (KPPE) (n=19) grubu olarak ayrıldı. İstatistiksel değerlendirmeler önce tüm PPE olguları dahil edilerek, daha sonra KPPE olguları dışarıda bırakılarak yapıldı.Serum - plevral sıvı CRP ve ADA düzeyleri arasında pozitif anlamlı bir korelasyon saptanırken, serum - plevral sıvı AAG düzeyleri arasında korelasyon olmadığı görüldü. ROC analizi ile ölçümler değerlendirildiğinde, serum CRP (>60,3 mg/l) ve plevral sıvı CRP düzeylerinin (>20,1 mg/l) yüksek ve plevral sıvı ADA düzeyinin düşük (<54,3 U/l) ölçülmesinin PPE tanısı koymaya yardımcı olabileceği; plevral sıvı ADA düzeyi ölçümünün (>82,9 U/l) TBP tanısına katkı sağlayacağı, AAG serum ve plevra sıvısı düzeyi ölçümlerinin ise MPE tanısına katkı sağlamadığı görüldü. Plevral sıvı CRP düzeyi için 20,1 mg/l değeri eşik-değer olarak alındığında, PPE olgularını diğer olgulardan ayırmada duyarlılık ve özgüllük sırasıyla %72,7 ve %60,7; plevra sıvısı ADA düzeyi için eşik-değer 82,9 U/l olarak alındığında ise TBP grubunu diğer olgulardan ayırmada duyarlılık ve özgüllük sırasıyla %75 ve %84,5 olarak saptandı. Bakılan tüm parametrelerin multinominal logit regresyon analizi ile aynı anda değerlendirilmesi sonucunda ise, olguları doğru tanıya sınıflandırma başarısı %73 olarak belirlendi. Sonuç: Eksudatif plevral efüzyonlu olguların ayırıcı tanısında, plevral sıvı ADA ve CRP ölçümü, TBP ve PPE olgularını ayırmada kullanışlı parametrelerdir. Ancak, ne serum ne de plevral sıvı AAG ölçümünü MPE olgularını diğer gruplardan ayırmada yararlı değildir. Tanı güçlüğü çekilen TBP olgularında halen en sık kullanılan biyokimyasal tanı yöntemi, plevral sıvıda ADA düzeyinin ölçümü olmakla birlikte, bu çalışmada plevral sıvıda CRP düzeyi ölçümünün bu olguların en azından PPE grubundan ayrılmasına yardımcı olabileceği saptanmıştır.

Aim: The aim of the study was to evaluate the diagnostic efficacy of measuring serum and pleural effusion adenosine deaminase (ADA), high-sensitive C-reactive protein (Hs-CRP) and alpha-1 acid glycoprotein (AAG) levels in the differentiation of the patients with exudative pleural effusions. Materials and method: The patients with diagnoses of PPE, TBP and MPE were included in the study. ADA, CRP and AAG levels were measured in the serum and pleural effusion samples. Mean ł SD of serum and pleural fluid CRP, ADA and AAG levels were calculated in all three patient groups; correlations were investigated for all parameters between measured serum and pleural fluid levels. The sensitivity and specificity of the biomarkers for the differentiation of exudative pleural effusions were assessed by the analyses of ROC curves and AUC (area under the curve) measurements. The diagnostic value of concomitantly assessing all the parameters was investigated by the multinominal logit regression analysis. Results: 37 PPEs, 40 MPEs and 16 TBPs were included in the study. The PPE group were divided as typical PPEs (n=18) and complicated PPEs (n=19). Statistical analyses were performed first including the whole PPE cases and later excluding the cases with complicated PPEs. There was a statistically significant correlation between serum and pleural fluid CRP and ADA levels, whereas no significant correlation was observed between serum and pleural fluid AAG levels. The assessment of measurements by the ROC curve analyses revealed that a high serum CRP (>60.3 mg/L), pleural fluid CRP (>20.1 mg/L) and a low pleural fluid ADA level (<54.3 U/L) might help in diagnosing PPE while a high pleural fluid ADA level (>82.9 U/L) might help in diagnosing TBP. However, neither serum nor pleural fluid AAG measurements were useful in diagnosing MPEs. When a cut-off value of 20.1 mg/L was chosen for pleural fluid CRP level, the sensitivity and spesificity of the test in differentiating PPE cases from other patients was found as 72.7% and 60.7%, respectively; while a cut-off value of 82.9 U/L for the pleural fluid ADA level revealed a sensitivity and specificity of 75% and 84,5%, respectively, in differentiating TBP group from other cases. With concomitant evaluation of all parameters by multinominal logit regression analysis, the patients were accurately classified according to their diagnoses, with a ratio of 73%. Conclusion: Pleural fluid ADA and CRP measurements are useful parameters in differentiating TBP and PPE cases among patients with exudative pleural effusions. Neither serum nor pelural fluid AAG level measurements provide help in differentiating MPE cases from other patient groups. Although, the most commonly used diagnostic biochemical method for diagnosing the TBP cases is the measurement of pleural fluid ADA level, measuring the pleural fluid CRP level was found to be helpful in differentiating at least cases with TBP and PPE, in the current study.
