Sağlık alanında yapılan çalışmalarda, birden fazla faktörlü ve seviyeli faktöriyel düzenlemeler yapılmakta kombinasyon sayısının fazla olması sebebi ile de daha fazla hasta sonuçlarının elde dilmesinde yaşanan zorlukların azaltılabilmesi ve zamandan tasarruf sağlayabilmek için kombinasyon sayısını azaltan bir yöntem olan Taguchi yönteminin sağlık alanında uygulanabilirliğini test etmek amaçlanmıştır. Taguchi yöntemi, ortogonal dizilerden yararlanarak, hedeflenen değere ulaşmak amacıyla deney sayısını azaltmayı amaçlamıştır. Bu sayede daha az deney sayısı ile kontrol edilemeyen faktörlerin etkisini de en aza indirgemeyi sağlamaktadır. Bu çalışmada Taguchi yönteminin kullanılabilirliğinin test edilmesi adına, farklı seviyelerdeki faktörlerin değişkenlere etkisi araştırılmıştır. Bu amaçla üç farklı uygulama yapılmıştır. İlk uygulamada mekanik ventilatör ve akut böbrek yetmezliği faktörlerinin yoğun bakım yatış süresine etkisini incelemek için, iki seviyeli iki faktörlü ANOVA uygulanmış faktörlerin ana etkileri ve interaksiyonlar istatistiksel olarak önemli bulunmuştur. Uygulanan Taguchi yönteminde tesadüfi olacak şekilde L32 ortogonal dizini oluşturulmuş ve sonuçlar incelendiğinde akut böbrek yetmezliğinin yoğun bakım yatış süresine etkisinin daha önemli düzeyde olduğu saptanmıştır. Faktörlerin seviyelerine bakıldığında ise hem mekanik ventilatör için hem de akut böbrek yetmezliği için 2. seviyelerinin en yüksek yoğun bakım yatış süresi kombinasyonunu oluşturduğu gözlenmiştir. İkinci olarak tanı ve glomerular filtrasyon hızı faktörlerinin kreatinin ortalaması için üç seviyeli iki faktörlü ANOVA uygulanmış faktörlerin ana etkileri ve interaksiyonları istatistiksel olarak önemli bulunmuştur. Taguchi yöntemi uygulamasında tesadüfi olacak şekilde L27 ortogonal dizini tasarlanmış ve kreatinin değeri ortalaması için en önemli faktörün glomerular filtrasyon hızı olduğu gözlenmiştir. Faktör seviyeleri incelendiğinde en yüksek kreatinin ortalamasını tanı faktörünün 2. seviyesi ve glomerular filtrasyon hızı faktörünün 1. seviyesi kombinasyonu vermiştir. Son uygulama olarak ta mekanik ventilatör ve kalp hızı faktörlerinin satürasyon ortalaması için iki ve üç seviyeli iki faktörlü ANOVA uygulanmış ve ana etkilerin ve interaksiyon etkisinin istatistiksel olarak önemli olduğu gözlenmiştir. Taguchi yöntemi uygulamasında L18 ortogonal dizin kullanılmış ve satürasyon ortalaması için en önemli faktörün mekanik ventilatör olduğu belirlenmiştir. Faktör seviyeleri incelendiğinde ise mekanik ventilatör ve kalp hızının 1. seviyesi en yüksek satürasyon ortalaması kombinasyonunu oluşturmaktadır. Sonuç olarak üç uygulamada da ANOVA ve Taguchi yönteminde benzer sonuçlar elde edilmiştir. Bu sayede Taguchi yönteminin sağlık araştırmalarında uygulanabilir olduğu ki böylece deneme kombinasyonu sayısı azaltılarak uygulanacak olan çalışmaların zaman tasarrufu sağlayabileceği gösterilmiştir.

In the field of health, multi factorial analysis or factorial design levels are are used. The aim of this study is to test the applicability of the Taguchi method that reduces the number of combinations, to reduce the difficulties experienced in obtaining more patient results and save time due to the high number of combinations. The Taguchi method uses the orthogonal indices to reduce the number of experiments. Therefore, it also minimizes the effect of uncontrollable factors with less number of experiments. In this study, the effects of factors at different levels on the variables were investigated to test the applicability of the Taguchi method. For this purpose, three different factorial structure were designed. Firstly, the effects of mechanical ventilator and acute renal failure factors on length of stay in intensive care unit were examined, and the main effects of the factors and interactions were found to be statistically significant. By applying the Taguchi method, the L32 orthogonal index was created randomly, and it was determined that the effect of acute renal failure on the length of stay in the intensive care unit was more significant. When the levels of the factors were examined, it was determined that the 2nd level constituted the highest ICU length of stay combination for both mechanical ventilator and acute renal failure. Second, the main effects and interactions of the three-level two-way ANOVA were found to be statistically significant for the mean creatinine of the diagnosis and glomerular filtration rate factors. In the application of the Taguchi method, the L27 orthogonal index was designed randomly and it was determined that the most important factor for creatinine was the glomerular filtration rate. When the factor levels were examined, the combination of the 2nd level of the diagnostic factor and the 1st level of the glomerular filtration rate factor gave the highest mean creatinine. As a final application, two- and three-level two-way ANOVA was applied for the mean saturation of mechanical ventilator and heart rate factors, and it was determined that the main effects and interaction effects were statistically significant. L18 orthogonal index was used in the application of the Taguchi method, and it was determined that the most important factor for the mean saturation was the use of mechanical ventilator. When factor levels are examined, the first level of mechanical ventilator and heart rate constitutes the highest average saturation combination. As a result, similar results were obtained in ANOVA and Taguchi method in all three applications. It can be concluded that the Taguchi method can be applied in health researchs, so that the number of trial combinations can be reduced and it can be time saving.

When the factor levels were examined, the combination of the 2nd level of the diagnostic factor and the 1st level of the glomerular filtration rate factor gave the highest mean creatinine. As a final application, two- and three-level two-way ANOVA was applied for the mean saturation of mechanical ventilator and heart rate factors, and it was determined that the main effects and interaction effects were statistically significant. L18 orthogonal index was used in the application of the Taguchi method, and it was determined that the most important factor for the mean saturation was the use of mechanical ventilator. When factor levels are examined, the first level of mechanical ventilator and heart rate constitutes the highest average saturation combination. As a result, similar results were obtained in ANOVA and Taguchi method in all three applications.
