AmaçHashimoto tiroiditi otoimmün tiroid hastalığıdır. Aterosklerozdaarterlerin subintima tabakasında lipidler,karbonhidratlar, fibröz doku, kalsiyum gibi maddelerinlokal birikimlerinin oluşturduğu patolojik bir durumsöz konusudur. Vasküler endotelyal büyüme faktör(VEGF) özellikle hipoksi varlığında seçici olarak endotelhücrelerinde mitojenik etki ile damar gelişiminiuyararak yeni damar oluşumunda yaşamsal rol oynar.Biz bu çalışmamızda ötiroid hashimoto hastalarındaserum VEGF düzeyi ile akım aracılı dilatasyon(flow-mediated dilation, FMD) ve karotis intima mediakalınlığı (KİMK) ölçümü yaparak, otoimmünitenin tiroidhastalarında ateroskleroz ile ilişkisini ortaya koymayıamaçladık.Gereç ve YöntemGaziantep Üniversitesi Tıp Fakültesi Dahiliye Kliniği’ndeHashimoto tiroiditi tanısı almış 40 hasta ile yaşve cinsiyet uyumlu 40 sağlıklı gönüllü çalışmaya dahiledildi. Serum örneklerinde VEGF düzeyleri ELISAyöntemiyle ölçüldü. Hasta ve kontrol grubunda ultrasoneşliğinde KİMK ve FMD değerlendirilerek mevcutsonuçlarla ilişkili olup olmadığı tespit edildi.BulgularHasta ve kontrol grubunun demografik özellikleri benzerdi.Hasta ve kontrol grupları arasında FMD veKIMK ölçümleri karşılaştırıldığında istatiksel olarakanlamlı fark saptandı (p=0,001; p=0,033). VEGF düzeyininde hasta grubunda kontrol grubuna göre anlamlıolarak yüksek olduğu tespit edildi (p=0,024).SonuçTüm bu sonuçlar tiroid hormon eksikliği olmadan daotoimmüniteye sekonder ateroskleroz gelişebileceğinive hashimoto hastalarının erken dönem takiplerindenoninvaziv uygulanabilen FMD, KİMK ölçümlerinin veVEGF düzeylerinin prediktif değerinin olabileceğinigöstermektedir. |

ObjectiveHashimoto thyroiditis is an autoimmune thyroiddisease. Atherosclerosis is a pathological conditioncharacterized by local accumulation of substancessuch as lipids, carbohydrates, fibrous tissue andcalcium in the subintima layer of arteries. In thisstudy, we aimed to reveal the relationship betweenautoimmunity and atherosclerosis by measuringserum VEGF levels, flow-mediated dilation (FMD)and carotid intima-media thicknes (CIMT) in euthyroidpatients with Hashimoto thyroiditis.Materials and MethodsForty patients diagnosed with Hashimoto thyroiditis inthe Internal Medicine Clinic of Gaziantep UniversityMedical Faculty and 40 healthy volunteers matchedfor age and gender were included in the study.VEGF levels were measured by ELISA method inserum samples. CIMT and FMD were evaluatedwith ultrasound guidance in the patient and controlgroups and it was determined whether there was aassociation with the present results.ResultsThe demographic characteristics of the patient andcontrol groups were similar. There was a significantdifference between the patients and healthy controlgroup in terms of FMD and CIMC measurements (p =0.001; p = 0.033). It was also found that VEGF levelsof patients were higher than those in the control group(p = 0.024).ConclusionAll these results show autoimmunity may leadatherosclerosis even without thyroid hormonedeficiency and also noninvasive FMD, CIMTmeasurements and VEGF levels may have predictivevalue in the early follow-up of Hashimoto patients. |
