Araştırmanın amacı büyükşehirlerdeki özel hastanelerden sağlık hizmeti alan hastalar/müşteriler ile daha küçük ölçekteki şehirlerdekilerin memnuniyetlerini belirleyen unsurlar ile sadakatleri arasındaki farklılığı ortaya koymaktır. Araştırmada Kocaeli Büyükşehir’de ve Aksaray ilinde toplamda 322 hasta/müşteri ile yüz yüze anket yapılmıştır. Verilerin analizinde tanımlayıcı istatistikler dışında gruplar arası kıyaslamalarda Mann Whitney U Testi ve Kruskal Wallis H analizi kullanılmıştır. Analiz sonuçlarına göre; her iki şehirde de gelir seviyesine göre memnuniyet ve sadakatte gruplar arasında farklılaşma olmadığı, ilde cinsiyete göre gruplar arasında fark yokken, büyükşehirde erkeklerin memnuniyet ve sadakat düzeyinin daha yüksek olduğu, hem ilde hem de büyükşehirde öğrencilerin hasta memnuniyeti ve sadakatinin daha düşük olduğu belirlenmiştir. Genel itibariyle değerlendirildiğinde ise özel hastanelerde hizmetten memnuniyet ve hasta sadakati bakımından sosyo-demografik özelliklere göre büyükşehir ve il arasında farklılıklar olduğu saptanmıştır.  The purpose of the study is to reveal the difference between the loyalties and the factors that determine the satisfaction of patients/customers receiving healthcare services from private hospitals in metropolitan cities and those in smaller cities. Three hundred twenty-two face to face questionnaires are conducted among the patients/customers in Kocaeli metropolitan and the city of Aksaray. In addition to descriptive statistics, Mann Whitney U Test and Kruskal Wallis H analysis are used to compare groups. Results show no differences in income in both cities; men are more satisfied and loyal in the metropolitan whereas there is no difference among the groups regarding gender in the city. The level of satisfaction and loyalty is lesser among student patients both in the metropolitan and the city. Results indicate that there are differences regarding the socio-demographics between the metropolitan and the city private hospitals in patients' satisfaction and loyalty.
