Diş eksikliği bireylerin yaşam kalitesini düşüren, sosyo-ekonomik yükü olan ve yüksek prevelansa sahip önemli bir halk sağlığı sorunudur. Diş eksikliğinde kullanılan tedavi seçenekleri, bireyler için oldukça maliyetlidir ve bu maliyetlerin hane halkı üzerinde de katastrofik bir etkisi bulunmaktadır. Bu araştırmanın amacı tek diş eksikliğinin tedavi yöntemleri olan diş destekli sabit protez ve implant destekli tek kronun maliyet etkililiğini analiz etmektir. Araştırmanın evrenini, Isparta Ağız ve Diş Sağlığı Merkezi'nde 2019 Ocak – 2020 Aralık tarihlerinde diş eksikliği tedavisi almış 149 hasta oluşturmaktadır. Araştırmada Markov modeli kullanılarak 20 yıllık zaman diliminde maliyetler ve kaliteye göre ayarlanmış protez yılı (QAPY - Quality-Adjusted Prosthetic Year) değerleri hesaplanmıştır. Hesaplanan değerler üzerinde %5 indirgeme uygulanmıştır. Sonuçlar ilave maliyet-etkililik oranı (İMEO) olarak sunulmuştur. Maliyet-etkililik analizinde belirsizliklerin etkisini değerlendirmek için tek yönlü ve olasılıksal duyarlılık analizi yapılmıştır. Araştırmada Markov modeli sonucuna göre; tek diş eksikliğinde kamu sağlık kurumunda alınan tedaviler için diş destekli sabit protezin maliyeti 9.087 ₺ (985 $), implant destekli tek kronun maliyeti 19.930 ₺ (2.161 $) olarak tespit edilmiştir. Özel klinikte oluşan maliyetler ise diş destekli sabit protez için 27.855 ₺ (3.021 $), implant destekli tek kron için 39.902 ₺ (3.718 $) olarak saptanmıştır. Her iki perspektifte de etkililik değeri diş destekli sabit protez için 7,3 QAPY iken, implant destekli tek kron için 8,2 QAPY olarak belirlenmiştir. Maliyet-etkililik analizi bulgularına göre; kamu sağlık kurumunda alınan alternatif tedavi yöntemlerinin QAPY başına İMEO'su 12.294 ₺ (1.333 $), özel klinikte alınan tedavilerin QAPY başına İMEO'su ise 13.659 ₺ (1.481 $) olarak saptanmıştır. Tek yönlü duyarlılık analizi sonucunda İMEO'nun, Markov modelindeki parametre girdilerinden, implant destekli tek kron tedavisinin başarılı durum maliyetinde yüksek hassasiyet gösterdiği tespit edilmiştir. Olasılıksal duyarlılık analizinde ise her bir simülasyonda implant destekli tek kronun ilave QAPY değeri ile birlikte ilave maliyet de getirdiği saptanmıştır. Eşik değer, kişi başına düşen Gayri Safi Yurtiçi Hasıla (GSYH) olarak kabul edildiğinde, her iki kurumda da iki tedavi yönteminin İMEO'su eşik değerin altında kalmaktadır. Eşik değer, kişi başına düşen GSYH'nin %15'i olarak kabul edildiğinde ise yalnızca kamu sağlık kurumundan alınan diş destekli sabit protez tedavisi eşik değerin altında kalarak maliyet etkili olduğu tespit edilmiştir. Sonuç olarak, tek diş eksikliği tedavi yöntemleri Markov modelinde her iki perspektiften de değerlendirildiği zaman implant destekli tek korunun daha yüksek QAPY değeri sağlamakla birlikte daha fazla maliyetli olduğu saptanmıştır. Bu bağlamda, implant destekli tek kron tedavisinin belirli bir oranda geri ödeme kapsamına alınması ve hasta üzerindeki mali yükün azaltılmasına yönelik sağlık politikaları geliştirilmelidir. Tooth-loss is an important public health problem that reduces the quality of life of individuals, has a socio-economic burden and has a high prevalence. Treatment options used in missing teeth are very costly for individuals and may have a catastrophic effect on households. The aim of this study is to analyze the cost-effectiveness of dental-supported fixed prosthesis and implant-supported single crown, which are the treatment methods of single missing-tooth. The population of the study consists of 149 patients who received missing-tooth treatment between January 2019 and December 2020 in Isparta Oral and Dental Health Center. In the study, using the Markov model was carried out and costs and quality-adjusted prosthetic year (QAPY) values were calculated over a 20-year time period. 5% discount was applied on the calculated values, and the results are presented as the incremental cost-effectiveness ratio (ICER). One-way and probabilistic sensitivity analysis (Monte Carlo simulation) was performed to evaluate the effect of uncertainties in cost-effectiveness analysis. According to the result of the Markov model in the research; the cost of dental-supported fixed prosthesis for treatments taken in a public health institution in the case of a single missing tooth was determined as 9,087 ₺ (985 $), and the cost of a single implant-supported crown was 19,930 ₺ (2.161 $). The costs incurred in the private clinic were determined as 27,855 ₺ (3.021 $) for dental supported fixed prosthesis and 39,902 ₺ (3.718 $) for implant supported single crown. In both perspectives, the efficacy value was determined as 7.3 QAPY for tooth-supported fixed prosthesis and 8.2 QAPY for implant-supported single crown. According to the cost-effectiveness analysis findings; the ICER per QAPY of the alternative treatment methods received in the public health institution was 12,294 (1.333 $), and the ICER per QAPY of the treatments received in the private clinic was 13,659 (1.481 $). As a result of one-way sensitivity analysis, it was determined that ICER, from the parameter inputs in the Markov model, showed high sensitivity in the successful case cost of implant-supported single crown therapy. on the other hand, In the probabilistic sensitivity analysis of each simulation, it was determined that the implant-supported single crown brought additional cost along with the additional QAPY value. When the threshold value is accepted as the Gross Domestic Product (GDP) per capita, the ICER of the two treatment methods in both institutions falls below the threshold value. When the threshold value is accepted as 15% of GDP per capita, only dental-supported fixed prosthesis treatment in a public health institution remains below the threshold value. As a result, when single missing-tooth treatment methods were evaluated from both perspectives in the Markov model, it was determined that the implant supported single crown provided higher QAPY value but was costlier. In this context, health policies should be developed to include implant-supported single crown treatment within the scope of reimbursement at a certain rate and to reduce the economic burden on the patient.
