Hodgkin lenfoma (HL) patogenezinde Cox-2 ve Bcl-2'nin önemi net olarak bilinmemektedir. Çalışmamızda Klasik HL'de (KHL) Cox-2, Bcl-2 ve LMP-1 ekspresyonları ile bunların klinikopatolojik parametreler ve prognoz ile ilişkilerinin incelenmesi amaçlandı. Çalışma 40 KHL (29 miks hücreli (MH), 11 nodüler sklerozan (NS)) olgusu içerdi. Bu olgulara ait arşiv bloklarından doku mikroarrayleri hazırlandı ve immünhistokimyasal yöntemle Cox-2, Bcl-2 ve LMP-1 antikorları uygulandı. Olgulara ait klinik bilgiler arşiv dosyalarından elde edildi. Çalışmamızda KHL alt tipleri arasında evre ve cinsiyet yönünden anlamlı farklılık saptandı. Yüksek evre olma (evre III-IV) oranı ile erkek cinsiyet oranı MHKHL alt tipinde NSKHL.ye göre anlamlı yüksek bulundu (sırasıyla, p=0.031, p<0.001). Cox-2, 20 (%50) KHL olgusunda pozitifti. Erkek cinsiyette Cox-2 negatifliği anlamlı olarak daha fazla bulundu (p=0.041). NSKHL.nin %81,8'i (9 olgu) ve MHKHL'nin %37,9'u (11 olgu) Cox-2 pozitifliği gösterdi ve bu istatistiksel olarak anlamlı farklıydı (p=0.031). Cox-2 ekspresyonu evre ile anlamlı negatif korelasyon gösterdi (p=0.042, r=-0.328). Bununla birlikte, istatistiksel olarak anlamlı olmasa da Cox-2 negatif olgular yüksek evreli grupta olma eğilimindeydiler. Bcl-2 ve LMP-1 sırasıyla 14 (%35) ve 23 (%57,5) KHL olgusunda pozitifti. LMP-1 pozitifliği MHKHL alt tipinde (%75,9) anlamlı daha fazla saptandı (p<0.001). Kaplan Meier log rank analizinde yüksek evre (evre III-IV) ile Cox-2 negatifliği kötü prognostik parametreler olarak bulundu (sırasıyla, p=0,004, p=0,010). Ancak, Cox-2, Bcl-2, LMP-1 ekspresyonları ile evreyi içeren Cox regresyon analizinde anlamlı bağımsız prognostik parametre saptanmadı. Sonuç olarak bu çalışmada KHL.de evrenin prognostik değeri doğrulanmıştır. Ayrıca bulgularımız Cox-2 negatifliğinin kötü prognostik parametrelerden biri olabileceğini düşündürmektedir. The importance of Cox-2 and Bcl-2 in the pathogenesis of Hodgkin.s lymphoma (HL) is not known exactly. The aim of this study is to examine Cox-2, Bcl-2 and LMP-1 expressions immunhistochemically in HL and to investigate their relation with clinicopathologic parameters and prognosis. The study included 40 classical Hodgkin.s lymphoma (CHL) (29 mixed cellularity (MC), 11 nodular sclerosis (NS)). Cox-2, Bcl-2 and LMP-1 antibodies were performed to tissue microarrays immunohistochemically. Clinical data of the cases were obtained from the medical files. In this study, the advance stage (stage III-IV) and male gender were significantly higher in MCCHL in contrast to NSCHL (p=0.031, p<0.001, respectively). Cox-2 were positive in 20 cases (50%) of CHL. Male gender had significantly more Cox-2 negativity (p=0.041). 81,8% of NSCHL and 37,9% of MCCHL were Cox-2 positive, which was significantly different (p=0.031). There was a negative correlation between Cox-2 expression and clinical stage(p=0.042, r=- 0.328). However, Cox-2 negative cases tend to be higher in advance stage, although it was not significant. Bcl-2 and LMP-1 were positive in 14 (35%) and 23 (57,5%) in CHL, respectively. LMP-1 positivity was significanly higher in MCCHL subtype (%75,9) (p<0.001). By Kaplan Meier log rank test, advance stage and Cox-2 negativity were found as poor prognostic parameters for survival (p=0,004, p=0,010, respectively). However, significant independent prognostic parameter was not found by Cox regression analysis. In conclusion, our study confirm the prognostic value of clinical stage in CHL.
