Baker kisti (BK) 1877'de Baker tarafından açıklanan fakat daha önce 1840'da Adams tarafından tarif edilen bir kisttir. BK'den kaynaklanan en sık hasta şikayetleri arasında aktivite ile artan şişlik, kitle, ağrı, sertlik sayılabilir. Dizin arkasında yürüme esnasında gerginlik hissi, kabarıklık veya müphem bir sızı olabilir. BK.lerin aspirasyonu, eş zamanlı kist içerisine steroid enjeksiyonu, baldır içerisine açılan kistlerin aspirasyonu, BK.lerin eksizyonu ve eğer mevcutsa eklem aralığıyla bağlantısının cerrahi olarak eksizyonu gibi tedavi yöntemleri tanımlanmıştır. Çalışmamızda radyolojik tanı yöntemleri ile BK tanısı almış, diğer klinik servislerden tedavi amaçlı gönderilmiş, semptomatik yakınmaları olan toplam 32 hasta alındı. Bireylerin klinik şikayetlerinin derecesini belirlemek amacıyla vizüel analog skala (VAS) kullanıldı. 32 hastada 32 BK.ya yönelik US eşliğinde aspirasyon ve 1 ml deksametazon enjeksiyon uygulaması yapıldı. Hastalar işlem sonrası 6. ayda klinik olarak ve US ile takip edildi. Hastaların 6. ay takiplerinde VAS skorlaması yenilendi. US ile değerlendirilen işlem öncesi BK hacimleri ile 6 ay sonraki BK hacimleri karşılaştırıldı ve BK hacimlerinde anlamlı düşme gözlendi. İşlemden önceki ve 6 ay sonraki VAS skorları karşılaştırıldığında skorlarda anlamlı düşme izlendi. BK.lerdeki hacim küçülmesi yanında hastaların klinik şikayetlerinde düzelme gözlendi. BK tanı, tedavi ve takibinde US'nin temel görüntüleme yöntemi olduğunu düşünmekteyiz. Popliteal bölgedeki diğer lezyonlarla ayırıcı tanıda, kistin aspirasyonu ve eş zamanlı kortikosteroid enjekte edilmesinde ve tedaviye yanıtın değerlendirilmesinde ucuz, tekrarlanabilir ve kolay ulaşılabilir olması nedeniyle US.nin BK yönetiminde özellikle tedavisinde önemli yer alacağı kanısındayız. A Baker cyst (BC), described by Baker in 1877, has since borne his name even though it had been described previously by Adams in 1840. The usual patient complaints resulting from BC are swelling, a mass, pain or stiffness, often aggravated by activity. There may be a bulge and tightness in the back of the knee on walking or vague posterior knee pain. There are numerous descriptions of aspiration of BC, concomitant steroid injection into the cyst, aspiration of calf dissections, excision of the cyst and its connection to the joint, if present. Thirty two patients with symptomatic BC which is diagnosed by Ultrasonography (US) were included in the study. To determine the grade of the symptoms, Visual Analogue Scale (VAS) was administered. Thirty two US-guided cyst aspirations and concomitant 1 ml dexamethasone injections were performed for thirty two cases. Patients were followed clinically and with US for 6 months after the procedures. VAS scores were repeated at 6th month of the follow-up. Volumes of BK which were evaluated by US before and 6 months after the procedure were compared and a significant decrease was observed. VAS scores before and 6 months after the procedure were compared and a significant decrease was observed. In addition to decreased volumes, clinically improvement was observed. We believe that, US is the basic imaging modality in the diagnosis, treatment and follow-up of BC. We think that US would take an important role in differential diagnosis with other pathologies around the knee, cyst aspiration, concomitant steroid injection and evaluation of respond to the treatment due to its cheapness, easy accessible and reproducible nature.
