Çalışmamızda cinsel şiddete uğradığı iddiası ile başvuran ve olay tarihinde 18 yaşını doldurmuş olan olguların sosyodemografik özellikleri, bedensel, zihinsel, ruhsal özgeçmişleri, saldırganla ve olayla ilişkili faktörler ile olay sonrasındaki muayene bulguları değerlendirilerek risk faktörlerini belirlemek, cinsel şiddeti önlemeye yönelik öneriler sunmak, bu alanda yapılan çalışmalara veri sağlamak amaçlanmıştır. Bu kapsamda 1 Ocak 2013 – 31 Aralık 2022 tarihleri arasında Süleyman Demirel Üniversitesi Tıp Fakültesi Adli Tıp Anabilim Dalına cinsel şiddet iddiası ile başvuran 294 erişkin olgu hakkında düzenlenen bilirkişi raporları incelenmiştir. Çalışmamız retrospektif bir çalışma olup istatistiksel karşılaştırmalarda Pearson ki-kare testi kullanılmıştır. Elde edilen bulgulara göre cinsel şiddet mağdurlarının çoğunluğunun kadın, bekar, 18-24 yaş grubunda, eğitim seviyesi lise ve üstü, çalışmayan bireylerden oluştuğu, olguların %51,4'ünde ekstragenital yaralanma bulgusu olmadığı, %23,8'inde ruhsal patoloji geliştiği belirlenmiştir. Erkeklerin kadınlara göre anlamlı derecede daha fazla bekar ve eğitim durumunun düşük olduğu, Eğitim seviyesi yüksek olan, olayda kolaylaştırıcı faktör bulunmayan, partneri tarafından psikolojik şiddet gören olgularda anlamlı derecede daha fazla ruhsal patoloji geliştiği, Adli bildirim süresi kısa olanlarda anlamlı derecede daha fazla penetrasyonun tıbbi kanıtlarının bulunduğu, saldırganın yabancı veya tanıdık olduğu olgularda adli bildirim süresinin anlamlı derecede daha kısa olduğu, Saldırganın partner/eski partner olduğu olgularda anlamlı düzeyde daha az ruhsal patoloji geliştiği ve mağdurların daha fazla fiziksel şiddet gördüğü saptanmıştır. Cinsel şiddet maruziyeti açısından daha fazla risk altında olan genç yaş grubunun bu konuyla ilgili toplumsal yanlış kabuller ve inançlar hakkında bilgilendirilmesi ve gençlere cinsel şiddet konusunda zorunlu eğitim verilmesi, suçu önleme ve suça maruz kalınması durumunda nasıl bir yol izleneceği açısından önemlidir. Cinsel şiddet olgularında ruhsal bulguların olayın aydınlatılmasındaki önemi düşünüldüğünde ayrıntılı ruhsal muayenenin yapılması şarttır. Kadınların cinsel şiddetten korunabilmeleri için sosyal ve ekonomik anlamda özgür olabilmeleri ve partnerlerinden gördükleri psikolojik şiddeti anlamlandırıp profesyonel yardım istemeleri için gerekli sosyal, ekonomi ve sağlık politikalarının niteliklerinin ve uygulanabilirliğinin artması sağlanmalıdır. This research entails a retrospective analysis of individuals who sought assistance from the Department of Forensic Medicine at Süleyman Demirel University Faculty of Medicine, alleging victimization of sexual assault between the years 2013 and 2022. The study explores the socio-demographic characteristics, physical, mental, and emotional histories of cases of victims aged 18 or above at the time of the incident. Additionally, it examines factors associated with the assailant and the incident, alongside post-incident examination findings. The primary objectives are the identification of risk factors, the formulation of recommendations for the prevention of sexual violence, and the contribution of empirical data to the existing body of literature in this domain. The research reviews expert reports from 294 adult cases of alleged sexual assault during the period from January 1, 2013, to December 31, 2022, taking place within the Süleyman Demirel University Faculty of Medicine's Department of Forensic Medicine. The methodological approach adopted is retrospective, employing the Pearson chi-square test for statistical comparisons. According to the findings, most victims of sexual assault were identified as female, single, aged between 18-24, with an educational background at the high school level or above, and predominantly unemployed. Notably, in 51.4% of cases, there was no evidence of extragenital injury, while 23.8% of victims exhibited signs of psychological pathology. Male victims were significantly more likely to be single and have a lower educational status than female victims. Cases involving more highly educated victims, without contributing factors, and experiencing psychological violence from a partner demonstrated a significantly higher incidence of psychological pathology. Furthermore, cases legally reported within a shorter time frame showed a significantly higher incidence of medical evidence of penetration. In instances where the assailant was a stranger or an acquaintance, the time to report the incident was significantly shorter. In cases where the assailant was identified as a current or former partner, a significantly lower incidence of psychological pathology was observed. Victims in these cases also experienced a higher degree of physical violence. It is crucial to inform younger age groups, which are at a higher risk of sexual assault, of the societal misconceptions around sexual violence. Providing mandatory education is essential not only for crime prevention, but also in educating victims on the appropriate course of action in case of victimization. Considering the significance of psychological indicators in clarifying incidents of sexual violence, a comprehensive psychological examination is required. Improvements in the quality and adoption of social, economic, and health policies would empower women socially and economically to enable them to comprehend psychological violence from their partners, and encourage them to seek professional help.
