YART'ın çok basamaklı planlama ve uygulama sürecinde hastalara doğru dozun verildiğini kontrol eden QA yöntemleri gerekmektedir. Lineer hızlandırıcıya entegre aSi elektronik portal EPG sistemlerinin öncelikli amacı hastanın tedavi pozisyonunun kontrolü iken, tedavi planının dozimetrik QA'de kullanımı ortaya atılmıştır. Tedavi öncesi EPG dozimetriyle YART QA yaygın kullanılmaktadır. Hasta tedavisi sırasında (in vivo) EPG dozimetrisiyle YART QA ise halen araştırılan bir konudur. Bu çalışmada Süleyman Demirel Üniversitesi Hastanesi Radyaysyon Onkolojisi Anabilim Dalı'nda bulunan Varian Clinac DHX LINAK'a entegre aSi1000 EPG sisteminin in vivo dozimetri için uygunluğunun araştırılması amaçlanmıştır. İlk olarak EPG sisteminin ölçüm tekrarlanabilirliği ve dedektörün doz yanıtı farklı fantom kalınlığında ölçümler alınarak incelendi. YART tedavi alanında EPID ölçümlerinin fantomlu, fantomsuz tekrarlanabilirliği ve değişen fantom kalınlığına göre EPID yanıtı gama analiziyle karşılaştırıldı. Değerlendirme sonunda EPID sisteminin yüksek tekrarlanabilirlik ve soğurucu kalınlık değişimlerine hassas yanıt verdiği görülmüştür. Homojen fantom içinde oluşan hava boşluğunun ve hava kavitesinin konum değişikliğinin farkılı gama kriterlerinde belirlenen uygunluk şartını sağlayıp sağlamadığı incelenmiştir. Ayrıca bu çalışmada fantom içinde bulunan hava boşluğundaki farklı yoğunlukta bir yapının konum değişiminin, QA fantomu konum değişimlerinin, LINAK gantri hareketi ve kolimatör açısı değişiminin çıkış dozuna etkisi %1-1mm, %2-2mm ve %3-3mm gama kriterlerine göre analizler yapılarak uygunluk şartının bozulduğu sınır değerler tespit edilmiştir. Hasta çıkış dozlarından radyoterapi tedavilerinin doğruluğu EPG dozimetriyle hızlı, kolay, tekrarlanabilir ve hastaya ek doz verilmesine gerek olmadan kullanılabilecek bir yöntemdir. In the multi-step planning and implementation process of IMRT, QA methods are required to check that the correct dose is given to the patients. While the primary purpose of the aSi electronic portal EPID systems integrated into the linear accelerator is the control of the patient's treatment position, the use of the treatment plan in dosimetric QA has been suggested. IMRT QA with pretreatment EPID dosimetry is widely used. IMRT QA with EPID dosimetry during patient treatment (in vivo) is still under investigation. In this study, it was aimed to investigate the suitability of the Varian Clinac DHX LINAK integrated aSi1000 EPG system for in vivo dosimetry in the Radiation Oncology Department of Süleyman Demirel University Hospital. First, the measurement repeatability of the EPG system and the dose response of the detector were examined by taking measurements at different phantom thicknesses. Phantom and non-phantom reproducibility of EPID measurements in the IMRT treatment field and EPID response according to varying phantom thickness were compared with gamma analysis. At the end of the evaluation, it was seen that the EPID system had high repeatability and sensitive response to absorber thickness changes. It has been investigated whether the position change of the air gap and air cavity formed in the homogeneous phantom meets the conformity condition determined in different gamma criteria. In addition, in this study, the effect of the position change of a structure with different density in the air space inside the phantom, the position changes of the QA phantom, the linac gantry movement and the change of the collimator angle on the output dose were analyzed according to the criteria of %1 - 1 mm, %2 - 2 mm and %3 - 3mm gamma. The cut-off values that does not meet the gamma analysis criteria have been determined. The accuracy of radiotherapy treatments from patient exit doses is a fast, easy, repeatable method that can be used without the need for additional doses to the patient with EPG dosimetry.
