In this study, we enrolled 118 endoscopic specimens which were send between 1997-1998 to the department of pathology (Süleyman Demirel University, School of Medicine-Isparta Turkey) for histopathological evaluation. The relationship between gastric lesions, Helicobacter pylori (HP) and the serum levels of IgG, IgM and IgA were studied. The mean age of the patients were 48 years and majority of the cases were in the age group 40-49 years (27.1%). HP was detected in 77.1 % of the cases in the antrum and in 51.7 % In the corpus. The severity of colonization was also higherin the antrum when compared to the corpus. Among all of the specimens, inflammation was detected in 107 (90.7 %) specimens in the antrum and in 78 (66.1 %) specimens in the corpus. The severity of inflammation was higher in the antrum when compared to the corpus. For both regions, the relation between age and inflammation and the relation between HP colonization and inflammation was found to be significant statistically. Atrophy was detected in 14 (11.9 %) specimens in the antrum and in 2 ( 1.7 %) specimens in the corpus. HP was positive in 13 (11.01 %) and negative in 3 (2.54 %) specimens. Intetinal metaplasia (IM) was detected in 3 (2.54 %) specimens in the antrum and in 5 (4.2 %) specimens in the corpus. it was determined that the number of the HP (+) IM were 8 (6.77%) and the number of the HP (-) IM were 4 (3.38 %).The correlation between HP colonization and IM was found to be statistically significant in the corpus. In respect to severity and ratio, activation was found to be higher in the antrum. The correlation between HP colonization and activation was fomıd to be significant in both regions. Rapid urease test results were negative in 30 (25.4 %) specimens and positive in 88 (74.6 %) specimens. The sensitivity and specifity of the test was determined as 86.74 % and 73.68 %, respectively. 'There was a statistically significant relation between HP colonization and activation with rapid urease test for the antrum. Ig G was detected positive in 91 (77.1 %) cases. While there was a sitatistically significant relation between Ig G and HP colonization in the antrum, this relation was not detected in corpus. The significant relation which have been detected between inflammation and Ig G levels in the Corpus and antrum were also detected in the antrum between activation and Ig G. The sensitivity and specifity of Ig G were detected as 90.9 % and 84.21 % respectively. Serum levels of Ig M were detected negative in 71 (60.2 %) and were positive in 30 (25.4 %) oftbe specimens. However there was no significant relation between Ig M level with inflammation and activation in the antrum and corpus. There was a significant relation between the severity of HP colonization and serum levels of Ig M in the antrum by means of qualitativity. The sensitivity and specifity ratio of Ig M were detected as 23.91 % and 97.2 %, respectively. Serum levels of Ig A were detected negative in 93 (78.8 %) and were positive in 25 (21.2 %) of dıe cases and there were no significant relatian between Ig A and the other lesions. According to the results there is no sensitivity of determining the serum levels of Ig A in the diagnosis of HP . but it had 92 % specifity. In conclusion, there was no relation between serum levels of HP spesific antibodies and the severity of the gastric lesions, on the other hand serological studies are simple useful in supporting the diagnosis of HP. |
